Nrth Shreview Fld Imprvement Prject REQUEST FOR PROPOSALS FOR CONSTRUCTION MANAGEMENT SERVICES ON THE NORTH SHOREVIEW FLOOD IMPROVEMENT PROJECT CITY OF SAN MATEO, CALIFORNIA The City f San Mate hereby requests prpsals fr Cnstructin Management Services (CMS) n the Nrth Shreview Fld Imprvement Prject. All questins regarding this Request fr Prpsal (RFP) shall be directed t Jimmy V, Senir Engineer, at (650) 522 7319. Prpsals shall be submitted by firms that have a capable and demnstrable backgrund in the type f wrk described in Sectin II, "SCOPE OF WORK," f this ntice. In additin, all interested firms shall have sufficient, readily available resurces in the frm f trained persnnel, supprt services, specialized cnsultants and financial resurces t carry ut the wrk withut delay r shrtcmings. The prpsals shall be submitted t the City Clerks, City f San Mate, 330 West 20th Avenue, San Mate, Califrnia, 94403, n later than 2:00 p.m. n February 22, 2019. Each prpser shall submit six (6) bund sets and ne (1) digital cpy f the prpsal in accrdance with Sectin III, "PROPOSAL," f this ntice. I. Prject Backgrund and Overview The City f San Mate (City) maintains a system f levee and fld walls that prtects against high tides, strm surges and wave actin frm the San Francisc Bay and perates a series f pump statins t drain residual interir flding during rain events. Presently, segments f the levee are in need f rehabilitatin and the existing pumping capacity f bth pump statins is deemed insufficient by the Federal Emergency Management Agency (FEMA) t prtect against 100 year strm events. The City is nearing cmpletin f the design n a series f imprvements and is expecting t begin cnstructin in Summer 2019. The Nrth Shreview Fld imprvement prject
Nrth Shreview Fld Imprvement Prject includes wrk at three separate lcatins; the Cyte Pint Levee, Cyte Pint Pump Statin and Pplar Avenue Pump Statin (cllectively referred t as the Prject ). The Cyte Pint pump statin and levee are lcated west f the Peninsula Humane Sciety & SPCA Animal Intake facility, nrth f Airprt Bulevard and suth f the San Francisc Bay alng the shreline. The Pplar Avenue pump statin is lcated suth f the Pplar Creek Glf Curse, east f the PG&E substatin and west f the San Francisc Bay alng the shreline. The Prject cnsists f replacing tw pump statins with new structures, pumps, electrical equipment, and discharge piping t San Francisc Bay. In additin, apprximately 1,300 feet f levee will be raised by apprximately 3 5 feet. Prject plans fr the system imprvements are attached as Appendix B II. Scpe f Wrk At minimum, the City is requesting that prpsers prvide (1) a dedicated Cnstructin Manager (CM), (2) at least ne full time, n site inspectr fr the duratin f the prject, and (3) supprting staff deemed necessary t prvide CMS services. Additinal inspectrs, either at full time r part time, may als be required depending n the specified cnstructin activities. The selected firm will retain all necessary sub cnsultants t prvide specialty services and inspectin t include but nt limited t envirnmental permit cmpliance, bilgical surveys, cnstructin surveys, material testing, electrical/structural inspectin, and SWPPP cmpliance. The scpe f services may include, but is nt limited t, the activities prvided in this sectin. The specific scpe f wrk and staff assignments will be negtiated with the selected firm prir t cntract award. Prject and Cntract Administratin Crdinate pre cnstructin and weekly prgress meetings and prepare and distribute agenda and meeting minutes. Evaluate the Cnstructin Cntractr s cntract schedule fr actual versus planned prgress and dcument all scheduling discrepancies and deviatins. Cmmunicate effectively and crdinate with Cnstructin Cntractr and Prject Manager as defined belw. Prject Manager The individual r entity authrized t represent the City f San Mate fr the Nrth Shreview Fld Imprvement prject (Prject) Cnstructin Cntractr The individual, partnership, crpratin, jint venture, r ther legal entity having a Cntract with the City f San Mate t perfrm prpsed imprvement prject t be determine during advertisement and award f the cnstructin cntract. Prepare and submit cnstructin mnthly prgress reprts alng with mnthly CM payment invice. Mnthly prgress reprts shall include the fllwing: Overview f wrk accmplished, including phts; Wrk t be cmpleted the fllwing mnth;

Review f Cnstructin Cntractr s wrk fr cmpliance with cntract dcuments, envirnmental dcuments, state laws, regulatins, etc. Review Cnstructin Cntractr submittals fr cmpliance with cntract dcuments and crdinate Prject Manager and Design Engineer review. Review Cnstructin Cntractr Requests fr Infrmatin (RFIs) and crdinate with Design Engineer fr reslutin and respnse. Lg all RFIs and assciated respnse. Review Cnstructin Cntractr mnthly invices and recmmend payment requests. Prvide Cnstructin Cntractr with acceptability r deficiencies in wrk and required actins fr cntract cmpliance. Maintain prject dcuments files in the frmat and lcatin specified by the City. Cnstructin Observatin/Inspectin Inspect and bserve Cnstructin Cntractr s wrk daily fr cmpliance with the cntract dcuments. Maintain daily inspectin reprts f wrk, including phts, descriptins f daily activities, any crrective actins t be taken by Cnstructin Cntractr, persns and equipment nsite, and wrk t be cmpleted the fllwing day(s). Review f materials and equipment delivered t site fr cmpliance with submittals and cntract dcuments. Crdinate and prvide material testing and specialty inspectins as required. Review f Cnstructin Cntractr s cmpliance with all permits, strmwater regulatins and permits (Strmwater Pllutin Preventin Plan (SWPPP)), envirnmental dcuments, and mitigatin measures. Review f Cnstructin Cntractr s cmpliance with health and safety standards and regulatins and reprting f nn cmpliance. Mnitr recrd dcuments t ensure prper maintenance and cmpletin by Cntractr. Change Order Management Crdinate, review, and evaluate ptential change rders and cst estimates/qutatins. Crdinate, review, evaluate, and recmmend curse f actin fr cntract change rder requests and submittals. Inspect and evaluate site cnditins that are perceived t be different than shwn in cntract dcuments. Review, gather, and evaluate infrmatin fr reslutin f ptential claims r disputes.

