We are staying with Harvard Pilgrim this year for both the HMO and PPO plans. There are some minimal changes with our new plans, Best Buy HMO 1000 - Flex and Affordable PPO 25 Flex, so be on the lookout for your new cards. HEALTH PLAN HIGHLIGHTS HMO Employee contribution amounts for all 4 tiers of the HMO plan will remain exactly the same no increase! See HMO Medical premium and Summary page. We now have an added Flex Benefit that lowers the cost of copays for diagnostic, therapeutic and outpatient surgeries while eliminating altogether the copay for laboratory and radiology tests when members use specific Flex facilities. Please see Flex Summary attachment. The single health plan deductible will remain at $1000 while the deductible for 2 persons and family plans will have an increase from $2000 to $2500. FCI will continue to offer the Health Reimbursement Account (HRA) which reimburses half of the deductible costs for each employee; $500 for individual, $1250 for 2 persons and family plans. In addition to the 10% increase of Out-of Pocket Maximum for medical and RX, some copays have increased. Please see the Medical Renewal Summary attachment for details. PPO For this year the PPO plan has some minor benefits changes. The PPO Plan continues to be more expensive for employees, with the employer contribution amount the same as for the HMO plan. Please see the PPO Medical Renewal Summary Page. The Plan design changes for the HMO plan are summarized in this memo, along with the changes in the PPO plan. DENTAL PLAN Good news, the Delta Dental contract is in effect through December 31 st, 2019. As such, all benefits and employee contributions remain the same. OPEN ENROLLMENT Five Colleges, Inc. will hold its annual open enrollment November 1 November 21, 2018. During this time, you may switch plans, add/drop eligible members, or sign up for the first time. If you are going to make a change, please contact the business office and complete a change form. If we do not hear from you by November 21st, you will be automatically re-enrolled in our new plan. POLICY 4- TIER PLAN Five Colleges offers a 4-tier medical plan, covering not only individual and families, but two rates for two party families (one for Employee + Spouse and one for Employee + 1 or more Children), for both the HMO and PPO plans.
