Terms & Conditions Nature of the Exhibit: The exhibits are to provide professional, educational information concerning goods and/or services provided by the Exhibitors for bankruptcy attorneys, judges and others involved in the bankruptcy/insolvency profession attending the California Bankruptcy Forum Insolvency Conference. Exhibit Space: All table-top-spaces include one draped 6-foot by 30-inch table top and two banquet chairs with space behind the booth for a representative. Pop-up displays will have a 10-foot wide by 4-foot deep area and are located only at certain locations. Exhibit Fee: All booths & pop-up displays are $2,500, which includes a Conference Attendee Registration for one person. The registration includes several meals and social functions. Additional Exhibitor Registrations may be purchased for $495 each, if you register before March 16. Recreational activities are optional activities and are an additional cost. Booth Selections: Space will be assigned on a first come, first-served basis. Special consideration will be given to unique space requests. Requests for exhibit space will not be considered without payment of a $1,250 deposit per booth. Installing and Dismantling: Installation of exhibits will take place on Friday, May 17, 2019 from 8:00am -11:00am. Tear down will take place on Saturday, May 18, 2019 from 12:30pm - 3:00pm. Electrical, Telephone or AV: Any electrical, telecommunication or audio visual needs must be ordered in advance from the hotel using the forms in this brochure. Email your forms to PSAV Audio Visual to DWheaton@psav.com before April 26, 2019 to coordinate your electrical, telecommunication or audio visual needs. If you do not reserve these services by April 26 the hotel will not guarantee that they can accommodate your request. Exhibitor assumes entire responsibility and hereby agrees to protect, defend and save harmless the California Bankruptcy Forum and its officers, directors, employees, representatives and shareholders, JBS & Associates and its officers, directors, employees, representatives and shareholders, Westin Mission Hills its subsidiaries and affiliates, their respective agents, officers, directors, servants and employees from and against all claims, losses or damages to persons or property, governmental charges or fines and attorney's fees arising out of or caused by Exhibitor's actions in Westin Mission Hills. California Bankruptcy Forum will provide the security services of Westin Mission Hills staff during the periods of installation of exhibitors and Exhibitor agrees that the provision of such security constitutes adequate discharge of all obligations of the California Bankruptcy Forum and JBS & Associates to supervise and protect the Exhibitor's property. Exhibit Limitations: The exhibit activity of each Exhibitor must be within the Exhibitor's allocated space. Exhibitors may not use noise, lighting or other distractions judged by the conference manager to be offensive to other Exhibitors. Visual aid devices, tape recording equipment or other mechanical devices shall not unduly interfere with other Exhibitors. Refunds and Cancellations: Refunds will not be given for cancellations within 60 days of the start of the scheduled exhibit dates. Cancellations more than 60 days before the initial exhibit date will be charged a $200 service fee per space and the balance of any deposit refunded. Cooperation: The foregoing terms and conditions have been formulated to service the best interests of Exhibitors and attendees. The cooperation of all Exhibitors and patrons is requested. All points not covered herein are subject to settlement by the Conference Manager. Page 3
Exhibit Contract We hereby apply for exhibit space at the California Bankruptcy Forum (CBF) Annual Conference, May 17-19, 2019 at the Westin Mission Hills. We agree to abide by the CBF rules and regulations as set forth in the Terms and Conditions attached hereto. We understand that any additional requirements, such as electrical outlets, special signs, telephone lines, Internet, service of hotel maintenance staff, porters, drayage, etc., will incur charges from the hotel, which are payable by the exhibitor. Set-up day is May 17-18, 2019. Agreement dated, between (herein called "Exhibitor") and CBF. It is agreed between the parties that the Exhibitor shall pay the sum of $2,500 as follows: A deposit of $1,250 per booth space must accompany this form. Final confirmation of the space will be mailed to you and will include an invoice for the balance of the exhibit space charge. Each space is $2,500 and includes one Conference Attendee Registration. Additional Conference Registrations may be purchased for $495 each if you register by March 16, 2019. After that date, the late registration fee of $595 applies. The Conference Registration includes several meals and social functions. Booth Spaces and Conference Registrations are payable 30 days after the date of final confirmation or 30 days prior to the Conference, whichever is earlier. Space or registrations received after March 16, 2019 require full payment with the reservation. Assignment of spaces is on a first come, first-served basis. CBF assignment of booths is final and shall constitute an acceptance of the Exhibitor's offer to occupy space. After assignment, space location may not be changed or transferred without the written consent of CBF.
