TENDER FORM HIRING OF STAFF CAR FOR ESIC MODEL HOSPITAL BAPUNAGAR, AHMEDABAD Last Date & Time of receipt of tender: - 11/06 /2013 up to 02:00 PM Date & Time of opening of tenders: - 11/06 /2013 at 02:30 PM Duly sealed and signed tenders under two bid system are invited from reputed, registered and experienced parties for hiring of STAFF CAR under the following terms and conditions for 08 HOURS in a DAY for all working days in a MONTH and as and when required:- 1. Specifications of the vehicle should be as under:- (a) Air Conditioned Any Brand Car with 4+1 Capacity, (b) The vehicle must have a commercial permit, (c) The Vehicle must be in very good condition and of latest model i.e. 2013. 2. The Staff Car service contract will be for a period of three years, however, the Medical Superintendent has the right to discontinue the contract without assigning reasons thereof. The contract can also be extended for further period on mutual understanding. 3. The rates shall be fixed for three years and no interim hike will be accepted/ allowed whatever the reasons. 4. The rates shall be fixed inclusive of vehicle charges, wages of drivers, fuel charges, servicing and repairing charges, taxes and insurance etc. and no extra charges shall be paid by this hospital. It is the responsibility of contractor to renew the documents where required for tenure of contract. 5. The Driver must have a valid driving license and shall wear valid uniform and bear an identity card. His behavior with the officials must be polite, courteous and helpful. 6. The agency shall not engage the drivers beyond the permitted weekly hours by the rules. 7. The agency shall maintain one logbook as per the proforma approved by the hospital and produce the same along with the bills and also produce the records for verification as and when required. 8. The agency solemnly shall be responsible for any mis-happenings / unforeseen situation & claims. 9. No tender will be accepted after the last date of acceptance of tender given in the advertisement. 10. Tender form shall be accompanied with Earnest Money Deposit of Rs. 18,000/- (Rupees Eighteen Thousand only) payable through Demand Draft / Banker's Cheque drawn on any scheduled bank in favour of ESIC Fund Account No. 1 payable at Ahmedabad. Tender not accompanied with Earnest money deposit shall be rejected outrightly. Earnest Money Deposit of unsuccessful bidders will be returned after award of the contract to successful bidder. 11. The Successful bidder shall have to pay Security Deposit of an amount of Rs. 45,000/- (Rupees Forty FiveThousand only) payable through Demand Draft drawn on any scheduled bank in favour of ESIC Fund Account No.1 payable at Ahmedabad. No interest will be
payable on Security Deposit. The Security deposit will be refunded after satisfactory completion of work. 12. The contract may be terminated by Medical Superintendent ESIC Model Hospital, Bapunagar, Ahmedabad after giving one month's notice. In case contractor leaves the contract in between currency of the Agreement, Security Deposit will be fortified. 13. The Medical Superintendent reserves the right to accept or reject any tender without assigning any reason for the same. 14. The Successful Bidder shall be required to sign an Agreement on a non-judicial stamp paper worth Rs. 100/- to be purchased by bidder. 15. The tender form must be accompanied by following documents:- (a) Photocopy of R.C.Book (In case vehicle is already purchased), (b) Photocopy of valid Insurance Policy (In case vehicle is already purchased), (c)photocopy of Sales Tax/VAT/Service Tax Registration No. Certificate (d) Photocopy of PAN card of Firm/Proprietor/ Managing Partner/ Managing Director, (e) Photocopy of Income Tax Returns for the last year i.e. 2011-12, (f) Experience cum Satisfactory Performance Certificate from clients where he/she had provided Staff Car service. 16. The tenderer fill the following format in tender form without fail. (1) Undertaking as per Annexure-I (2) Checklist as per Annexure-II Technical Bid without above mentioned documents will not be evaluated as per Sr. No. 15 & 16. 17. The rates should be quoted in figures as well as in word, covered with transparent tape. The rate quoted in words would be considered for price comparative. 18. The tenderer shall not give contract to any other party by sale or subcontract in any circumstances, after agreement of contract. 19. Conditional Tenders shall not be accepted. 20. Bids should be submitted in two separate envelopes namely, 'Technical Bid' and 'Financial Bid'.The technical and financial bids duly sealed envelope and be kept inside a single third envelope which shall clearly superscribed as Tender for hiring of Staff Car for ESIC Model Hospital Bapunagar, Ahmedabad. The envelope should be clearly marked as 'Technical Bid' (Envelope-I) and 'Financial Bid' (Envelope-II). Earnest Money Deposit (EMD) along with covering letter should be kept in 'Technical Bid Envelope'. The completely filled Tender should be dropped in the Tender Box kept in the office of the Dy. Medical Superintendent at 2 nd floor of ESIC Model Hospital Bapunagar, Ahmedabad by the stipulated date and time. Tender submitted or received after the closing date and time will not be considered. 21. The Tender shall be opened at 02:30 PM on 11/06/2013 at ESIC Model Hospital Bapunagar, Ahmedabad in the presence of the authorized representatives of the tenderer, who wish to be present at that time. 22. All the Technical Bids will be scrutinized, by the Technical Evaluation Committee constituted by the Medical Superintendent to check all relevant documents for their authenticity and the Tenderer whose Technical tenders are accepted will be informed about the date and time for opening the Financial Bids. 23. No Tenderer will be allowed to withdraw after submission of the tenders within the bid
validity period of 60 days from date of opening of tender otherwise the EMD submitted by tenderer would stand Forfeited. 24. In case the successful Tenderer declines the offer of Contract, for whatsoever reason(s), his EMD will be forfeited. 25. This agreement shall be subject to Ahmedabad jurisdiction only. 26. Vehicle has to report at 08:30 AM to General Branch and perform duties until release. As and when require after duty hours on any day as called from authority contractor is supposed to provide the services. 27. Late coming penalty will be charged of Rs.100/- per hour. 28. Penalty for Absence of Staff Car on any working day will be Rs. 1000/- (One Thousand Only) per day and it may result the termination of contract also. 29. Vehicle must be kept clean & hygienic at all the times. 30. Monthly Bill will be produced in the first week of following month alongwith Log Book duly countersigned by the officials who performed the journeys. Mode of payment will be ECS/RTGS on monthly basis subject to satisfactory services. Medical Superintendent
