It does not apply to Apple Watch 1, Apple Watch Series 1 and Apple Watch Series 2. If you have collected your Apple Watch Series 3 Sport 38mm or Apple Watch Series 4 Sport 40mm from an istore and would like to transfer the Vitality Active Rewards with Apple Watch benefit, please complete the following transfer form. The Vitality Active Rewards with Apple Watch benefit can only be transferred to another person (the transferee) who is eligible for the Vitality Active Rewards with Apple Watch benefit. If the transferee is not eligible for the benefit, you may not transfer the benefit. To be eligible for the Vitality Active Rewards with Apple Watch benefit the transferee must: Be the main, secondary or adult dependant member on the same Vitality policy as the transferor; Have Vitality Active Rewards activated; You must have a Discovery Gold Card or higher. If you don t have a qualifying Discovery Card, you can apply for one or request an upgrade on www.discovery.co.za. You can also call us on 0860 99 88 77. If you do have a qualifying Discovery Card, it must be in good standing*. Have a Discovery Gold Card or higher with a straight limit of at least R15 000 and an available balance of at least R5 000 after paying the R799 Apple Watch activation fee and when you collect your Apple Watch from istore. Pay a R600 non-refundable transfer fee for the Vitality Active Reward with Apple Watch benefit. * Your Discovery Card account and Discovery Card is in good standing if it is open, active and not in arrears for more than three months. Your account is not in good standing if you need to undergo debt counselling. If you activate the Vitality Active Rewards with Apple Watch benefit from 28 September 2018, a non-refundable transfer fee of R600, will be debited against the Discovery Card account linked to the benefit. This fee is over and above the original non-refundable R799 activation fee already paid by the transferor. The transferor will not be refunded the original R799 non-refundable activation fee. The Vitality Active Rewards with Apple Watch benefit can only be transferred once within 60 days of the Apple Watch being collected from the istore. The transferee is unable to transfer the Apple Watch benefit to anyone else. Please complete this form and submit it to us by sending us a query through the Vitality Help page on www.discovery.co.za. Ensure that you sign this application form. The completion of this form does not guarantee that a transfer will be granted. Discovery Vitality reserves the right to decline the transfer of the Vitality Active Rewards with Apple Watch benefit. Option 1 By transferring the Vitality Active Rewards with Apple Watch benefit to the transferee, I, the transferor, agree that I will be responsible for the Vitality Active Rewards with Apple Watch benefit penalty payments and its associated billing rules. I (the transferor) authorise Discovery Vitality to issue and deliver debit order instructions to the bank for collection against the Discovery Card account linked to the benefit. I authorise Discovery Vitality to debit the Discovery Card account (issued by FNB Ltd) with a non-refundable transfer fee of R600. Option 2 I, the transferor, want to transfer the Vitality Active Rewards with Apple Watch benefit and the associated Discovery Card account linked to the benefit. If I don t meet my goals, the qualifying Discovery Card linked to the benefit will be debited by a monthly amount (penalty) that is calculated as a percentage of the retail price at the time of collection (pricing subject to change). I, the transferee, agree that I cannot transfer the Vitality Active Rewards with Apple Watch benefit to anyone else. I, the transferee, acknowledge that the benefit start date will remain the original date of the benefit when the transferor activated the benefit and the Apple Watch billing cycle will remain as the original date of when the transferor collected the Apple Watch Series 3 Sport 38mm or Apple Watch Series 4 Sport 40mm from the istore. I, the transferee, agree that I am fully aware of the Vitality Active Rewards with Apple Watch benefit rules which appear on www.discovery.co.za and agree to be bound by these rules. I also authorise Discovery Vitality to debit the Discovery Card account (issued by FNB Ltd) with a monthly amount (penalty) on the 10th day of each month, for 24 months, if I don t meet my Vitality Active Rewards goals. I, the transferee, agree that if I default on any payment amount (penalty) due, I accept that Discovery Vitality may cancel this benefit. I understand that I cannot then return my Apple Watch to Vitality and I will be liable for the full remaining cost of my Apple Watch. I authorise Vitality to collect any remaining amount owed by me from the Discovery Card account straight facility linked to this benefit. If Discovery Vitality is unable to do so, I accept that the outstanding balance will be for my account and default interest will be charged on it. Print name - 3 -

