Eligibility is administered by The Centers for & Medicaid Services (CMS) PART A is health insurance for people: 65 years of age or older Under 65 but have been receiving Social Security Disability Insurance for more than 24 months Have End State Renal Disease (ESRD) or Amyotrophic Lateral Sclerosis (ALS) PART B You become eligible for at age 65, regardless of whether you are receiving Social Security benefits at the time.
Enrolling In How you enroll in Part A and B varies based on if you are: PART A PART B Collecting Social Security You are automatically enrolled on the first of the month of your 65 th birthday (unless your birthday is on the 1 st ) You are automatically enrolled on the first day of 25 th month after your Social Security Disability benefits began If you want it, keep the Card sent to you. This is proof of your Entitlement Not collecting Social Security Enrollment is not automatic, you must contact the Social Security office to apply (www.ssa.gov or call (800) 772-1213) May apply 90 days in advance of eligibility
Already Eligible for but Still Working? When will your active employersponsored group coverage end? Contact your Human Resources Department to determine when your coverage will end once you leave employment Your Parts A and B will begin the first of the month after your active group-sponsored coverage ends Enroll in Parts A and B Contact Social Security Administration to enroll in Parts A and B You can enroll up to 90 days prior to your active employersponsored group coverage ending Enroll in a Supplement plan (Medigap or Advantage Plan) The effective date of your Supplement Plan will be the same date as the effective date of your Parts A and B Select and enroll in a Supplement plan (Medigap or Advantage Plan) You cannot enroll in PHIP until Parts A and B have been obtained
