B: PLANNING APPLICATION FORM Instructions: VLCs will complete this form in its entirety including the attestation letter and submit with required attachments to cohorts@carecompassnetwork.org for consideration. VALUE-BASED PAYMENT LEAD CONTRACTOR (VLC) LEGAL NAME OF THE NETWORK LEAD (VLC): Must be a Safety Net organization with the Heartwood Hospital, Inc. ability to administer a Network. COHORT POPULATION TO BE SERVED GENERAL DESCRIPTION: Provide diagnostic criteria (if known) and any other defining characteristics of the cohort. Medicaid Members, ages 30-31 with 20+ ED Visits per Medicaid member per year TARGET SIZE: Must be between 50-200 Medicaid members 58 VLC S ABILITY TO ADMINISTER NETWORK Describe the VLC s experience and/or ability in each of the following areas. FLOW FUNDS: Distributes funds to one or more organizations for purposes of achieving a shared goal(s). Heartwood Hospital, Inc. has flowed funds to community partners for other collaborative work for the last 5 years. Legal processes are in place to review necessary changes to existing agreements and additions of new community partners. The Forestland Network has met to determine distribution of funds and is in the process of finalizing negotiations, at which time all Forestland Network partners will have agreed upon the basis and frequency of payment and completed Form C. ESTABLISH AND MAINTAIN LEGAL ARRANGEMENTS: Enters into one or more legal contracts with another party, providing the legal oversight for stated agreements, either directly or through a contractual arrangement. Heartwood Hospital, Inc. has engaged payors including Medicaid Managed Care Organizations through the execution payor agreements and has experience in executing other types of legal agreements. Legal agreements with community partners follow a template format vetted by the Heartwood Hospital s (VLC) Legal Counsel. MANAGE THE NETWORK: Regularly monitors performance of partners in the network through reporting and analytics. Implements performance improvement or quality control measures to improve network performance. As the referring agency for the network, Heartwood Hospital, Inc. is best suited to manage the network and is naturally a convener for identifying potential network members. Since the network will aim to center around the patients mentioned above, the information will come from Heartwood Hospital, Inc. and then be relayed to community partners. We hope to establish a way to do this realtime in the future. In the meantime, we will be meeting at regular intervals to discuss the care of the patients referred from Heartwood Hospital, Inc. to our network partners. Page 2 of 5

REPORT TO CCN: Submits report(s) through the appropriate channels to meet Milestones and/or speed and scale commitments as defined by Phase III contracting. Since Phase I of the Care Compass Network contract roll-out, Heartwood Hospital has reported on a regular basis to CCN, utilizing PPS templates to capture data from reports out of our EHR. SUPPORT EXCHANGE OF DATA: Collaborates with partners to integrate population health data, following all HIPAA requirements. Heartwood Hospital Inc will enter into a BAA agreement with network partners. A template BAA has been distributed to all potential network partners in order to enable data exchange. Logistically, this will occur at regular intervals when meeting with network partners to discuss patients referred until a real-time solution can be identified. MONITOR PERFORMANCE OF THE COHORT: Regularly monitors patient-level data to identify opportunities to improve quality, reduce cost of care and improve coordination of care and services. Heartwood Hospital, Inc. already monitors performance metrics. To calculate metrics for the specific population, a data analyst will be assigned a refresh this information once per week which will be reviewed at network meetings. The network will track how often cohort patients are in the ER, and will assess percentage of their visits are avoidable, and pre-post utilization indices to determine the success of the program. NETWORK PARTNERS List the legal names of all agencies/organizations in the Network and what role they serve. VLC Heartwood Hospital TIER 1 CBO TBD. We will be pursuing a Tier 1 CBO in the coming weeks. REFERRAL SOURCE OTHER TBD. We will be pursuing additional partners in the coming weeks. Heartwood Hospital, Inc agrees to serve as the lead agency, or Value-Based Payment Lead Contractor (VLC), in the above outlined Network; is a Safety Net Provider and understands and accepts the requirements as follows: a. Receive and distribute funds to Network Partners. As the VLC, we will work with our Network Partners, as defined in the Cohort Management Program documents, to determine how best to use the Cohort Management Funds in a mutually agreeable manner to the Network Partners. b. Communicate with CCN on behalf of the Network. c. Coordinate communication for the Network. 2.
