Verification Information Verification is the process Midwestern University uses to confirm that the data reported on the Free Application for Federal Student Aid (FAFSA) is accurate when a student s file has been selected for review by the Department of Education. Midwestern University has the authority to contact students for documentation that supports income and other information that was reported. All verification items requested are specified in the Verification Worksheet emailed to selected students. Please see the attached examples of verification worksheets.

2015 2016 Verification Worksheet (V1: Standard) Your 2015 2016 Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification. The law says that before awarding Federal Student Aid, we may ask you to confirm the information you reported on your FAFSA. To verify that you provided correct information, your Financial Aid Administrator will compare your FAFSA with the information on this worksheet and with any other required documents. If there are differences, your FAFSA information may need to be corrected. You must complete and sign this worksheet, attach any required documents, and submit the form and other required documents to the Office of Student Financial Services. We may ask for additional information. If you have questions about verification, contact our office as soon as possible so that your financial aid will not be delayed. A. C. STUDENT S INCOME INFORMATION TO BE VERIFIED (Independent) 1. TAX RETURN FILERS Complete this section if you and spouse (if married) filed or will file a 2014 income tax return with the IRS. Important Note: Notify Student Financial Services if you or your spouse filed separate IRS income tax returns for 2014 or had a change in marital status after December 31, 2014. The best way to verify income is by using the IRS Data Retrieval Tool that is part of FAFSA on the Web at FAFSA.gov. Follow the instructions to determine if you are eligible to use the IRS Data Retrieval Tool to transfer 2014 IRS income tax information into your FAFSA. It takes 2-3 weeks for the IRS income information to be available for electronic IRS tax return filers, and 8-11 weeks for paper IRS tax return filers. If you need more information about when or how to use the IRS Data Retrieval Tool see your Financial Aid Administrator. If you (or spouse) filed or will file an amended 2014 IRS tax return, see page 3. STUDENT INFORMATION- Check the box that applies: I have used the IRS Data Retrieval Tool in FAFSA on the Web to transfer my (and spouse if married) 2014 IRS income information into my FAFSA. I have not yet used the IRS Data Retrieval Tool, but I will use the tool to transfer my (and spouse if married) 2014 IRS income information into my FAFSA once I have filed my 2014 IRS tax return. We cannot complete the verification process until your (and spouse if married) IRS information has been transferred into your FAFSA. I am unable or choose not to use the IRS Data Retrieval Tool in FAFSA on the Web, and I will submit to Student Financial Services my 2014 IRS tax return transcript(s) of the income tax return. You will need your Social Security Number, date of birth, and the address on file with the IRS (normally this will be the address used when your 2014 IRS tax return was filed). If you are married and you and your spouse filed separate 2014 tax returns, you must submit tax return transcripts for both you and your spouse. A 2014 IRS Tax Return Transcript may be obtained through the: Online Request - Go to www.irs.gov, under the Tools heading on the IRS homepage, click "Get Transcript of Your Tax Records." Click Get Transcript ONLINE or Get Transcript by MAIL. Make sure to request the IRS Tax Return Transcript and NOT the IRS Tax Account Transcript. Check here if a 2014 IRS tax return transcript(s) will be provided later. Verification cannot be completed until the IRS tax return transcript(s) has been submitted. 2. TAX RETURN NONFILERS Complete this section if you (and spouse if married), will not file and are not required to file a 2014 income tax return with the IRS. Check the box that applies: I (and spouse if married) was not employed and had no income earned from work in 2014. I (and spouse if married) was employed in 2014 and have listed below the names of all employers, the amount earned from each employer in 2014, and whether an IRS W-2 form is attached. (Attach copies of all 2014 W-2 forms issued to you (and spouse if married) by employers. List every employer even if the employer did not issue an IRS W-2 form. The following information is a guide if you were either: Granted a filing extension by the IRS, filed an amended IRS tax return, were a victim of tax administration identity theft, or filed a non-irs income tax return. Individuals Granted a Filing Extension by the IRS If you are required to file a 2014 IRS income tax return and have been granted a filing extension by the IRS, provide the following documents: A copy of IRS Form 4868 Application for Automatic Extension of Time to File U.S. Individual Income Tax Return, that was filed with the IRS for tax year 2014; A copy of IRS' Approval of an Extension Beyond the Automatic Six-Month Extension if you requested an additional extension of the filing time for tax year 2014; and A copy of IRS Form W 2 for each source of employment income received for tax year 2014 and, if self-employed, a signed statement certifying the amount of your Adjusted Gross Income (AGI) and the U.S. income tax paid for 2014. Individuals Who Filed an Amended IRS Income Tax Return If you have filed an amended IRS income tax return for tax year 2014, provide both of the following two (2) items: A signed copy of the original IRS income tax return filed with the IRS or an IRS Tax Return Transcript for the 2014 tax year; and A signed copy of the 2014 IRS Form 1040X, Amended U.S. Individual Income Tax Return, that was filed with the IRS. Individuals Who Were Victims of Tax Administration Identity Theft A victim of tax administration identity theft who is not able to obtain a 2014 IRS Tax Return Transcript or use the IRS DRT must contact the IRS at 1-800-908-4490. Upon authentication of your identity, the IRS will provide, by U.S. Postal Service, a printout of your 2014 IRS income tax return information. Individuals Who Filed Non-IRS Income Tax Returns If you have filed or will file a 2014 income tax return with Puerto Rico, another U.S. territory (e.g., Guam, American Samoa, the U.S. Virgin Islands, the Northern Marianas Islands), or with a foreign country, you must provide: A signed copy of that 2014 income tax return(s); or A transcript obtained from a government of a U.S. territory or commonwealth, or a foreign central government that includes all of the tax filer s income and tax information required to be verified for tax year 2014. D. Other Information to Be Verified Answer each question below as it applies to you (and your spouse, if married). 1. Receipt of SNAP Benefits I certify that, a member of the student s household, received benefits from the Supplemental Nutrition Assistance Program (SNAP) sometime during 2013 or 2014. SNAP may be known by another name in some states. For assistance in determining the name used in a state, please call 1-800-4FED-AID (1-800-433-3243). The student s household includes: The student. The student s spouse, if married. The student s or spouse s children if the student or spouse will provide more than half of the children s support from July 1, 2015, through June 30, 2016, even if the children do not live with the student. Other people if they now live with the student and the student or spouse provides more than half of the other people s support and will continue to provide more than half of their support through June 30, 2016. Note: If we have reason to believe that the information regarding the receipt of SNAP benefits is inaccurate, we may require documentation from the agency that issued the SNAP benefits in 2013 or 2014.

