The life of your policy Policy set up and administration Annual Renewal Review Once we have received your membership data we will send you a statement of accounts and policy schedule, which give you more detail on the ins and outs of the policy you have taken out with us. Payment for the first year is needed by the start date agreed. Congratulations! You now have a back-up plan in place. Each year before your policy renews we will ask you to provide us with updated membership data so that we can ensure your employees remain covered. You will also make your next payment unless you have monthly billing. At this point our underwriters will look at your scheme again. A number of factors will be assessed to ensure that your premium is set at the correct level. Cover start date Renewal (1 year) Review (3 years) 4

Claims Management Every claimant and employer is unique, which is why we ll work with you to develop a customised plan to meet your needs. You will benefit from: Your own dedicated Claims Management Specialist (CMS) who will guide you through the claim process and keep everyone in the loop An in-house team of experts, including medical and vocational professionals, who ensure we deliver the right support for each situation Our Round Table gets all of our experts in one room to ensure a complete understanding of each claim - Claims Management Specialists, an Insurance Nurse Specialist, a Vocational Rehabilitation Consultant, and a Doctor. 6

Rehabilitation Services Return to work support from day 1 Our Vocational Rehabilitation Consultants (VRCs) have backgrounds in a number of disciplines, including occupational therapy, psychology, nursing and disability management that allow us to effectively support your employees back to work. A simple phone call to Unum can provide practical guidance, designed to help you navigate the difficulties of sickness absence. Our team of experienced professionals specialise in tackling the issues you may be puzzling over. In the event of a claim We hope that your employees remain healthy and in work but in the event of a claim we have made our process simple and straightforward. What we need from you: You can download all the forms you need to fill in for your claim from the table below. Alternatively, contact details for our Claims Department can be found on page 13 of this booklet. You can find a detailed timeline and more about when to submit a claim here. Medical Underwriting ensuring the right level of cover With time a precious commodity, we ve made our medical underwriting process as simple as possible. No need for routine medical evidence Telephone and face-to-face underwriting options as an alternative to our paper application form No repeat underwriting requirements for salary-related changes for schemes It is important to check that those who need to be medically underwritten have been, so that they are fully protected if the unexpected happens. Innovative underwriting for a modern workforce By challenging the traditional medical underwriting model and keeping up-to-date with the latest medical advances, we are often able to provide cover for people with pre-existing conditions such as heart attack, cancer, paraplegia, back pain and diabetes. We also offer cover up to age 70 and can consider protection for: Second heart attacks Stable angina, or Following heart valve surgery Download a telephone interview application form Download Download a written application form benefits of being medically underwritten (employee letter template) 10

Serious Illness Benefit As a valued customer introduced to us by JLT, you already have the reassurance that your Income Protection policy will give you and your employees access to a wide range of support tools and practical help. Income Protection also comes with Serious Illness Benefit at no extra cost. With Serious Illness Benefit, we will pay a single benefit payment of 5,000 after only two weeks of sickness absence caused by a heart attack, a stroke or cancer - even if the employee returns to work before the end of the policy deferred period. Whether it s to pay for equipment, help with transport to and from medical appointments, or practical care in the home, a quick injection of cash could help ease the immediate financial concerns that can go hand-in-hand with serious illness. A few things to note: The heart attack, stroke or cancer must be of a certain severity to qualify for benefit payment. We have detailed the qualifying criteria in your policy supplement Benefit is not payable if the employee has previously had a heart attack, stroke or cancer Benefit is not payable if the employee has a related medical condition that may lead to the event that has caused the absence. Again, details of the related conditions that can affect benefit payment are set out in your policy supplement Only one payment will be made for each employee that meets the criteria while we are providing your Income Protection cover Typically, benefit is paid to the employee through your payroll system. We have assumed that it is subject to normal PAYE income taxation and National Insurance contributions - the same as the regular monthly benefit payments we make Benefit is not payable if an employee dies before being absent from work for two weeks 11

Glossary of terms Claim (otherwise known as loss) Deferred period Exclusions Financial Ombudsman Service When you apply to your insurance company for a payment under the insurance policy you hold. The time between when your employee goes off sick and the date they start receiving insurance benefits. For example, if your employee became ill and could not work from January, and your Income Protection policy had a 6-month deferred period, your policy would start paying out in July if their illness continued beyond those 6 months. Things your insurance won t cover. An independent organisation which deals with disputes between consumers and financial businesses, like banks or insurers. Free cover limit Material fact Non - disclosure On risk Policy Policyholder Premium Risk Schedule The maximum amount of benefit an insurance company will insure members to without medically underwriting them. This is anything that an insurer might take account of when deciding whether to provide cover or deciding on the premium and terms and conditions of the policy. For example, this could include information on a medical condition that you already have when you apply for medical insurance, or for Income Protection. You have to tell the insurance company about any material facts when applying for insurance. When you fail to tell your insurer about a material fact - such as a pre-existing medical condition - before the insurer assesses your application. This is when your policy has started and you have a back-up plan in place. The agreement you have with your insurer - what you re covered for, what it costs, and the all-important small print. You, the company who is insured. Sometimes known as the insured. A policyholder may also take out cover on the life of another, in which case the policyholder and insured are different people. The amount you pay for your insurance. The danger insured against, or the probability of the danger happening which the insurer is willing to insure against. The detail of what a policy does and doesn t cover. 13
