Be sure to also read the general terms and conditions.) 1. What is this product about? This policy provides Comprehensive cover, Third Party, Fire & Theft cover and Third Party cover. What is the Period of Cover and Renewal Option? Duration of cover is usually for one (1) year. You need to renew your insurance cover policy annually. 3. How much premium do I have to pay? The total premium that you have to pay may vary depending on the sum insured of the vehicle, cubic capacity, no-claim-discount (NCD) entitlement and other underwriting criteria that the insurance company imposes, for example, policyholder age and vehicle age. What is No Claim Discount (NCD)? NCD is awarded if no claim was made against your policy during the preceding twelve (12) months of policy. Your NCD entitlement will depend on the class of your vehicle. 4. What are the fees and charges I have to pay? 5. What are some of the key terms and conditions that I should be aware of? DUTY OF DISCLOSURE Consumer Insurance Contract Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance wholly for purposes unrelated to your trade, business or profession, you have a duty to take reasonable care not to make a misrepresentation in answering the questions in the Proposal Form (or when you apply for this insurance). You must answer the questions fully and accurately. Failure to take reasonable care in answering the questions may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. In addition to answering the questions in the Proposal Form (or when you apply for this insurance), you are required to disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. Non-Consumer Insurance Contract Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for a purpose related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. Depending on the circumstances of the loss or claims made during the term of policy, we may review the policy terms and conditions upon renewal or decline renewal. CHANGE IN RISK If at any time or from time to time any change shall occur materially varying any of the facts existing at the date of the proposal, the Insured shall within seven (7) days give notice in writing to the Company and shall pay such additional premium as the Company may require. PREMIUM PAYMENT AND CASH BEFORE COVER Full premium must be paid before the effective date of the Policy. Payment can be made by cash, credit card or GIRO to Berjaya Sompo Insurance Berhad. Insist on a receipt of the premium paid as proof of payment for future reference. Compulsory excess - that is the amount of loss you have to bear if your vehicle is driven by a person not named in your policy: RM400.00 Excess It is the amount of loss you have to bear while your insurance company will pay for the balance of your vehicle damage claim. Contribution we shall only liable our rateable proportion of the loss in the event an insured has more than one policy to cover your vehicle. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of terms and conditions under this policy. 6. What are the major exclusions under this policy? This Policy does not cover certain losses, such as:- Your own death or bodily injury due to a motor accident. Your liability against claims from passengers in your vehicle. Loss, damage or liability arising from an act of nature i.e. flood, storm or landslide. 2

Theft of non-factory fitted vehicle accessories unless otherwise declared. Consequential loss, depreciation, wear and tear, mechanical or electrical breakdown, failures or breakages. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of exclusions under this policy. 7. Can I cancel my policy? You may cancel the policy at any time by giving written notice and your original Certificate of Insurance to Berjaya Sompo Insurance Berhad. Upon cancellation, any refund of the premium would be based on the conditions stipulated in the policy contract. 8. What do I need to do if there are changes to my contact/personal details? It is important that you inform us of any changes in your contact and personal details to ensure that all correspondence reach you in a timely manner. You may inform our branch office or customer service of the Company. 9. How do I make claim? Report to the police within twenty-four (24) hours and immediately notify your insurance company in writing with full details. Repairs must be conducted by Authorized Panel Workshop selected and approved by us if you make an own damage claim against your own comprehensive policy. If you are not at fault in the accident, you can submit the claim either directly to the insurance company of the party at fault, or if you have a comprehensive policy, you are encourage to submit to your insurance company for speedier claims processing without losing your NCD entitlement. You will lose your entire NCD entitlement once an own damage or a third party claim is made against your policy. 10. Where can I get further information? Polisi ini memberikan perlindungan Komprehensif, Perlindungan Pihak Ketiga, Kebakaran & Kecurian serta Perlindungan Pihak Ketiga. Apakah tempoh perlindungan dan pembaharuan pilihan? Tempoh perlindungan biasanya selama satu (1) tahun. Anda perlu memperbaharui polisi insurans anda setiap tahun. 