2. Child Support Paid Complete this section if you (or spouse if married) paid child support in 2014. Instructions: If the student and/or spouse, who is a member of the student s household, paid child support in 2014, provide in the space below the names of the persons who paid the child support, the names of the persons to whom the child support was paid, the names and ages of the children for whom the child support was paid, and the total annual amount of child support that was paid in 2014 for each child. If you need more space, attach a separate page that includes your name and Student ID Number at the top. High School Completion Status (You must complete) You must provide one of the following documents that indicate your high school completion status before beginning college in 2015-2016: Copy of high school diploma A copy of your final official high school transcript showing the completion date when your diploma was awarded. A state certificate or transcript received by a student after the student passed a State-authorized examination (GED test, HiSET, TASC, or other State-authorized examination) that the State recognizes as the equivalent of a high school diploma. For students who completed secondary education in a foreign country, a copy of the secondary school leaving certificate or other similar document An academic transcript showing successful completion of at least a two-year program fully acceptable toward a bachelor s degree. If you re a homeschooled student and from a state where state law requires you to obtain a secondary school completion credential for homeschool (other than a high school diploma or its recognized equivalent), a copy of that credential. If you re a homeschooled student and from a state where state law does not require you to obtain a secondary school completion credential for homeschool (other than a high school diploma or its recognized equivalent), a transcript or the equivalent, signed by your parent or guardian, that lists the secondary school courses you completed and documents the successful completion of a secondary school education in a homeschool setting. Note: Any student unable to obtain the documentation listed above should contact the financial aid office.

E. Identity and Statement of Educational Purpose Identity and Statement of Educational Purpose (To Be Signed at the Institution) The student must appear in person at Midwestern University to verify his or her identity by presenting a valid government-issued photo identification (ID), such as, but not limited to, a driver s license, other state-issued ID, or passport. The institution will maintain a copy of the student s photo ID that is annotated by the institution with the date it was received and reviewed and the name of the official at the institution authorized to collect the student s ID. In addition, the student must sign, in the presence of the institutional official, the Statement of Educational Purpose provided below. Identity and Statement of Educational Purpose (To Be Signed in the Presence of a Notary) If the student is unable to appear in person at Midwestern University to verify his or her identity, the student must provide: (a) A copy of the valid government-issued photo identification (ID) that is acknowledged in the notary statement below or that is presented to a notary, such as, but not limited to, a driver s license, other state-issued ID, or passport; and (b) The original Statement of Educational Purpose, which is provided below, must be notarized. If the notary statement appears on a separate page than the Statement of Educational Purpose, there must be a clear indication that the Statement of Educational Purpose was the document notarized.