Terms and conditions What you pay If you activate the Vitality Active Rewards with Apple Watch benefit from 28 September 2018, a non-refundable transfer fee of R600, will be debited against the qualifying Discovery Card linked to the Vitality Active Rewards with Apple Watch benefit. If the transferee accepts the transfer of this benefit, the transferee warrants that they will meet all their weekly Vitality Active Rewards goals each month for 24 months. Discovery Vitality will then cover the cost of the Apple Watch Series 3 Sport 38mm or Apple Watch Series 4 Sport 40mm in full. If the transferee doesn t meet their goals, the Discovery Card account linked to the Vitality Active Rewards with Apple Watch benefit will be debited by a monthly amount (penalty) that is calculated as a percentage of the retail price at the time of collection (pricing subject to change). For example, the table below is based on an illustrative retail price at the time of collection of R7 400 (this is an example costing for illustrative purposes). If the transferee didn t meet all their goals in a month, the Discovery Card account linked to the benefit would be debited by up to R308.33 a month for 24 months (pricing subject to change). If the transferee achieved three of their four Vitality Active Rewards goals that month, just R154.16 would be debited due to their engagement in Vitality Active Rewards. Weekly Vitality Active Rewards goals met in a month The monthly amount (penalty) that will be debited from your Discovery Card (for 24 months) 0-1 2 3 4 100% 75% 50% 0% (Discovery Vitality will fund the monthly amount for your Apple Watch in full.) The transfer may not occur if: The transferor has outstanding debt on the Apple Watch benefit if it s only the benefit transfer. The member has a freeze instruction loaded as a result of not being able to achieve their Vitality Active Rewards goals because of specific personal circumstances. Link your Apple Watch to earn Vitality fitness points Link your Apple Watch Series 3 Sport 38mm or Apple Watch Series 4 Sport 40mm to Discovery Vitality to start earning fitness points towards your weekly Vitality Active Rewards goals. Use the Discovery app to link your Apple Watch by navigating to the Vitality section and selecting Devices and apps. Keep achieving your weekly goals to fund your Apple Watch in full The transferor s weekly goal cycle commences from the first full month after the transferor collects their Apple Watch Series 3 or Apple Watch Series 4 from the istore. The transferee will take on the remaining weekly goal cycle period for the Apple Watch from the transferor from the month in which the transfer is complete. The transferee s weekly goals contributing to the Vitality Active Rewards with Apple Watch benefit will only be calculated from the month in which the transfer is complete, no weekly goals achieved prior to the month in which the transfer is complete will be considered. For example, the transferor collected their qualifying Apple Watch Series on 29 December 2017. Their first goal cycle period commenced on 1 January 2018. The goal cycle for January 2018 commenced on 1 January and ends on 31 January. A request to transfer the benefit to the transferee was received and completed by Vitality in January 2018. All weekly goals achieved by the transferee in the month of January will contribute to the January goal cycle and he/she will be responsible for any associated penalty charges should goals not have been achieved. If the transferee reaches all their goals each month, the Discovery Card account linked to the benefit will not be debited as Vitality will fund the Apple Watch benefit for that month in full. - 4 -

If the transferee does not meet any of their weekly Vitality Active Rewards goals, this will be seen as a breach of their commitment and their warranty. The transferee will then be liable to Discovery Vitality for a monthly amount (penalty) that will be debited from the Discovery Card account linked to the benefit. This amount can vary from person to person depending on the retail price of Apple Watch when the transferor collected the Apple Watch from the istore and depending on the number of goals not achieved for that month. If the transferee reaches some of their weekly goals in a given month, the penalty will be reduced by their engagement in Vitality Active Rewards (see example under What you pay ). The transferee will still be considered liable to Discovery Vitality for breaching their warranty for that month. Discovery Vitality will set a personalised Vitality Active Rewards goal for you each week. As Vitality is a dynamic programme which is based on a behavioural economics model, designed to consistently encourage you to improve your health, your goals can increase or decrease based on your goal history. The Vitality programme design will evolve as benefits are continuously enhanced as a result of scientific research. You will be notified timeously of all changes. By activating the Vitality Active Rewards with Apple Watch benefit, you give Discovery Vitality permission to debit monthly penalty amounts from the Discovery Card account straight facility linked to the benefit. To ensure that Discovery Vitality has time to calculate all your weekly goals in a month, you give Discovery Vitality permission to debit the Discovery Card linked to the benefit, if applicable, on the 10th day of the following month, for 24 months. We will start calculating your weekly goals from the first full calendar month after the transferor collected the Apple Watch from the istore. The first debit, if applicable, will occur on the 10th of the next month. If the debit order date falls on a weekend or a public holiday, you agree that your Discovery Card linked to the benefit will be debited on that day. Debit orders will continue, if applicable, for 24 months or until you settle the full cost of your Apple Watch Series 3 Sport 38mm or Apple Watch Series 4 Sport 40mm. Please note that the penalty payment date for this benefit may be different to the usual debit order