GETTING READY FOR BIND Meet Priyah, Josh, Vanessa, and Marco. They are Medtronic employees who plan to enroll in a medical plan in November. Get to know their personal situations and what they consider as they think about their options and how Bind could work for them. MEET PRIYAH MEET JOSH MEET VANESSA MEET MARCO NEXT

GETTING SET TO MAKE YOUR ENROLLMENT DECISIONS Priyah, Josh, Vanessa, and Marco are back, and they are getting set to make their enrollment decisions. First, let s see what the four have learned about the new Bind medical plan so far: It s mobile-friendly With Bind, you can use the Bind app or website to get the details you need fast check your coverage for specific procedures, request a virtual visit, find copay costs, and more. There s an out-of-pocket maximum Sometimes the frequency of your plan use and the associated costs vary from year to year. If you have a year of high medical expenses, there s an out-of-pocket maximum that protects you against significant medical costs. Free preventive care Like our other medical plans, all preventive care is covered at 100%, which means there is no cost to you. Core and prescription drug coverage Core coverage in the Bind plan ensures the care most people need is always covered. That means when you or any family member on your plan needs to see their primary doctor, get a prescription filled, have a baby, or go to the emergency room, they re covered by Core coverage. Add-In coverage If you or any covered family member has a medical issue that requires a planned procedure outside of Core coverage, such as knee replacement surgery, you can add coverage for that specific procedure. Add-In coverage can be purchased at any time, which means you re only paying for the coverage you need when you need it. Health Care Flexible Spending Account (FSA) compatible If you enroll in the Bind plan, you can also contribute to a Health Care FSA if you want. This type of account provides you a pre-tax option to pay for your healthcare expenses, including the copays associated with the Bind plan. With these features in mind, how does Bind compare to the other medical plan options for Priyah, Josh, Vanessa, and Marco as they think about their personal situation? BACK 2 NEXT

MEET PRIYAH Single with no children Healthy Has student loan debt Tech-savvy Needs a new doctor Priyah is a recent college graduate and started her career as a Surgical Sales Representative in Sarasota, Florida last year. She is single and has no children. She has one or two doctor visits annually, is healthy, and loves to exercise. Here s what s top of mind for her: Student loan debt and a goal to move into her own apartment within the next year or two. Every dollar counts! The value of having good healthcare coverage especially for the unexpected crisis but doesn t want to pay for benefits she doesn t use. Given her sales role, she is always on the go, whether it s on the road or in hospitals. She d prefer a mobile-friendly tool and the ability to speak to an expert when she needs help. Since Priyah is new to Sarasota, she needs to find a doctor for regular check-ups and any health needs that come up during the year. WHY BIND COULD WORK FOR PRIYAH: The on demand approach of the Bind plan may be ideal for Priyah. She s looking for the most affordable option to meet her limited healthcare needs. Many aspects of the Bind experience also match how Priyah likes to purchase and use other services. Stay tuned to learn how Priyah compares her options leading up to open enrollment. BACK 3 NEXT

As Priyah learns more about Bind, she starts to think about how much it might cost compared to her other Medtronic medical plan options. Priyah knows that part of the cost for her healthcare coverage comes in the form of regular paycheck contributions. But, Priyah also knows just looking at paycheck contributions alone won t provide an accurate cost estimate for the entire year. She starts to think more about how she might use her medical plan in 2019 and begins to compare the Bind plan with the Consumer Health Plan (CHP) with Health Savings Account (HSA). Here are some services she typically incurs every year: PRIYAH GETS PREPARED Priyah Compares with ALEX Now that Priyah understands what she ll pay for care under Bind, she decides to use ALEX to take her decision-making process to a new level. Not only will ALEX do the rest of the math for her, but it will also help her compare her options and costs across all of her available plans. SERVICE Annual physical/well-woman exam Doctor s visit with a primary care physician for a sinus infection Urgent care visit Dermatologist visit One 90-day Tier 1 prescription for her skin condition using a Choice Rx pharmacy Virtual visit for migraine headache symptoms TOTAL COSTS FOR COVERAGE AND CARE COST UNDER THE BIND PLAN FREE because it is a preventive service $40 copay $100 copay $60 copay $10 copay FREE $210 IN COPAYS + PAYCHECK CONTRIBUTIONS BACK 4 NEXT

MEET JOSH Married with two sons Active family On-the-go Back pain issues needing possible surgery Josh has been working as a Senior Manufacturing Director in Boulder, Colorado for 10 years. He s married with two very active sons, ages 8 and 10, who are in a traveling soccer league. Josh knows that finding great medical coverage is an important part of maintaining his family s sporty lifestyle. Here s what he considers: Sometimes a bad flu season means many doctor visits for the family. Josh is always on the go. Whether it s at the office or at a soccer practice, he needs coverage and provider information at his fingertips. Josh s family loves to be outdoors and is very athletic. They need good coverage for potential injuries that could arise with two boys that may even need a trip to the emergency room. He s experienced a lot of back pain lately, and his doctor has been suggesting a surgery option. He needs to be prepared, just in case he ll need an operation. WHY BIND COULD WORK FOR JOSH: The flexibility of Bind could be just what Josh s active family needs. It also offers him protection and coverage for any injuries, illnesses, or accidents that pop up throughout the year. In the event Josh decides to move forward with his back surgery, he also has the ability to buy Add-In coverage for that at any time during the year. He doesn t have to make that decision during open enrollment. If Josh s situation sounds a lot like your own, there s more to follow on his coverage selection journey. BACK 5 NEXT

