The scholarship is awarded to students who come from an economically and/or environmentally disadvantaged background. All recipients must meet the Economically Disadvantaged requirements (Part II A). Environmentally Disadvantaged Background: An individual who has an environmentally disadvantaged background comes from an environment that has inhibited the individual from obtaining the knowledge, skill and abilities required to enroll in and graduate from a health professions school. COMMITMENT AND COMPLIANCE AGREEMENT The Scholarships for Disadvantaged Students is a scholarship program of the Health, Resources and Services Administration, U.S. Department of Health and Human Services. The program addresses a major barrier to disadvantaged students access to health professions education -- namely, high tuition costs that often result in failure to complete due to the lack of funds and the scholarship awards may allow students to complete their education sooner without interruption. The SDS Program gives funding priority to (1) underrepresented minorities, (2) graduates practicing in primary care, and (3) graduates working in medically underserved communities. The SDS program provides substantive grant awards to schools such as ours to increase primary care, minority and disadvantaged students retention and graduation. Our school requires all SDS Applicants to sign this letter of commitment and compliance. If awarded, you must respond by checking the boxes, date and affix your signature below in order for you to receive this scholarship award. 2014 2015 Verification Worksheet Independent Student (HRSA Definition) Your 2014 2015 Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification.