date for the Discovery Card account. When you activate this benefit, you agree that all payment instructions from Discovery Vitality may be treated by your bank as if the instruction has been issued by you. It is your responsibility to ensure you have enough funds in your straight facility for the non-refundable transfer fee of R600 and the monthly penalty amount, if any, to be debited. If the debit order is unsuccessful on the 10th of each month, you will be sent an email or SMS. You will be liable for any related charges or fees. Please make sure you have enough funds in the straight facility so Discovery Vitality can try again on the 27th and the last working day of the month. You ll need to allow for any necessary clearance periods. If the second attempt fails, you will be regarded as being in default and a default notice will be sent to your address on record. If a debit order is unsuccessful, Discovery Vitality will stop calculating your Vitality Active Rewards goals that count toward your Vitality Active Rewards with Apple Watch benefit. Once you have paid all amounts that are in arrears Vitality will start calculating these goals again. Any goals achieved during the period that you were in arrears will not be retrospectively allocated once you bring your account up to date. Any default on monthly penalties may result in an additional debit the following month to include the defaulted month s penalty debit amount. If Discovery Vitality is unable to collect the amount from your Discovery Card, the outstanding value of the Apple Watch Series 3 or Apple Watch Series 4 will be for your account and default interest will be charged on it. Default interest will be charged in line with the requirements of the National Credit Act (Act 34 of 2005). Returns or gifts Returns on Apple Watch will be governed by the distributor s policies. To view these, visit www.istoreonline.co.za If you return your Apple Watch, your R799 activation fee will not be refunded. If you gift your Apple Watch to someone else, your personal Vitality Active Rewards goals will still be used for this benefit. This means that if you don t meet your commitment to achieve all your weekly Vitality Active Rewards goals for 24 months, it will be considered a breach of your warranty and the Discovery Card account linked to the benefit will be debited monthly, as applicable. - 5 -

If you want to change your current Apple Watch to an Apple Watch Series 3 or Apple Watch Series 4, the following will apply: First, you must settle the remaining cost of your current Apple Watch with Discovery Vitality. If you choose not to settle the remaining cost on your current Apple Watch, the benefit will remain active and you will continue to pay the monthly penalty debits for the remainder of the 24 months. You must not have defaulted on any penalty debits with regards to your existing Apple Watch benefit. Once the cost is settled, the existing Apple Watch benefit will be cancelled. You will need to activate Vitality Active Rewards with Apple Watch for Apple Watch Series 3 or Apple Watch Series 4 and pay the R799 non-refundable activation fee. This is subject to passing the eligibility criteria. You can then use your proof of payment to collect your Apple Watch Series 3 or Apple Watch Series 4 at the istore. Transfers Transfers can only occur once in a Vitality Active Rewards with Apple Watch benefit cycle within 60 days of the original collection of the Apple Watch from the istore by the transferor. Trade-in If you wish to trade in your current Apple Watch, please contact istore directly. Loss or damages Should your Apple Watch be lost, stolen or damaged, the Discovery Card account linked to the benefit will still be liable for any Vitality Active Rewards with Apple Watch penalty debits incurred for the remainder of the benefit. Product warranties are subject to istores and Apple rules. Ending the benefit, downgrades and re-instatements If you do not collect your Apple Watch within three months of activation, your proof of payment will expire (see Use your proof of payment to collect your Apple Watch above). Your Vitality Active Rewards with Apple Watch benefit will be cancelled if: You cancel your benefit, your Vitality policy, your Vitality Active Rewards benefit, or your qualifying Discovery Card. You downgrade to a Vitality policy or a Discovery Card that does not cater for this benefit, like a KeyFIT policy or a Discovery Blue Card. You reduce the straight limit of your Discovery Card to lower than the limit required to be eligible for this benefit. Your qualifying Discovery Card is no longer in good standing.* * Your Discovery Card account and Discovery Card is in good standing if it is open, active and not in arrears for more than three months. Your account is not in good standing if you need to undergo debt counselling. If your benefit is cancelled, your R799 activation fee will not be refunded and you cannot return your Apple Watch to Discovery Vitality. You will then become liable for the full remaining cost of the Apple Watch Series 3 Sport 38mm or Apple Watch Series 4 Sport 40mm which Discovery Vitality will collect from your qualifying Discovery Card. If you cancel your Apple Watch debit order with your bank, you are still liable for the full remaining cost and you cannot claim back any amounts that have already been debited. Sharing of information The transferee agrees that Discovery Vitality and istores in South Africa may share your personal information in order to administer the benefit.
