INTRODUCTION TO YOUR SELECT TREKS TOUR TO UGANDA Welcome and thank you for your interest in traveling with Select Treks! You will be going to visit the Abayudaya community in eastern Uganda. The basic trip begins upon arrival in Uganda on January 9, 2019, and ends with your departure from Uganda on January 19, 2019, unless you opt to participate in the add-on Gorilla Trek and Safari, which begins on January 19 and ends with a departure on January 24, 2019. Our goal is to make your trip as easy and as purposeful as possible. In order to make sure that all of the trip arrangements are made in a timely manner, we ask that you review and complete the requests for information on the following documents,sign them as indicated and return all completed documents with your non-refundable deposit (or full payment) payable to Select Treks at 5536 Loring Lane, Golden Valley, MN 55422. IMPORTANT INFORMATION 1. A valid passport and visa(s) as indicated are necessary for entry into Uganda American citizens will need to obtain a visa in order to enter Uganda. You can obtain an e-visa online before departure or you can obtain a visa upon arrival. Both cost $50 for a single entry visa (at own expense). An e-visa is recommended to avoid any problems upon arrival. You can apply for an e-visa on the website listed below: https://washington.mofa.go.ug/data-smenu-153-e-visa.html It can take 2 to 3 weeks to receive your visa, so allow yourself sufficient time for this to be processed. In addition, your passport must be valid for at least 6 months after your return date in order for you to travel. 2. Proof of a yellow fever vaccination is required to enter Uganda This can be obtained from your physician or at a travel clinic. It is also recommended that you check with your physician or travel clinic professionals to see if they recommend additional vaccinations and/or preventative malaria medication. https://wwwnc.cdc.gov/travel/destinations/traveler/none/uganda) 3. All trip participants must be covered by travel and evacuation insurance Proof of coverage must be submitted at least 15 days before departure. These are usually sold together and must be obtained at your own expense. You can find information on travel insurance vendors at Insuremytrip: https://www.insuremytrip.com/ A minimum amount of $250,000 for evacuation coverage is recommended. 4. The permit is included in the cost of the trek and safari, but the permit must be purchased as soon as possible to ensure availability. MEDICAL INFORMATION QUESTIONNAIRE Please complete, sign and return this questionnaire with your application for booking. In order to protect your health and safety as well as that of the group, we need your medical history to ensure that you will be able to safely participate in our trip to Uganda. All of your answers will be held in the strictest confidence. Although we attempt to accommodate everyone, we do reserve the right to refuse participation on medical grounds if we feel that your safety, or the safety of your fellow travelers will be compromised. Such a decision will only be made in consultation with yourself and your personal physician. If there is a change in your medical condition after submission of your application, Select Treks must be notified immediately. Booking Terms and Conditions Your application for booking must include a signed Acceptance of Booking Conditions, Waiver and Release of Liability Form (attached) The following are the binding terms and conditions that apply to your Select Treks trip to Uganda. These terms shall constitute the entire agreement between Select Treks (the Company) and the individual(s) wishing to participate in this trip. Please note that Select Treks is the sole party responsible for all arrangements as described in the program brochure. There is no verbal or written representation, warranty, prior agreement, or description of services, other than as expressed herein. By booking a trip, you agree to be bound by these terms and conditions that govern the relationship, cancellation policy and limitations of liability. These terms and conditions affect your rights and designate the governing law and forum for the resolution of any and all disputes. 1.The contract All persons wishing to make a booking have carefully read and understood the terms and conditions that follow. By making a booking with the Company you accept on behalf of yourself and all those named on the booking (including minors) to be bound by these terms and conditions. A booking is only accepted and confirmed from the date when the Company sends a confirmation letter or email acknowledging receipt of: - a signed application for booking - a completed Medical Information Form - the non-refundable deposit, and - the signed waiver of liability. (If the application is being made less than 45 days prior to departure, payment in full must be included.) It is at this point that a Contract between the Company and the Client comes into existence. All person(s) named on the booking are hereinafter referred to as the Client ; references to the Client include all persons on the booking application. Please note that children under the age of 5 are not allowed on this trip. In addition, a Parental or Guardian Consent Form must be submitted for any travelers who are under the age of 18 at the time of travel. Please contact the Company if such waiver is needed.

2. Payment and cancellation Payment Once the Company has accepted and confirmed the Client s application by email or by letter, payment of the balance must be submitted in full or as a series of payments no later than 45 days prior to departure of the trip. If full payment is not received by the due date, the rates published herein as well as tour space cannot be guaranteed and cancellation fees may result. If a booking is made 45 days or less before the departure date, the full amount is due upon booking confirmation. When final payments are not received the Company reserves the right to treat the Client s booking as canceled unless arrangements are made otherwise. Cancellation by Client If a Client notifies the Company in writing that they wish to cancel a booking for which the client has sent in a completed application the following cancellation fees apply: - If the Company has accepted the application and given written notice to the Client of such acceptance, the deposit will be forfeited. -If Cancellation is made 60 to 90 days before the date of arrival, 50% of the cost of services and accommodations will be forfeited. -If Cancellation is made 31 to 60 days before the date of arrival, 75% of the cost of services and accommodations will be forfeited. -Cancellation 30 days or less before the date of arrival will result in complete forfeiture of the trip cost in total. - If Cancellation of a gorilla trek is made after application is accepted, the $630 gorilla permit fee is non-refundable. Cancellation by the Company The Company reserves the right to cancel the trip for any reason, but will not cancel a tour less than 60 days before departure except for force majeure, or unusual or unforeseen circumstances outside the Company s control -When a tour is canceled by the Company before the agreed date of departure for any reason other than the fault of the Client, the Client will receive a full refund of any and all funds paid by the Client -If the Company does not accept a Client based upon the information submitted in the medical form, the $500 deposit shall be refunded in full to the Client 3. Unused services There will be no discounts or monies refunded for missed or unused services, including voluntary or involuntary termination/departure from the tour, late arrival or voluntary or

