2017 ANNUAL ENROLLMENT OUC EMPLOYEE SUMMARY OCTOBER 17 NOVEMBER 18, 2016 Enrollment is your opportunity to review and make changes to your benefit options that will take effect January 1, 2017. This Benefits Edition provides important updates and new rates to assist you in understanding the plans being offered. OUC is committed to creating a sustainable Well-Being culture to successfully attract, motivate, and retain employees while achieving organizational growth & success. The Employee Self Service (ESS) website will again be your online tool to make any changes to your 2017 benefits. This is the one time of year when you can make changes to your health enrollment benefits without having a qualifying status change. 18 Changes, enrollments and cancellations must be submitted by Friday, November 18, 2016 for coverage starting January 1, 2017. If you want to have a Flexible Spending Account (FSA) for the 2017 calendar year, you must re-enroll. Also, forms are required for additional voluntary term life insurance enrollment. Aetna specialty pharmacy benefit design change. $500 Roll over allowed for your 2017 FSA HRA Plan subsidy increased to $1,020 per year. GETTING STARTED ANNUAL HEALTH PLAN CHANGES THROUGH EMPLOYEE SELF SERVICE At work, access the ESS website by going to OUCweb and clicking on the E1/Employee Self Svc. link or type http://enterpriseone/. From home, go to https://ess.ouc.com and sign in. For assistance logging in, please contact IT Support Center at ext. 20010. Go to the Employee Self Service tab and look for the ANNUAL ENROLLMENT column. 2017 EMPLOYEE RATES OUC is proud to offer competitive benefits to our employees and retirees. One of the most important of these is comprehensive health care for you and your family. To assist employees with costs in 2017, OUC will be responsible for the largest share, but there will be a small increase in premiums for dependents, which we believe are offset by significant enhancements to benefits. At this time, the IRS has not confirmed the 2017 Calendar Year Limits. As soon as information is released, HR Benefits will post on the OUCweb and ESS. FSA is an annual enrollment process and can now be done online through the ESS website. NEW THIS YEAR - Effective in the 2017 Plan Year: Employees will now be able to roll over $500 of unused 2017 funds into their 2018 medical FSA without penalty and without losing those funds. Employees will still need to be mindful of their allocation and ensure they are saving appropriately. 3

UPDATE SPECIALTY MEDICATION BENEFIT Some medical conditions require treatment with special prescriptions classified as specialty medications, which are not available through retail pharmacies or the normal mail order program. Effective January 1, 2016 the annual deductible was waived. Effective January 1, 2017 we have made an additional improvement by adding a $200 co-insurance maximum. 4

