And after we pay, you generally pay any difference between the Plan allowance and the billed amount if you use an Out-of-Network physician or other health care professional. 3

Programs to support your needs Aexcel Designated Providers Aexcel designations are for high-performing specialty physicians in 12 medical specialty areas, including: Cardiology Neurology Orthopedics Urology Obstetrics and gynecology and more Aetna evaluates these providers using specific standards for volume, clinical performance, and efficiency. Visit fsbphealth.com, select Find a Provider, and look for the blue star «next to the provider s name. Advanced Illness (AI) Program AI helps improve the quality of life and reduce the costs for those with advanced and/or terminal illness. It provides resources and information on living wills, advance directives, and tips on how to talk about these issues with loved ones. AI is voluntary and free. To learn more, call 800-923-2220. AbleTo Support Program AbleTo offers a web-based video conferencing personalized 8-week support program. It addresses the emotional and behavioral needs of those with conditions like heart disease, type 2 diabetes, or life events such as losing a loved one or having a baby. Members work with the same therapist and coach each week to set goals toward healthier lifestyle changes. To enroll or learn more about AbleTo, call 866-287-1802 or visit AbleTo.com/enroll. Livongo Livongo is a remote diabetes monitoring program. It offers diabetic supplies and real-time interventions by Certified Diabetes Educators. This voluntary program provides a cellular enabled In Touch glucose meter, a mobile app, and unlimited test strips and lancets. Participants have access to personalized interventions to assist with their diabetes management. To learn more about Livongo, contact 800-945-4355. Smart90 Smart90 requires U.S. based members to obtain non-speciality maintenance medications either through Home Delivery or through a participating retail network pharmacy. It adds another venue for obtaining a 90-day supply of covered medications through participating Smart90 Retail Network pharmacies. SafeGuardRx The SafeGuardRx program addresses targeted therapies for specific chronic conditions such as high cholesterol, diabetes, and Asthma/COPD. Members receive appropriate therapy and specialized care, which may include prior authorization, step therapy, and quantity limits. Some medications may be preferred based on FDA indications. Non-FEHB Programs offered by the American Foreign Service Protective Association (AFSPA), sponsor of FSBP EyeMed Vision Care Program Plan members save up to 35% with your EyeMed Vision Care discount program on exams, glasses, and contact lenses. Save 15% off standard prices for LASIK services obtained through the U.S. Laser Network. Jenny Craig Discount Plan members can receive a three month program plus $50 in food savings or save 50% off premium programs. Simple Steps to Living Well Together Program The FOREIGN SERVICE BENEFIT PLAN designed a wellness program, Simple Steps to Living Well Together, to help you identify your health risks and achieve your health goals. The program includes three simple steps and you can earn rewards along the way. Step 1 The Health Risk Assessment Reward: $100 Wellness Incentive Fund Credit A Health Risk Assessment (HRA) is like an online interview. It asks questions like those asked during an annual checkup. You share information about your health habits and history. Then, you receive a personalized health summary to help you understand your health needs. Take your Health Risk Assessment 1. Visit www.afspa.org/fsbp and select the Helpful Links tab 2. Select Aetna Navigator and log on or register 3. Look for the Stay Healthy icon and proceed 4. Complete your free assessment 5. Or, call 866-237-1442 to complete the HRA by phone Step 2 The Biometric Screening Reward: $100 Wellness Incentive Fund Credit A Biometric Screening includes measurements of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness. It serves as a benchmark and can help evaluate changes in health status over time. Obtain a Biometric Screening at a Quest Diagnostics Patient Service Center (PSC), or from your physician. After completing Step 2, you will receive your results and a personalized action plan to help you better understand your health risks. Register for your Biometric Screening 1. Visit my.questforhealth.com and enter the registration key FSBP 2. Locate a PSC location 3. Or, print a copy of the Biometric Screening Physician Results Form to take to your physician 4. Call 855-623-9355 to register by phone Wellness Incentive Fund Account credits help reimburse you for Eligible Medical Expenses like your deductible, coinsurance, and copayments, as defined by IRS Code Section 123(d). You can obtain each reward once per calendar year. You must complete the Health Risk Assessment (Step 1) to be eligible to complete a Biometric Screening (Step 2). But you can perform a Healthy Action at any time, independent of the other two steps! Completing your HRA annually is an important first step to guiding your personal health goals. Step 3 A Healthy Action Reward: $50 Wellness Incentive Fund Credit A Healthy Action is obtaining a routine physical exam or participating in any of the programs, described on page 6. Choose the Healthy Action that fits your individual needs Routine Preventive Examination Obtain a routine preventive examination through an in-network provider or provider outside the 50 United States at no cost to you. Digital Coach Programs We offer nine programs designed to coach you through the unique health challenges you face and goals you create. weight management nutrition blood pressure tobacco use exercise depression management stress management cholesterol management sleep improvement Health Coaching Progam Mediterranean Wellness Program Healthy Pregnancy Program Dietary and Nutritional Counseling Diabetic Education Disease Management Programs Cancer Support Programs TherapEase Cuisine Program Virtual Lifestyle Management Control Blood Pressure Control A1c Get your Prenatal Care Sets you up with a one-on-one health coach to help you meet your health and wellness goals. An 8-week interactive online program with access to a registered dietitian, as well as a support manual full of information to help you manage your weight. Provides support and educational materials during healthy and high-risk pregnancies. Participate in individual and group behavioral counseling. Speak with a professional to assess your dietary habits and health needs. Access to educational materials and support to help you manage and control diabetes. For nurse support and education about chronic conditions, including asthma, diabetes, chronic kidney disease, and others. Gives you the support and educational materials you need when facing cancer. Nutritional information for cancer patients based on guidelines from the Academy of Nutrition and Dietetics. Access to online self-management education and tools and the involvement of a trained coach to assist you with nutrition and weight management. If you have documented high blood pressure, you can earn a reward for keeping it under control. We will send you a form that your doctor can complete to demonstrate good control of your blood pressure. Just return the form for your reward. Members with documented diabetes can earn a reward for keeping their A1c level below 8%. Ask your doctor to send us your A1c lab results. If your A1c is below 8%, you will earn the reward. If you are pregnant, make sure to have a prenatal care visit during the first trimester. Send us a copy of the prenatal care medical report or Obstetric Panel report, and you will earn the reward. All these Healthy Actions are available to you, but you can earn only one $50 Wellness Incentive Fund credit. Your $50 credit can reimburse you for your deductible, coinsurance, and copayments, as defined by IRS Code Section 123(d). Refer to Section 5(h) of your Brochure, for more details. 6
