BACKGROUND & NEEDS Impact Statement Continue to Strengthen our great relationships with all local military organizations.granted Four Different Wishes to local Military Heroes and their families in 2016Granted Three Different Wishes to local Military Heroes and their families in 2017Granted Three Different Wishes to Local Military Heroes and their families in 2018Creating partnerships and collaborations with various local groups and organizations. Needs Statement Awareness from our returning Military Heroes - $5,000Exposure to the general public - $5,000Granting Wishes - $25,000 Background Statement Sgt. Denis V. Cooper bravely battled a cancer that went misdiagnosed and ravaged his body. A United States Marine who protected his family and country, was not able to take that one last trip fishing with his family. Because they didn't want their Dad's Last Wish to go unfulfilled, The Denis V. Cooper Foundation Wishes for Heroes, was created. During the Summer of 2011, Maverick Johnson - long-time media personality in Sarasota/Bradenton/Venice, FL (and his family) lost a Father, Husband, Brother, Son & Uncle... (USMC - Sgt. Denis V. Cooper) to lung cancer as a result of his exposure in Vietnam to Agent Orange. While in the V.A. Bay Pines - Hospital Hospice Unit (St. Petersburg, FL), Maverick and his family were informed of a Grant A Wish Program the VA does on a limited basis. Unfortunately for Sgt. Cooper, his last wish: Take a fishing trip with his family, was not possible, due to the severity of his medical condition. Unfortunately, this seems to be the case, all too often. "The Denis V. Cooper Foundation - Wishes for Heroes" was created with a simple goal. To develop a program that reaches out within Sarasota & Manatee Counties on a grass-roots level to honor our military veterans and their families. Currently the 13th Congressional District in Florida which encompasses most of Sarasota & Manatee Counties, is one of the largest Congressional Districts with military veterans in the entire country. The goal to serve our returning veterans as well as those terminally ill veterans and their families, by honoring their service. Statement from the Board Chair Statement from the CEO/Executive Director We are truly blessed in life, when we realize why we are here. In 2011, I lost my dad, the greatest man I will ever know. In the following months, as I was looking for answers as to why, I woke up one night with some clarity. Honor my dad and his love of the military, while at the same time honoring those who have served our great country. Dad treasured his family, his faith and the USA. My father did not get to participate in his "last wish", so we have created "Wishes For Heroes" hoping you can fulfill many wishes for our local military heroes and their families. The Denis V. Cooper Foundation - Wishes for Heroes is an official 501c3 non-profit organization. Areas Served FL- Sarasota FL- Manatee Service Categories 3

Cooper Foundation honors the goals and desires of our brave military service members and their families, by granting "Wishes for Heroes".
