Surrender Charges Per $1,000 of face amount during the first 19 years, and for 19 years from the effective date of any face amount increase. Surrender charges will go past age 100, if applicable. Withdrawals 5 Partial Withdrawals Minimum of $250, and at least $500 of net surrender value must remain after a withdrawal. A $25 withdrawal charge applies to each withdrawal. Loans Preferred: During the surrender charge period, available on gain. Charged and credited 3.00% for zero net cost. After surrender period, all loans are considered preferred. The minimum amount of increase is $10,000. Current policy fee will not change bands for face increases. Decreases become effective on or after the monthly activity day the request is approved. The policy fee is based in the total of base and PTR after the decrease. The minimum face amount in effect after any decrease must be at least equal to the age based minimum face 3
amount requirement based on the attained age. Monthly per unit charges will be imposed for elective increases and charges will vary by band. Riders 1,6 Accelerated Death Benefit - (Terminal Illness) (UL0619) 7,8,9 Issue ages: All Fees: There is no cost for this rider; however, the amount paid by the rider will be reduced by the benefit discounts resulting from early payments, loans, uncollected insurance charges, and administrative fees. This rider provides for an advance of a portion of the death benefit if the insured is diagnosed with a terminal illness and satisfactory proof of the terminal illness is provided. A terminal illness is a medical condition of the insured that, not withstanding appropriate medical care, will result in death within 12 months. Minimum payout is $5,000. Maximum payout is the lesser of: 80% of the death benefit as of the date the first request is paid; or, $250,000, including all other accelerated benefit amounts paid under all policies issued on the life of the insured. Accelerated Death Benefit - (Permanent Confinement) (UL0416) 7,8 Issue Ages: All Fees: There is no cost for this rider; however, the amount paid by the rider will be reduced by the benefit discounts resulting from early payments, loans, uncollected insurance charges, and administrative fees. With the rider, a portion of the death benefit will be paid out in the event the insured has been confined to a nursing care facility for at least a year. And, in the last twelve months, the insured has been certified by a licensed health care practitioner as being unable to perform at least two activities of daily living without substantial assistance from another individual due to loss of functional capacity that is expected to be permanent, or the insured is requiring substantial supervision to be protected from threats to health and safety due to severe cognitive impairment. Activities of Daily Living include bathing, continence, dressing, eating, toileting and transferring. The minimum face amount remaining after a payment is $4,000 and a maximum of three payments may be made. Minimum payout is $5,000. Maximum payout is the lesser of: 80% of the death benefit as of the date the first request is paid; or, $250,000, including all other accelerated benefit amounts paid under all policies issued on the life of the insured. Accidental Death Benefit (UL0613) Issue ages: 18-60 (all classes) Fees: Rates vary by issue age. This rider pays an additional benefit amount to the beneficiary if the insured dies from accidental bodily injury as defined in the rider. The minimum coverage is $5,000 with a maximum coverage of the lesser of $250,000 of accidental death benefit coverage from all sources or total coverage of respective insured. Coverage terminates on anniversary following insured's 70th birthday. This rider can be added, deleted, increased, or decreased after issue. Additional Insured Rider (UL0703) 4 Issue Ages: 18-60 Preferred Elite 18-75 Preferred Nonsmoker 18-75 Standard Nonsmoker 18-75 Preferred Smoker 18-75 Standard Smoker 0-17 Juvenile Provides additional term insurance on the primary insured, the insured's spouse or grandchildren, brothers, children, parents and sisters. The minimum face amount is $50,000 and covers up to four additional insureds. The rider may be exchanged to any flexible premium adjustable life plan prior to the insured's age 75 and after the first policy year. Evidence of insurability is not required for exchange and death benefit must not exceed AIR amount or be less than $50,000 for new policy. Coverage terminates at the earlier of the additional Insured's age 99 or the policy anniversary following the base insured's 99th birthday. This rider can be added, deleted, increased (by increments of $10,000), or decreased (to $50,000) after issue. Children's Level Term Rider (UL0615) Base insured's ages: 18-60 (Preferred Elite), 18-64 (all other classes) Covered children issue ages: birth to 17 years (but not stillborn) This rider provides term insurance coverage for natural or legally adopted children of the insured. A stepchild of the primary insured may be covered if the child became a stepchild by reason of a marriage between the primary insured and one of the child's parents, and shares a residence with the primary insured at the time the rider is issued and who looks to the primary insured for support and maintenance. Each unit provides up to $5,000 of coverage for a flat monthly cost no matter how many children are eligible for coverage. Minimum coverage is $2,500(1/2 unit) with a maximum of $20,000 (4 units) for all CLTRs issued by Allstate and Lincoln 4