Develp crrective item wrk lists (punch lists) and inspectin f crrective actins perfrmed. Recmmend retentin release t Cnstructin Cntractr. Verify all O&M Manual submittals as well as spare parts as required by the cntract dcuments. Crdinate final submittal f rganized and cmplete recrd drawings in accrdance with cntract dcuments. Verify Cnstructin Cntractr has made all payments and that all required releases including warranty release, lien release(s), and release f claims have been submitted by Cnstructin Cntractr. Prepare and submit final cnstructin reprt. III. Prpsal The prpsal shall cnsist f six (6) bund sets and ne (1) electrnic cpy n flash drive cntaining 8 1/2" x 11" sheet sizes fr the text, 11" x 17" sheet sizes fr any fld ut drawings, and PDF cpies. The cst prpsal shall be limited t twenty (20) pages (resumes fr prpsed persnnel will nt be cunted). Prpsals shall be structured as utlined in the fllwing sectins and cntain the : Cver Letter The prpsal shall be submitted with a cver letter. The letter accmpanying the prpsal must prvide the name, title, address, telephne number, and signature f the individual(s) authrized t negtiate and bind the firm cntractually. An unsigned prpsal r ne signed by an individual unauthrized t bind the firm may be rejected. The cver letter shall prvide a summary f the firm s bench strength and availability f cnstructin management staff, infrmatin and qualificatins f the prpsed lead wh will be representing the firm, and agreement r exceptins t the prvided subcnsultant terms and cnditins. Prject Apprach Prvide a detailed descriptin f the firm s prpsed apprach t implementing CMS fr the Scpe f Wrk described in Sectin II. The apprach shall at a minimum include the fllwing: Anticipated challenges typical with levee and pump statin facility prjects within envirnmentally sensitive areas (ESAs), and hw the firm will apprach these challenges. Implementatin plan fr the tasks described in the scpe f services and any recmmended revisins r additins t the list f tasks. Any innvative appraches t implementing these services.
Nrth Shreview Fld Imprvement Prject Experience and Qualificatins Prvide a brief histry f the firm, including: name f the firm, general services, experience with CMS, the year the firm was established under the current name, the principal place f business, and the lcatin f lcal ffice(s). Indicate any ther previus names fr the firm during the last five (5) years and the year any name change was effective. Prvide a list f at least three (3) prir prjects within the last five (5) years in which firm prvided relevant cnstructin management and inspectin services similar t this assignment. The list shall include the fllwing fr each assignment: Name f client Lcatin (city and state) Name and brief descriptin f cnstructin prject Start date f cnstructin and (expected) date f cmpletin fr CMS Cnstructin value and CMS cntract value Respndent s scpe and rle in the prject (include explanatin f methdlgies and appraches used) Client reference fr services prvided including name, psitin, phne, and email. Up t three (3) recent relevant prjects; include a brief descriptin f the prject (scpe, size, cst, etc.), the individual s specific rle n the assignment, the year the individual s wrk n the assignment was cmpleted, and the individual s emplyer while wrking n the assignment. Team Organizatinal Structure Describe the firm s team rganizatin, including identificatin f any partners r subcnsultants/ subcntractrs. Indicate the rle and respnsibilities f the Prime Subcntractr and all subcnsultants/ subcntractrs. Estimated Labr Hurs This sectin shuld include the estimated labr hurs by persnnel prpsed fr the prject fr each task identified in the Scpe f Wrk and prject assignments described in Sectin II. A standard hurly rate fee fr the firm and it s subcnsultants are required t be submitted and included within the prpsal. Resumes and Qualificatins f Prpsed Persnnel This sectin shuld include resumes fr all persnnel prpsed fr the Prject. Resumes will nt be cunted tward the page limit established in Sectin III.

The prpsed budget and fee schedule shall be included in a separate, sealed envelpe with the fllwing infrmatin clearly marked n the utside. Cnsultant Selectin Prcedure After the review f prpsals, the City may invite sme r all prpsers t present their qualificatins and prpsed apprach t the prpsed CMS r may decide t select ne prpser withut cnducting interviews and enter int cntract negtiatins directly. Prpser interviews, if necessary, are anticipated t be cnducted accrding t the schedule prvided in Sectin VIII. Details f the interview prcess will be prvided alng with the invitatin t present. VI. Insurance Requirements By submitting a prpsal, Prpser represents and warrants they will prvide the required insurance as identified in Attachment A. VII. Prfessinal Services Agreement By submitting a prpsal, Prpser represents and warrants they will execute the Agreement fr Cnsulting Services included in Attachment A. VIII. Estimated Prject Schedule The fllwing is the City s anticipated schedule fr selecting and awarding a cntract fr CMS fr the Prject.