3. Berapakah premium yang saya perlu bayar? Jumlah premium yang anda perlu bayar adalah berbeza bergantung kepada jumlah yang diinsuranskan, keupayaan enjin, kelayakan Diskaun Tanpa Tuntutan (NCD) dan keperluan menaja jamin syarikat insurans seperti umur pemegang polisi dan umur kenderaan. Apakah itu Diskaun Tanpa Tuntutan (NCD)? NCD akan diberikan sekiranya tiada tuntutan dibuat terhadap polisi anda dalam tempoh dua belas (12) bulan sebelum itu. 4. Apakah yuran dan caj yang saya perlu bayar? Apa yang anda perlu bayar sebagai tambahan kepada Jumlah premium Cukai Perkhidmatan 6% daripada premium Duti Setem RM 10.00 Apa yang termasuk dalam premium Komisen yang dibayar kepada pengantara insurans (jika ada) Jumlah 10% daripada premium 5. Apakah terma dan syarat utama yang saya harus ketahui? KEWAJIPAN PENDEDAHAN Kontrak Insurans Pengguna Menurut Perenggan 5 daripada Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini sepenuhnya untuk tujuan yang tidak berkaitan perdagangan, perniagaan atau profesion anda, anda mempunyai kewajipan untuk mengambil langkah yang munasabah untuk tidak salah nyata dalam menjawab soalan-soalan dalam Borang Cadangan (atau semasa memohon insurans ini). Anda dikehendaki menjawab soalan-soalan dalam Borang Cadangan ini dengan lengkap dan tepat. Kegagalan untuk mengambil langkah yang munasabah dalam menjawab soalan-soalan, mungkin mengakibatkan pembatalan kontrak insurans anda, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan diatas hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Sebagai tambahan kepada soalan-soalan di dalam Borang Cadangan (atau semasa memohon insurans ini), anda dikehendaki untuk mendedahkan apa-apa perkara lain yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami (atau semasa permohonan insurans ini), apa-apa maklumat yang dinyatakan dalam Borang Cadangan tidak tepat atau sudah berubah. Kontrak Insurans Komersial Menurut Perenggan 4(1) Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang berkaitan dengan perdagangan, perniagaan atau profesion anda, anda berkewajipan untuk mendedahkan apa-apa perkara yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan, dan apa-apa perkara yang munasabah yang boleh dijangka, jika tidak ia boleh menyebabkan pembatalan kontrak insurans, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan diatas hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami (atas semasa permohonan insurans ini), apa-apa maklumat yang dinyatakan dalam Borang Cadangan tidak tepat atau sudah berubah. Bergantung kepada keadaan kerugian atau tuntutan dibuat semasa tempoh polisi, kami boleh mengkaji semula terma-terma dan syarat-syarat polisi pada masa pembaharuan atau menolak pembaharuan. PERUBAHAN RISIKO Jika pada bila-bila masa atau dari semasa ke semasa, apa-apa penukaran berlaku dan mengubahkan secara material apa-apa fakta yang wujud pada tarikh cadangan, Pihak Diinsuranskan dan/atau Orang Diinsuranskan hendaklah memberi notis secara bertulis kepada Syarikat dalam masa tujuh (7) hari dan hendaklah membayar premium tambahan sedemikian sebagaimana diperlukan oleh Syarikat. PEMBAYARAN PREMIUM DAN TUNAI SEBELUM PERLINDUNGAN Pembayaran premium penuh mesti dibuat sebelum polisi ini berkuatkuasa. Bayaran boleh dibuat melalui Tunai, Kad Kredit atau GIRO kepada Berjaya Sompo Insurance Berhad. Sentiasa meminta resit untuk premium yang dibayar sebagai bukti pembayaran untuk rujukan masa depan. Lebihan Wajib Ia adalah jumlah kerugian yang harus anda tanggung sendiri jika kenderaan anda dipandu oleh seseorang yang tidak dinamakan dalam polisi anda: RM400.00.

Lebihan Ia adalah jumlah kerugian yang perlu anda tanggung manakala syarikat insurans akan membayar baki tuntutan kerosakan kenderaan anda. Sumbangan - kami hanya akan bertanggungjawab ke atas kadar kerugian kami jika berlaku sesuatu yang diinsuranskan itu mempunyai lebih daripada satu polisi untuk melindungi kenderaan anda. Nota: Senarai ini tidak menyeluruh. Sila rujuk kontrak polisi untuk senarai penuh terma dan syarat bawah polisi ini. 6. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak melindungi sesetengah kerugian, seperti: Kematian atau kecederaan diri anda sendiri yang disebabkan oleh kemalangan motor. Liabiliti terhadap tuntutan daripada penumpang di dalam kenderaan anda. Kerugian, kerosakan atau liabiliti akibat kejadian semula jadi seperti banjir, ribut atau tanah runtuh. Kecurian aksesori kenderaan yang bukan dipasang di kilang, melainkan sekiranya diisytiharkan. Segala kerugian berturutan, susut nilai, haus dan lusuh, kerosakan mekanikal atau elektrik, kegagalan atau kerosakan. Nota: Senarai ini tidak menyeluruh. Sila rujuk kontrak polisi untuk senarai penuh pengecualian bawah polisi ini. 7. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi anda pada bila-bila masa dengan memberi notis bertulis dan Sijil Insurans asal anda kepada Berjaya Sompo Insurance Berhad. Apakah yang harus saya lakukan jika terdapat perubahan dalam butiran peribadi saya? Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai butiran peribadi anda bagi memastikan semua maklum balas dapat disampaikan kepada anda tepat pada masanya. Anda boleh hubungi pejabat cawangan atau perkhidmatan pelanggan Syarikat kami. 9. Bagaimanakah saya membuat tuntutan? Buat laporan polis dalam masa dua puluh empat (24) jam dan maklumkan syarikat insurans anda secara bertulis dengan memberi butiran lengkap dengan segera. Pembaikan hendaklah dikendalikan oleh Bengkel Panel Disahkan yang telah dipilih dan diluluskan oleh kami sekiranya anda membuat tuntutan kerosakan sendiri terhadap polisi komprehensif anda sendiri. Sekiranya anda bukan pihak yang bersalah dalam kemalangan tersebut, anda boleh mengemukakan tuntutan sama ada secara terus kepada syarikat insurans pihak yang bersalah atau, sekiranya anda mempunyai polisi komprehensif, anda digalakkan mengemukakan tuntutan anda kepada syarikat insurans anda sendiri untuk mempercepatkan pemprosesan tuntutan tanpa kehilangan kelayakan NCD anda. Anda akan kehilangan keseluruhan kelayakan NCD anda apabila tuntutan dibuat kerana kerosakan sendiri atau pihak ketiga membuat tuntutan terhadap polisi anda. 10.