involuntary premature departure. 4. Changes and flexibility The Client hereby acknowledges that the nature of adventure travel in the third world requires considerable flexibility and a sense of humor. It is understood that the route, schedules, itineraries, amenities and mode of transport may be subject to alteration without prior notice due to local circumstances or events, which may include sickness or mechanical breakdown, flight cancellations, strikes, events emanating from political disputes, entry or border difficulties, extreme weather and other unpredictable or unforeseeable circumstances or any other reason whatsoever. In addition, Clients must understand that other cultures have different standards of timeliness, hygiene, appropriate vehicles, and accommodations. 5. Food The Company agrees to supply all food and beverages as outlined in the trip brochure and itinerary. The Client must understand that the cuisine and ingredients differ from that offered in North America and that the Company cannot reasonably warranty absence of certain substances or allergens. Special diets will be accommodated as much as is reasonably possible. Please contact the Select Treks office at least 30 days before departure so that we can assist if you have special dietary needs. 6. Itinerary changes The Company will endeavor to provide all experiences as advertised in the trip prospectus but reserves the right to make reasonable changes in the itinerary where deemed necessary or advisable in the Company s best professional judgment. If the Company must make a major change, defined as one which affects at least one in three full tour days of the itinerary the Client must be informed as soon as reasonably possible if there is sufficient time before departure. If the major change is due to force majeure or unforeseen circumstances no compensation is payable. The Client hereby accepts that any changes made to alter the itinerary will be made with the full authority of the Company or its authorized representative. If the Company has to make a major change to the itinerary before departure the Client will be notified as quickly as possible and may -Withdraw from the Booking and receive a full refund of all deposits and other payments made directly to the Company to date OR - Accept the change and proceed with the holiday. The Client hereby accepts that the Company s liability in the event of a major change is limited to the monies paid directly to the Company by the Client and not to the recovery from the Company of any other costs associated with the booking.

7. Airfare This booking does not include international airfare or any other flights unless specifically mentioned in the description of the itinerary. It is the Client s responsibility to arrange air travel from the Client s point of departure to the starting point, on the given day and time, of the booking. The Client hereby acknowledges that failure to arrive at the point of departure in a timely fashion will result in a non-refundable cancellation unless the Client pays additional funds to join the trip in progress. If such delay is due to unforeseen circumstances, such as bad weather or flight cancellations, the Company will attempt to pick up the Client at the airport and transport them to join the group at the client s expense. 8. Images and marketing The client agrees that any and all images or videos taken by the Company or by a third party may contain images of the Client. The Client agrees that these images may be reproduced by the Company and grants perpetual, royalty-free, worldwide and perpetual license to the Company to use these images and to share them with Kulanu, Inc. 9. Acceptance of risk and responsibility The Client acknowledges that adventure tours in a developing country involve a certain amount of risk as he or she will be visiting places where the political, cultural and geographical conditions present challenges greater than those present in his or her daily lives. The Client is solely responsible for acquainting themselves with customs, weather conditions, physical challenges and laws in effect in Uganda. By booking travel with the Company, the Client hereby assumes all such risks and does hereby release the Company from all claims and causes of action arising from any inadvertent incident resulting from such inherent risks. In addition, the Client voluntarily assumes any and all risks and responsibilities for damages, losses, and expenses incurred as a result of his/her participation on the trip caused by himself and/or any participant. The mandatory insurance coverage detailed in the application must cover any and all expenses that might arise as a result of loss, damage, injury, delay, or inconvenience that the Client might suffer. The Client hereby acknowledges that the Company cannot be held liable for any damage to property or person caused by an Act of God or any other cause which was beyond the reasonable control of the Company, the consequences of which could not have been avoided even if the highest standard of due care had been exercised or resulted from an event in which the Company or its suppliers could not foresee. In the event that the Company or its suppliers are responsible for any adverse event which is part of the specified itinerary, liability is limited to international convention and all disputes must be litigated in the state of Minnesota.

Waiver and release of responsibility Booking: Uganda Abayudaya Anniversary Trip, January 2019 I, have applied to accompany Select Treks on the above mentioned booking. I understand that this is not a conventional holiday and that adventure travel in a developing country entails acceptance of standards different than what I might expect in North America. I have read and understand the information provided to me and I have +provided all relevant medical information to the Company. I accept all of the conditions of this trip and fully assume the risks of travel. In return for my acceptance of this adventure, I hereby release, hold harmless and discharge Select Treks and any and all of its associates, including third-party vendors, from any and all claims and legal liability unless they arise from gross negligence and/or willful or wanton conduct. My acceptance shall be binding upon my heirs and assignees. I also indemnify and hold harmless the Company from any and all liability for damages to property or personal injury to any third party resulting from my participation in aforementioned trip. Furthermore, I understand that there may be opportunities during my trip to avail myself of additional activities which are not part of the itinerary and are not associated with the Company. I understand that Select Treks is in no way responsible for my safety if I participate in such activities for which I assume full responsibility of all risks involved. This waiver and release applies to such activities. I acknowledge that the Company will be arranging parts of the booking with subcontractors who may, in turn, use their own subcontractors. The Company will at all times endeavor to use reasonable and responsible local suppliers. The terms and conditions of the suppliers will be applicable and incorporated into the contract, and any disputes arising from their services will be judged according to the laws and regulations of the local area. I have reviewed and have understood all provisions and am signing this document voluntarily.
