1. INTRODUCTION AND INFORMATION 1.1. PURPOSE AND SCOPE GHC-SCW is seeking to repair, replace and/or otherwise mitigate future damages to its facilities that were damaged as a part of the August September 2018 floods (hereinafter referred to as the DR-4402 floods). The August 20 th, 2018 flood event caused significant damage to a GHC- SCW-owned outpatient health care facility located in Madison, Wisconsin (the Facility ). Flood waters resulted in significant damage to subterranean and ground-level clinical, laboratory space, and ancillary service space, in addition to electrical, plumbing, mechanical, and life-safety systems. GHC-SCW now seeks to procure a licensed, qualified engineering firm that will serve as the design team, provide construction oversight, and act as administrative project manager for the repair, replacement, temporary relocation of certain functions, and/or mitigation measures made necessary by the DR-4402 floods. The Facility in question is a multi-story (3) concrete-masonry building with in place concrete foundation walls. The total square footage of the Facility is approximately 35,334, with an estimated two-thirds of this space being directly impacted by flooding. The Facility contains a basement space where electrical, mechanical systems, and laboratory space were previously located. Due to a desire to mitigate future risk, the selected firm will assist GHC-SCW in evaluating and pursuing hazard mitigation practices to relocate these critical systems to safer, more desirable locations within the Facility and/or within one or more separate off-site facilities. The selected firm may be asked to provide assistance with identifying feasible temporary relocation space, to perform preliminary design work, and to manage any build-out activities required to permit the space to meet GHC-SCW s operational needs. Initial remediation has been completed and stabilization activities continue at the Facility. The basement has been completely stripped and remains unfinished. A two-foot drywall cut has been undertaken on the first floor and all damaged flooring and cabinetry has been removed. The building remains minimally climate-controlled but otherwise unoccupied. It has not been determined whether the totality of damage requires the Facility to be brought up to current codes and standards. The selected firm will work with GHC-SCW to develop the required designs, plans, specifications, cost estimates, and permitting requirements to facilitate the repair and restoration of the Facility. In addition, the selected firm will develop any required designs, plans, specifications, cost estimates, and permitting requirements related to mitigation measures. The firm will also provide construction oversight and administrative management services, on GHC- SCW s behalf, to a general contractor(s) to be selected by Request for Proposals ( RFP ) at a later date. GHC-SCW will not utilize a design-build contract arrangement for this project. Time is of the essence with this solicitation and interested firms should be willing and able to engage immediately and begin performance following selection and contract execution. Project funding may include disaster recovery funding from the Department of Homeland Security Federal Emergency Management Agency (FEMA). As such the selected firm will be responsible for assisting GHC-SCW and its third-party consultants with design team support in 3
compliance with FEMA s Public Assistance grant program as well as other compliance measures dictated from supplemental disaster-related funding sources. Interested firms should demonstrate the ability to comply with all federal procurement, contracting, and administrative requirements associated with FEMA s disaster recovery programs. 1.2. ENGINEERING DESIGN FEES Fees shall be negotiated with the selected firm as part of the contract negotiation. 1.3. PRE-PROPOSAL MEETING No pre-proposal meeting will be held for this RFQ. 2. STATEMENT OF WORK AND REQUIRED SUBMITTALS 2.1. SCOPE OF SERVICES needs: Selected engineering firm shall provide the following services based on GHC-SCW s a. Feasibility and design preparation for: Repairs to the Facility; Replacement of the Facility and/or components thereof; Mitigation measures; and Temporary and/or Alternate Operational Facilities b. Cost or Price Analysis for: Repairs to the Facility; Replacement of the Facility and/or components thereof; Mitigation measures; and Temporary and/or Alternate Operational Facilities c. Project scheduling d. Local, state and federal permitting support e. Construction administration and oversight f. Other administrative and professional tasks necessary to meet GHC-SCW s federal sponsor requirements 4
2.2. SUBMITTAL REQUIREMENTS a. General Submittal: Applicants shall submit one electronic copy of their proposal to Tison Rhine at trhine@ghcscw.com by the deadline stated above. b. Valuation of Submittal: GHC-SCW will base its scoring of proposals on the following Score Card criteria: Factors Firm/Team Qualifications and Experience Key Personnel Qualifications and Experience Capacity of Firm/Team and Office Location Federal Contracting Experience Small Business Participation Weight/Pts 0-35 pts 0-35 pts 0-20 pts 0-5 pts 0-5 pts c. Selection Process: Three staff members from GHC-SCW familiar with facility and contracting processes and procedures will review all timely submissions prior to convening to agree on scoring utilizing the criteria referenced above. Summary score sheets will be prepared for all timely submissions and GHC-SCW reserves the right to contact prospective firms to engage in contract negotiations. GHC- SCW reserves the right to not select or engage any firm following this process. d. Required Proposal Information: Applicants must provide responses to this RFP in the following format. GHC-SCW reserves the right to declare proposals nonresponsive in not in the following format. GHC-SCW also reserves the right to engage individual applicants for further information or clarification in response to this RFP. i. Main Proposal: - Letter of Interest: Provide a letter of interest from a duly authorized representative confirming the Applicant s active business status and authority to conduct work in the State of Wisconsin. Provide point of contact information for the Applicant s proposal. - Applicant Qualification and Experience: Provide a brief history of the company s formation. Provide project specific experience completed by the Applicant similar to the scope of work described above. - Key Personnel Qualifications and Experience: Provide a list of key personnel that will be involved with this project scope of work, including CVs and or resumes documenting their experience. 5
- Capacity of Applicant and Office Location: Provide explanation of Applicant s size and capacity to meet the needs of this project scope of work in addition to other company workload. Provide location of personnel supporting this scope of work and their ability to meet the ongoing needs for this project. Discuss ability to quickly engage and begin performance if selected. - Federal Contracting Experience: Provide brief history of the Applicant s experience designing and overseeing federal contracts, including those in accordance with disaster recovery funding programs. - Small Business Participation: Provide certification as a minority, woman-owned, or other government certified small business. If inapplicable, provide a brief explanation of how Applicant intends on soliciting small business participation to help meet the needs of this scope of work if needed. ii. iii. Commercial Liability Insurance: $1,000,000 combined single limit per occurrence for bodily injury, property damage, and personal injury with a $2,000,000 general aggregate limit. Business Automobile Liability: $1,000,000 combined single limit per accident for bodily injury and property damage. Workers Compensation Insurance: Adequate and reasonable limits consistent with Wisconsin state law. Professional Liability Insurance: $1,000,000 per occurrence and $2,000,000 per aggregate. 6
