Good News for 2018!! There s no increase in monthly premiums! For the first time in 17 years, premiums will not be increasing in 2018, unless you make an enrollment change for who you cover. It s important to know that retirees in the plan contribute only 30% of the cost of the premiums, while your former NAF-employer picks up the remaining 70%. Also exciting for 2018 - The Health Incentive Credit (HIC) Amount for the Biometric Screening is now $150 all year long (through November 30 th ) for covered retiree and spouses! Register with Quest (on www.nafhealthplans.com under Wellness on the far left side of the home page. Click on Biometric Screening/Metabolic Syndrome. Quest registration will further down on this webpage) Provider forms can be for any lab results from an10/1/2017-11/30/2018. Provider form must be signed by your provider. 2

Good News for 2018!! (cont.) Nov 6 th Dec 1 st is the time to drop dependents or coverage or switch plans (pre65 where available. Pre65 retirees can contact your HR to see if there is an additional plan in your area. The www.nafhealthplans.com website has been updated to include: a video library for educational videos Health Benefits 101, a straightforward educational guide on plans, payments and ways to save! (Overseas version coming soon! a pop up reminder has been added to encourage retirees to opt in to receive enews on helpful benefit information. 3

2018 Health Incentive Credits The maximum an individual can earn is $250, the family maximum is $600 To earn credits, after the health assessment, you must complete healthy actions as shown below: Activity Health Incentive Credit Amount Employees and Covered Spouses Complete metabolic syndrome screening through November 30 Register with Quest and print provider form to complete and submit Complete online Journey (average time 32 days) $150 each $50 each up to 4 journeys Dependent Children Under Age 18 Have a preventive care exam $50 for each child per year 4

Health Incentive Credit Retirees and their covered spouse must complete the Health Assessment in order for any of the Health Incentive Credits (HICs) earned to be deposited. No activities will have credits deposited until the assessment is completed. To take the assessment, log in to www.aetna.com and go to Stay Healthy > Complete Your Assessment on the home page. For more information on how to take the health assessment and view incentive dollars, please visit www.nafhealthplans.com>wellness>health Incentive Credit. 5

Log in to your secure member website at www.aetna.com. New to the site? Just click Register. Aetna.com Log in or Register if you are new to the site.

Under the Stay Healthy section click on Complete your Assessment. You can easily finish in one sitting. But you can always come back later. We ll save your answers. We ll ask you about things you do every day, like eating and sleeping. And have you tell us about any medical conditions you may have. You ll also see questions about your modifiable health risks. These are health habits you can change. For example, your eating and exercise habits and whether you use tobacco products. Tip: Try to have your latest numbers from any screening tests or a physical exam on hand.

To see incentive dollars earned, click on See Coverage & Costs from your Navigator home screen. Then click Medical balances & limits from the Financial Overview section.

At the top of the Plan Balances and Limits screen you will see how much was rolled over from previous year(s), how much you ve earned, how much you have used for medical expenses and have left for this year in incentive dollars. To see a listing of available Incentive Dollars and if you have completed, click on Activity Details.

You can select the incentive you are participating in from the Active Incentive Programs section to see details. This will show you program start and end dates as well as your progress. Each year, DoD NAF employees, retirees and their families have the opportunity to earn Health Incentive Credits for taking healthy actions. Those with individual coverage can earn up to $250 and those who cover dependents can earn up to $600. Direct Settlement Facilities You have access to our direct-settlement network for easier admissions and payment. This network includes leading hospitals and clinics throughout the world. And it helps cut your out-of-pocket costs at the point of service. Here s just a few of the advantages: - Easier claim submissions - Additional reimbursement and prepayment choices - Lower out-of pocket costs at the point of service - Verification of Benefits (VOB) letters sent directly to the treatment facility We give you the tools you need to find a direct-settlement facility where you need one. You can search our list of providers online by logging in to your secure member website. You can also download our Provider Directory apps for your smartphone. And, of course, you can always call our Member Service Center for help at the number listed on the back of your Member ID Card. Once you find a direct-settlement provider, you can begin the direct-settlement process online. And even if you can t find a direct-settlement provider in our database, you can request a one-time directsettlement with that provider, and if possible, we'll arrange a direct-settlement. 13

International Mobile Assistant App Life takes you many places. No matter where you are, you still want easy access to your health information and tools to make the best decisions you can. The International Mobile Assistant app makes it possible. It puts the most important features of the secure member website at your fingertips. You can use it to: - Submit claims - Search for providers - Find forms and resources and more The International Mobile Assistant app works alongside our Provider Directory apps, which can be used without internet access to help you find providers while in remote locations. The apps are free to download by searching Aetna International in the itunes or Google Play store. To use the International Mobile Assistant App, you have to be registered for the secure member website. If you have not yet registered for the website, it only takes a minute just visit www.aetnainternational.com and click Member under the Secure Login box. 14

The Aetna International Provider Directory Tool There is an iphone and Android application for members looking for direct-settlement providers around the world. Now available for free download in the Apple App store and the Google Play/Android Market, The Aetna International Provider Directory Plus application provides: - Listing of providers who offer direct-settlement for inpatient and outpatient procedures - Address and contact information for each provider - Access to view the providers on a map and calculate distance to the facility using the users GPS on their mobile device. - Helpful information on what to do before, on the day of and after an appointment -Links to the relevant forms -Integration with the users calendar to record any appointments made with the provider. To download, visit the App Store or Google Play/Android Market from your device and search "Aetna International Provider Directory Tool for U.S. Expats". 15

Recurring Reimbursement Elections You can sign up for recurring reimbursement to make sure you receive reimbursement the way you prefer every time. You can choose to be reimbursed by check, wire, or electronic funds transfer. We can even transfer the money directly to your bank account and we ll cover any fees! The easiest way to sign up for recurring reimbursement is online. 1. Log in to your secure member website 2. Click 'Your Account, Your Policy' then 'Recurring Reimbursement Election' 3. Complete and submit your information online If you haven't already, you can register for the secure member website in just a few steps. All you need to do is visit www.aetnainternational.com and click Member under the Secure login section. You ll need to enter your name, date of birth, and the number on your Member ID Card. 16
