BRIGHT BEGINNINGS SCHOOL POLICIES Name(s) of Student(s) 1. GENERAL RULES Students must provide Bright Beginnings pre-k Hawaii (hereinafter BB ) a scanned copy of their health insurance prior to the start of, or at the latest on the first day of their program. BB reserves the right to expel any student that becomes disruptive to the program, staff, or other students in the program. In such cases, a refund will not be issued. 2. SIGNING IN AND OUT The authorized adult is responsible for dropping off and picking up the student at school. The class starts at 9am and ends at 12:00pm. BB requires that all children are to be properly signed in and out by an authorized adult in order to ensure their safety. Upon arrival, please sign in your child before class starts. At 12:00pm, please sign out your child upon pickup. Please provide below at least one adult name who has authorization to sign in and sign out the child. 3. LATE PICK-UP Children not picked up at the end of the day will be supervised by our staff only until noon. The parent/legal guardian (hereinafter Parent ) is required to contact BB at the earliest convenience if they are to be late. A fee of $2 per minute will be charged after 12:00pm. If the parent fails to pick up the child by 1pm, BB will contact the police and hand the child over to the police for handling. 4. REGISTRATION & PAYMENT Available spots are filled on a FIRST-COME, FIRST-PAID BASIS. A non-refundable registration deposit will reserve your spot in the class. A month s tuition is due on the week before class starts, or on the first day of class. Failure to settle the initial month s tuition will lead to losing your spot in the class. For subsequent months, for customers who are paying monthly, an invoice will be sent by mail to your house before or near the end of the month. The invoice will show the amount due for the upcoming month. A full-month s tuition for the upcoming month must be settled by the 5th of every month. Failure to pay monthly tuition may lead to losing your spot in the class. We accept all forms of payment (credit card, cash or check). Customers who are paying by check, please send the check to the address provided online or at the following address: Bright Beginnings Hawaii 4211 Waialae Ave, #30 Honolulu, HI 96816 Checks that are not received within one week of enrollment will lose their enrollment spot. 5. CANCELLATIONS BEFORE START OF THE PROGRAM 14 or more business days prior to the start date: full refund (on tuition only). Registration fee is non-refundable. Between 7 and 1 day prior to the start date: 75% refund (on tuition only). CANCELLATIONS AFTER START OF THE PROGRAM After start of the program: tuition is not refundable. 6. LIMITED LIABILITY Parents understand that BB does not guarantee that the program participants will reach any specific level of performance or fluency. Parents agree that, should there arise any liability on the part of BB, its owner, directors, instructors and other employees with regard to the services provided under this agreement or as a

result of participation in all BB programs, BB s liability shall be limited to any tuitions paid. 7. MEDICAL PERMISSION AND MEDICAL INSURANCE Parents hereby grant permission to BB to secure any medial and/or surgical treatment and hospital service for my child in the event of an emergency, where the parent listed on the registration form can be reached. Parents understand and agree that all medical expenses shall be paid by the parent, including ambulance transportation if necessary. All students must have medical insurance prior to the start of their program at BB. Students with allergies or other medical conditions of any kind must inform BB prior to their start date. 8. BOUNCED CHECKS Payment is due in full in advance of all services. A service charge of $20.00 will be applied to returned checks, declined credit cards, or pre-authorized payment chargeback. 9. CONSENT FOR PUBLICATION Parents hereby consent to authorize BB the rights to publish, reproduce and/or use for advertising or any other purposes, any photograph, video image, audio recording or other likeness of the child. Parents hereby consent that any work produced by the child while at BB may be displayed or reproduced by BB in print media or electronic media such as a website for advertising or promotional purposes. Students retain copyright of their work. 10. RECORDING PROHIBITED Recording by any means of any and all BB s materials, classes, activities, and/or the entire program without prior written consent of BB is strictly prohibited. Distribution and/or publication of any such recordings are/is also strictly prohibited. Any violations of these prohibitions may subject parents to any and all legal remedies available to BB, in addition to removal of the child(ren) from any and all BB classes, activities, and/or the entire program without refund. 11. OTHER POLICIES Un-enrolled siblings of current students are not allowed in the classroom when the class is in session. BB is not responsible for any type of lost or damaged clothing or personal possessions or valuables. BB has the right to take any child(ren) out of the classroom if he or she becomes disruptive, and re-enter when the child(ren) is calm. BB reserves the right to modify or cancel classes and themes based on enrollment.

12. ASSUMPTION OF RISKS I understand that part of the BB program experience involves activities and group interactions that may be new to my child, and that they come with certain risks and uncertainties beyond what my child may be used to dealing with at home. I am aware of these risks, and I assume them on behalf of my child(ren). I realize that no environment is risk-free, therefore will instruct my child on the importance of abiding by BB s rules. On my own behalf and on behalf of the student, I assume them knowingly and willingly. In consideration of the benefits derived from the program, I, on my own behalf and on behalf of the student, voluntarily release and hold harmless BB, its offices, agents, directors, employees, representatives and chaperones, and the host parents, for any and all claims, liability, causes of action, costs and expenses (including, but not limited to, claims for loss or damage to the student s property and for illness, injury, death or other risks), which may arise out of or incident to the student s participation in the program. I further agree to indemnify, defend and hold harmless BB, its directors, employees, agents, representatives and chaperones, and the host parents, for any and all claims, liability, causes of action, costs and expenses, which may arise out of or incident to the student s negligent or intentional acts or omissions while participating in the program. I hereby acknowledge receipt of the BB s Policies. I have read and understood the policies and will agree to follow the policies and procedures. I give my permission for my child(ren) to participate in any or all BB programs and activities that are scheduled while he/she is enrolled. Parent Signature Date
