WHY SHOULD MY CLIENTS CONSIDER MAKING AN ADVANCE PAYMENT WITH GERBER LIFE? We created this option, by request, to meet the needs of clients who prefer to pay for a Gerber Life Grow-Up policy through a single advanced payment. 1 It provides your clients with simplicity and convenience and is best suited to individuals who have the funds to make a one-time payment and would like to give the gift of life insurance protection right away. HOW DOES THE ADVANCE PAYMENT WORK? By making a single payment, the policy will be paid, in advance, until the child reaches the age covered by the payment (the minimum is 5 years of premiums and the maximum is to age 121). If the payment covers premiums until the child is age 121, the child will not have to make premium payments. However, if the policy is not paid to age 121, the child will be responsible for future payments after the Pay to Age selected. The child becomes the legal owner of the policy at age 21. The Advance Payment goes into a separate, non-interest bearing account, to be used to pay the annual premium on the coverage purchased up until the age selected, when the payment is fully used. IS THERE ANY CHANCE OF A CHILD BEING DECLINED FOR COVERAGE? Any healthy child, from 14 days to 14 years old, is eligible, however, coverage depends on answers to health questions based on the child s age. IF MY CLIENTS ARE GRANDPARENTS, CAN THEY PAY FOR POLICIES FOR THEIR GRANDCHILDREN? Yes. Grandparents can purchase life insurance for their grandchildren. If multiple policies are purchased for the same grandchild, the maximum amount that can be purchased, for all policies combined, is 50,000. WILL MY CLIENT NEED TO OBTAIN THE CHILD S OR GRANDCHILD S MEDICAL RECORDS AND IS A PHYSICAL EXAM REQUIRED? There is no physical exam, and medical records/health statements are only required for special health situations. HOW CAN I OBTAIN A QUOTE FOR THE ADVANCE PAYMENT GROW-UP PLAN? You can run a quote on the Gerber Life Agent Portal or on our mobile app, Agent Zone, by clicking the Grow-Up Plan on the home page. There are two choices to quote the Advance Payment Grow-Up Plan: Lump Sum Amount Use this option if a client would like to see how much coverage can be purchased by targeting a specific advance payment amount. Face Amount Use this option to see the advance payments for three standard face amount choices. You can also customize the quote if a client wants to see the advance payment for another face amount not shown. 1 The single advanced payment does not create a Modified Endowment Contract. Gerber Life Advance Payment Grow-Up Plan 2 EXECUTING A RATE QUOTE THROUGH GERBER LIFE S AGENT PORTAL Refer to the step-by-step instructions below to execute an Advance Payment Grow-Up Plan rate quote through Gerber Life s Agent Portal. HELP ICONS Question marks appear by certain fields which provide explanations for fields that are either new or have been modified for the Advance Payment Grow-Up Plan. Click on any question mark and a corresponding explanation for the field will appear. STEP 1: STATE AND QUOTE FIELDS Select the applicant s state of residence in the State field. For the Quote field, select one of the two Advance Payment options: Lump Sum or Face Amount. Note that the Rates and Pay fields will be greyed out since they do not apply to the Advance Payment Grow-Up Plan. STEP 2: INSURED INFORMATION AND PAY TO AGE FIELD Enter information about the insured child, including: First and Last Name, Date of Birth, and whether the child is a Male or Female. In the Pay to Age field, enter an age for the child, (the minimum is the current age plus 5 years of premiums and the maximum is to age 121). The policy will be paid, in advance, until the child reaches the age covered by the payment. If the policy is not paid to age 121, the child will be responsible for payments after the Pay to Age selected. At age 21, the child becomes the legal owner of the policy. STEP 3: GET QUOTE ADVANCE PAY LUMP SUM In the Get Quote field, enter an amount your client would like to pay, in advance, as a total lump sum for all children. Be sure the lump sum amount you enter is within the range the system indicates. Note: For Advance Pay Face Amount, the Get Quote field will not appear. Instead, once all the necessary fields are completed as outlined above, a rate table will automatically appear as indicated in Step 4.

Gerber Life Advance Payment Grow-Up Plan 3 STEP 4: RATE QUOTES ADVANCE PAY LUMP SUM A rate quote table will appear that shows how the total requested payment amount is divided among multiple children with the face amount being equal for all children. The information in the table can be used to populate the sales proposal. The payment amount calculated could be slightly under the client s desired total amount in order to achieve equal face amounts for all children. You can save the quote by clicking either the Save Quote link or the Proceed to Application button. RATE QUOTES ADVANCE PAY FACE AMOUNT A rate quote table will appear that shows the advance payments for three standard face amount options. If your client wants to see another face amount from what is shown, you have the ability to enter the face amount in the far right column (from 10K - 50K). The information in the table for the option selected can be used to populate the sales proposal. You can save the quote by clicking either the Save Quote link or the Proceed to Application button. STEP 5: APPLICATION PAGE If you choose to submit an e-application, click the Proceed to Application button. The rate quote will be summarized on the application page and will display the coverage amount, total advance payment amount and the Pay to Age (the age the policy is paid through). A few additional pieces of information are required to complete the application. Once complete, have your client sign the application via e-signature and submit the application electronically. If the payment amount exceeds the allowable maximum for a debit/credit card payment (10,000 max.) and for a one-time ACH draft (25,000 max.), then you will need to print and mail the application and include a check or Money Order for the total amount. You can also make changes to the quote if needed by clicking Edit Quote at the top of the application page. Clicking this link will take you back to the quote page to re-quote the advance payment (for example, if you need to change the DOB or gender) or to add children to the application. Any data you entered on the application page will be saved if you need to go back to the quote page to make changes.

Gerber Life Advance Payment Grow-Up Plan 4 ADVANCE PAYMENT GROW-UP PLAN SALES PROPOSAL Once you ve selected a quote option, you can save it and have it populate into the Advance Payment Grow-Up Plan sales proposal which you can email to your client. The format of the proposal is illustrated below. COVER PAGE The cover page will be personalized with your name, address, and contact information, the client s name, and the date. SALES PROPOSAL RATE QUOTE The sales proposal includes a rate table. The two versions below show how the rate table can expand from a single quote for one child to a quote for up to six children. The table includes the name, age, and date of birth of the insured child, the face amount, Pay to Age, and total cost.
