EVENT APPLICATION FORM The Rottnest Island Authority, upon receipt of a completed application, reserves the right not to issue a permit for any reason. Any information regarding the availability or costs of venues given prior to the issue of a permit is an indication only and is not an assurance that a permit will be issued. The Rottnest Island Authority shall only issue a Permit when all relevant documents and payments are received by the Authority within the stated time frame (ALL APPLICANTS MUST BE OVER 18 YEARS OF AGE). 15. Will participants, officials and/or organisers require accommodation on the Island? 16. Will you require bookings for accommodation to be made outside normal booking procedures? Approval may be issued subject to compliance with set conditions, payment of service fees and approval from the Department of Health. Please contact the Authority s Event Department on 94320300 for assistance. 20. Will you have any form of amplified music at the event/function i.e. bands, juke box etc? If Yes, all applicants must abide by the Environmental Protection Act 1997 and associated Environmental Protection (Noise) Regulations 1986 in addition to The Rottnest Island Authority Accommodation Terms & Conditions. Details available http://www.legislation.act.gov.au 21. Do you intend to bring any equipment or goods onto the Island? If Yes, provide a detailed inventory. 22. Do you intend to store any goods or equipment on site for the purposes of the event? If, you will have to make a formal request indicating specific needs 23. Will you require assistance for the transport of goods or equipment? 24. Will you require assistance for the transport of people while on the Island? 25. Will you or any of your contractors or volunteers or staff need to drive on the Island? If you answered Yes to any of questions 23, 24, 25 you will be required to submit a Transport and/or Permit to Drive Request form. This may incur a permit or hire fee. Please see the schedule of fees and charges for more information. 26. Will you require road/path closures for the event? If, please attach your proposed traffic management plan?

27. Do you plan to erect any temporary structures i.e. marquees / stage? If, please include a schedule for the set up and pack down. In addition you will be required to complete a Form 1:Application to construct, extend or vary a Public Building. Please note that Marquees can only be erected in approved sites and must be weighted rather than spiked. Approval is not automatic. (See web site for details). A License fee will apply as per the Authority s schedule of fees. 28. Will you be utilising a generator for power? If, You will need to have a qualified Electrician to sign off on the Form 5: Certificate of Electrical Compliance after installation but before the commencement of the event. Please note that external power can be accessed from the Islands reserve sites. Additional toilets (compulsory for larger events) must be arranged privately at the applicant s expense. Please contact the Authority s Events Department prior to the event for guidance on the number of ablutions required. 29. Have you provided a current Emergency Evacuation and Risk Management Plan with this application? Your in-depth Risk Management and Emergency Response Plan must include but not be limited to; the number of First Aid posts and staff, security plans, site / route plans, evacuation sites, and contact personnel details, etc. The Authority s preferred risk management plan format can be down loaded from our web site. 30. Are you hiring a company or individual to provide services on the Island other than Island based organisations? If Yes, please complete and attach the Supplier Details form and include a copy of their Certificate of Currency. 31. Will you be conducting any commercial photography or filming? If yes, please complete a Commercial Filming/Photography Permit Application Please note that consumption of Liquor outside of licensed premises is not allowed and fines do apply. If you answered, please note that the consumption of alcohol outside of Licensed premises or accommodation is a breach of State Law and Rottnest Island Regulations. Limited 2 hour alcohol permits for the consumption of alcohol may be awarded in certain circumstances. If you wish to apply for a limited 2 hour alcohol permit you must complete the Application for the Consumption of Alcohol. Please note under no circumstances will The Rottnest Island Authority give approval for the selling of alcohol.

FUNCTION COMPLIANCE FORM Groups or organisations do not have exclusive use of any area or facility, unless otherwise specified; The use of confetti, rice, rose petals and balloons are prohibited. Organic decorations other than cut flowers (e.g. goldfish, potted plants) require the written approval of the Rottnest Island Authority; The consumption of alcohol is not allowed outside of licensed areas (Hotel Rottnest, Rottnest Lodge, Aristos Waterfront Rottnest) or domestic premises; Rottnest Island Authority Regulation 72 - Places where liquor may be consumed (1) Consuming alcoholic liquor in unauthorised place. Maximum Penalty $1000. Section 119 under the Liquor Control Act It is an offence in Western Australia for persons of any age to drink in public, such as on the street, park or beach. On the spot fine of $200. The beach area is to be left clean and tidy after your function dependant on the size of your event you may be required to provide extra bins at your own cost. Structures such as shade structures are not to exceed 3m x 3m in size and must be weighted not spiked; Regulation - 52. Erection of structures - (1) A person shall not without permission erect any structure within the limits of the Island. Maximum Penalty: $1 000; Boats are permitted to land/ beach for the purpose of dropping off equipment, but they are asked to abide by the no anchoring in sea grass requirement and providing they do not anchor within the Ferry approach channel; For your own safety, participants should stay clear of limestone cliff edges, caves and overhanging rocks; This compliance form is to be completed and returned prior to the function; You may be required to post an Environmental Preservation Bond of TBC. The Bond will be fully refunded on a satisfactory inspection of the venue. I, the permit applicant and hereby declare that I have read and fully understand this Permit Application and Function Compliance Form provided by the Rottnest Island Authority, pertaining to hold a function on Rottnest Island. I agree to abide by these conditions and accept all associated requirements of these conditions. I declare that I have disclosed all relevant information pertaining to this application and acknowledge any consequences of failing to do so including loss of Bonds and potential prosecution.