Benefit Life. Evidence of insurability is not required and each unit is convertible for up to five times the benefit amount in coverage. The rider may be exchanged for a new policy on each child's 25th birthday or at the base insured's age 65, if sooner. Note: This rider cannot be included on business-owned policies and if the policy is owned by a business at the time of an insured child's death, then the beneficiary of this rider benefit must be that child's estate. This rider can be added, deleted, increased, or decreased after issue. Continuation of Premium Rider (UL0614) Not available in California. Issue ages: 18-55 (all classes) Fees: Rates vary by issue age. Under this rider, Allstate will contribute a monthly amount to the policy value if the insured becomes totally disabled as defined in the rider. The maximum amount of coverage available is the lesser of two times the stipulated annual premium, or $10,000 a year. Coverage terminates on the policy anniversary following attainment of age 60. This rider can be added, deleted, increased, or decreased after issue. Full Surrender Charge Adjustment Rider (UL0202) Issue Ages: Available at all ages and underwriting classes. Fees: There is no charge for this rider. This rider must be chosen at issue (requires home office approval) and cannot be added after initial issue. Rider cannot be deleted once it has been elected and the policy issued. This rider provides for a complete waiver of surrender charges during the first five policy years on all business related cases that have planned first year premiums of $50,000 or more. Maximum loan and maximum partial withdrawal amounts will use the surrender value with full surrender charges. Reduction in surrender charges applies on a full surrender only; partial withdrawal charges are not reduced. Prime Term Rider (UL0702) Issue Ages: 18-60 Preferred Elite 18-75 Preferred Nonsmoker 18-75 Standard Nonsmoker 18-75 Preferred Smoker 18-75 Standard Smoker Fees: Rates vary by underwriting class, issue age, and gender. Provides level term insurance protection on the primary insured. Available at the time of policy issue only with a minimum coverage of $25,000 and a maximum coverage of three times the amount of the base plan. May be exchanged prior to the insured's age 75 for any flexible premium adjustable life plan sold by Lincoln Benefit Life or for additional permanent coverage under the base policy on or after the first anniversary. Coverage terminates at the insured's age 99. This rider may only be deleted, exchanged, or decreased after issue. Guaranteed Insurability Rider (UL 0706) Issue ages: 0-38 (standard or better rate classes Fees: Per $1000 charge This rider provides the opportunity to add additional insurance without underwriting requirements at preset dates. The additions are available on the anniversaries following the attainment of ages 25, 28, 31, 34, 37 and 40. Unscheduled increases are also allowed at life events including birth, marriage and adoption. Insured has 90 days after qualifying life event to request an increase and election of an unscheduled increase due to a life event results in forfeiting the next scheduled increase. The minimum increase amount is $10,000 with a maximum increase amount equal to the Guaranteed Insurability Rider (GIR) coverage amount purchased at issue not to exceed $50,000. Coverage terminates on anniversary following insured's 40th birthday. This rider cannot be added, increased, or decreased after issue. This rider can be deleted after issue. Not available on non-underwritten term conversion business. Overloan Protection Rider (UL0701) Issue Ages: All This rider will be issued with all contracts at time of issue (subject to state approval). This rider, when exercised, prevents the underlying policy from lapsing in the event policy loans exceed the policy value. When the rider is exercised, there is a one-time charge of no more than 4.5% of the policy value. Upon exercise, the policy becomes paid-up. At that point, all charges will be waived and no further premiums will be allowed, nor will any changes be allowed to the policy. Loan repayments will be allowed, and loan interest will continue to be charged. Customer must request activation of this rider in writing. All of the following conditions must be met to exercise rider: Inforce at least 15 years 5

Approval of this request will be based on a meeting the underwriting criteria for that rate class. Partial withdrawals and surrenders from universal life policies are generally taxed as ordinary income to the extent the withdrawal exceeds your investment in the contract, which is also called the "basis". In some situations, partial withdrawals during the first 15 policy years may result in taxable income prior to recovery of the investment in the contract. Loans are generally not taxable if taken from a universal life policy that is not a modified endowment contract. However, when cash values are used to repay a loan, the transaction is treated like a withdrawal and taxed accordingly. If a policy is a modified endowment contract, loans are treated as a taxable distribution to the extent of the policy gain. Loans, withdrawals and surrenders are treated first as distributions of the policy gain subject to ordinary income taxation, and may be subject to an additional 10% federal tax penalty if made prior to age 59 1/2. Loans, if not repaid, and withdrawals reduce the contract's death benefit and cash value. 6 See rider for terms, conditions and restrictions. Additional costs may apply. Riders and waivers may not be available in all states. 7 Accelerated Death Benefit may or may not qualify for favorable tax treatment under the Internal Revenue Code of 1986. This depends on factors such as the customer's life expectancy at the time benefits are accelerated or whether the customer uses the benefits to pay for required qualified long-term care expenses. If the Accelerated Death Benefit qualifies, the Benefit will not be subject to federal income taxation. In addition, receipt of the Accelerated Death Benefit may affect the customer or his family's eligibility for public assistance programs such as medical assistance (Medicaid), Aid to Families with Dependent Children (AFDC), Supplemental Security Income. 8 This rider is available at no additional cost. However, the amount paid by the rider will be reduced by benefit discounts resulting from early payment, loans and uncollected insurance charges. The $250,000 maximum includes all other accelerated death benefits paid under contracts issued by Lincoln Benefit Life Company on the life of the insured. Also, you may be assessed an administrative fee for each accelerated benefit request as provided by the terms of the rider. 9 In the states of IL, KS, MA and WA, terminal illness is defined as a life expectancy of less than 24 months.