EYEWEAR MATERIALS (FRAMES AND LENSES) COVERAGE This coverage is a great way to save money on contact lenses, frames, lenses and even LASIK surgery. You can choose from designer brands such as Oakley, Ray-Ban and Vogue. The Aetna network includes popular chains such as Pearl Vision, LensCrafters, JCPenney Optical, Target Optical and Sears Optical, along with many other neighborhood eye doctors and optical shops. Finding an eye care provider is easy. Just use the Aetna online directory at www.aetnavision.com. ALLSTATE VOLUNTARY SUPPLEMENTAL INSURANCE PLAN Allstate is the provider for the supplemental insurance program. The Allstate products available to employees are: Group Voluntary Accident Plan Group Voluntary Cancer Plan Group Voluntary Critical Illness Plan Group Voluntary Hospital Indemnity Plan You may enroll/add/drop/cancel by contacting Allstate at 877.750.0468. Please contact Allstate to make your plan changes by Friday, November 18. VOLUNTARY TERM LIFE INSURANCE During annual enrollment, you may apply for the following amounts of voluntary term life insurance. For additional information, please refer to the Voluntary Group Term Reliance Brochure, which is posted on the Employee Self Service /Annual Enrollment tab. Employee Voluntary Term Life Insurance Benefit Amount During annual enrollment, there is a guaranteed issue of $10,000 coverage per year with a maximum cumulative amount of $100,000 without medical evidence as long as you are under the age of 60 and have not been previously declined for insurance coverage by Reliance Standard, terminated coverage or withdrawn an application. Benefit Amount If you have $100,000 and are under the age of 60, you may apply for additional $10,000 coverage per year with a maximum of $50,000 (over five consecutive years) without medical evidence as long as you have not been previously declined for insurance coverage by Reliance Standard, terminated coverage previously or withdrawn an application. Optional Dependent Life Insurance Spouse: Employee and spouse may select an amount of insurance from a minimum of $10,000 in increments of $10,000 up to a maximum of $500,000. Evidence of insurability is required. You may enroll for the voluntary spouse term life coverage when purchasing any coverage on yourself. Dependent Child: $2,000 (age 14 days to 6 months) Unmarried Dependent Child: $5,000 or $10,000 (age 6 months to 19 years; 20 to 26 years if full-time student) Please note that in order to insure your dependent child(ren), either yourself or your dependent spouse must have current coverage through the Reliance Standard Voluntary Group Term Life policy. Reminder: If your child is no longer eligible, it is your responsibility to contact HR Benefits to cancel coverage. 2017 LIMIT FOR NATIONWIDE & VOYA DEFERRED COMPENSATION 457 (B) PLANS Currently, the elective deferral (contribution) limit is $18,000. The IRS is expected to post the same amount, $18,000 for 2017 calendar year. If you would like to change your contribution, please go to the ESS website. Current contributions will continue into 2017 unless a change is requested. Please note the deferred compensation programs offer employees age 50 and older a catch-up provision. For more details, contact Nationwide or Voya. The catch-up contribution limit is $6,000. Don t forget your Roth option which allows for post-tax contributions, but earnings are not taxable if held for at least five years. 6

AETNA NAVIGATOR As an Aetna member, you get tools and resources to help you easily manage your health and your benefits. All of your health benefits and health insurance plan information and cost savings tools are in one place your Aetna Navigator member site, www.aetnanavigator.com. NEW IRS INCOME TAX REQUIREMENT IRS Form 1095-C OUC is in compliance with the Affordable Care Act (ACA) mandate of offering health insurance coverage to full-time employees and their eligible children. The ACA, in conjunction with the IRS, is requiring OUC to issue Form 1095-C to all employees eligible for health insurance and non-medicare retirees enrolled in the retiree health insurance. OUC is also required to send this information to the IRS. More information regarding the distribution of Form 1095-C will be communicated at a later date. The IRS will use the information from Form 1095-C to administer the Employer Shared Responsibility provision. The Form also helps the IRS administer premium tax credits for any employee who qualified and enrolled for coverage at the Health Insurance Marketplace rather than enrolling in OUC s plan. DEPENDENT ELIGIBILITY & VERIFICATION Please review the eligible dependent definition carefully to ensure that the dependents you wish to cover will continue to meet the eligibility requirements during 2017. If you are eligible and you elect medical, prescription and dental for yourself, you also may elect to cover your eligible dependents. Part-time employees dependent(s) are now eligible for the health plan. Eligible dependents include: Your legal spouse (opposite-sex or same-sex) Children under age 26 who: Are your biological children; or Are your stepchildren; or Are your legally adopted children; or Are your foster children; or Is a child for whom the employee is the court-ordered legal guardian; or Live with you and whose parent is your child and the parent is covered as a dependent under the plan. Your unmarried children age 26 or older: Your unmarried child(ren) age 26 or older if fully handicapped: He or she is not able to earn his or her own living because of mental retardation or a physical handicap which started prior to the date he or she reaches the maximum age for dependent children under your plan; and He or she depends on you for support, maintenance and must live with you. REMINDERS! If your child is ineligible for any OUC benefits, it is your responsibility to contact HR Benefits to remove them from the plan. You have two years from your date of hire to enroll in the optional 1-2 percent pre-tax contribution to the OUC Defined Contribution 401(a) Pension Plan. After this period, you will not be eligible to enroll in the extra 1-2 percent optional pre-tax deferrals. You will always be eligible to enroll in the post-tax option. The deadlines for the 2016 Flexible Spending Account benefit are as follows: December 31, 2016: Deadline to utilize your account balance March 31, 2017: Deadline to file 2016 claims 7