Josh has time between his son s soccer games to sit down and think about enrollment. First, he evaluates his future paycheck contributions and notices the Bind plan is the least expensive option for the family coverage he needs. You may remember Josh s back issues, and that he s been having conversations with his doctor about spinal fusion surgery. If he and his doctor decide to go forward with the procedure next year, Josh can buy the Add-In coverage right before the surgery happens. He doesn t need to decide during open enrollment if he ll have back surgery, so he s comforted knowing he can add coverage at any point during 2019 should he need it. Last year, Josh s family had a lot of medical expenses that added up quickly. He knows that getting the full picture means both looking at paycheck costs and estimating the potential costs of care. Josh begins to think through some of the services his active family might need next year and compares the Bind plan to the CHP he s currently enrolled in: JOSH JUGGLES HIS OPTIONS Josh Compares with ALEX Now that Josh understands what he ll pay for care under Bind, he decides to use ALEX to take his decision-making process to a new level. Not only will ALEX do the rest of the math for him, but it will also help him compare his options and costs across all of his available plans. SERVICE Four doctor s visits for the flu Three Tier 1 prescriptions for antibiotics using a Choice Rx pharmacy Three urgent care visits for sports injuries One emergency room visit for a broken bone Annual physicals for the whole family Six virtual visits for various ailments Five physical therapy visits to help back TOTAL COSTS FOR COVERAGE AND CARE COST UNDER THE BIND PLAN 4 x $40 = $160 in copays 3 x $5= $15 in copays 3 x $100 = $300 in copays $300 copay FREE because they are preventive services FREE 5 x $40 = $200 in copays $975 IN COPAYS + PAYCHECK CONTRIBUTIONS If or when Josh decides to get back surgery, his total coverage cost for the surgery would be $2,750. And, he doesn t have to pay all of this amount at once. Because Josh chooses a high value provider, his Add-In coverage includes an additional copay to the provider of $150 and a set number of ongoing, pre-tax payroll deductions of $100 for 26 paychecks, so Josh can pay for the Add-In coverage over time. MEET VANESSA Pregnant with first child Husband is on his own insurance Seeking a simple medical plan Vanessa works in IT at Mounds View, Minnesota. She and her husband are expecting their first child. Having a good medical plan is on the top of Vanessa s pre-baby checklist. Here s why: Loves her current doctor Reading all the what to expect baby books makes Vanessa worried about the what ifs. With her husband on his own insurance, Vanessa needs an option that will protect her family and new baby for whatever life could throw their way. Vanessa is a planner and wants to make transitioning to a family of three easy. Between picking out nursery furniture to finding a daycare, she needs an easy way to read the details on what her medical insurance covers. Vanessa knows how a great relationship with a doctor is hard to find. She s finally found one who gets her and wants to keep that relationship going as long as possible. With maternity delivery on the horizon, she knows that a hospital stay will be in store. WHY BIND COULD WORK FOR VANESSA: Starting a family is an exciting time in life, but may require a lot of planning. The simplicity of Bind along with its comprehensive Core coverage, including maternity care, could appeal to Vanessa as she welcomes her first child. Stay connected to see what choices Vanessa will make this open enrollment. BACK 7 NEXT

Vanessa s getting all of the final preparations ready for her baby s arrival, and that includes choosing a medical plan that will cover not only the birth of her baby but then her baby s healthcare needs after she s born. With all of the costs around having a baby on her mind, Vanessa begins to map out a comparison of her paycheck contributions. Vanessa likes the idea of saving all that money in paycheck contributions because she s going to need to start buying a lot of diapers. But she knows her annual cost of coverage goes beyond paycheck contributions. Vanessa looks to see how her healthcare costs will add up with an upcoming hospital stay and appointments for her and her new baby for both the Bind plan and the PPO plan: VANESSA VALUES HER COST AND COVERAGE Vanessa Compares with ALEX Now that Vanessa understands what she ll pay for care under Bind, she decides to use ALEX to take her decision-making process to a new level. Not only will ALEX do the rest of the math for her, but it will also help her compare her options and costs across all of her available plans. SERVICE Hospital visit for labor and delivery using a high value facility Annual physical and well-baby visits Eight prenatal visits Four primary care visits Two visits to the doctor for baby illness Two Tier 1 prescriptions for baby using a Choice Rx pharmacy Two virtual visits for various ailments TOTAL COSTS FOR COVERAGE AND CARE COST UNDER THE BIND PLAN $800 copay FREE because they are preventive services FREE because they are preventive services 4 x $40 = $160 in copays 2 x $40 = $80 in copays 2 x $5 = $10 in copays FREE $1,050 IN COPAYS + PAYCHECK CONTRIBUTIONS Maternity Care Everywhere Regardless of which plan Vanessa chooses, she has access to a maternity support program. Vanessa reads the Parental Time Off Policy on the policy portal for all the details and to understand the requirements. BACK 8 NEXT

MEET MARCO Married Seasoned with how medical plans work Doesn t have much free time Needs preventive care Diabetic Marco works in Research and Development at Rice Creek, Minnesota. He loves his job and the network of friends and colleagues he s established throughout his career. Marco s been through two decades of open enrollment seasons, but every year he and his wife compare options to choose the plan that meets their needs and lifestyle. Here s what Marco thinks about when he compares his options: Marco works many hours. After work, he would much rather go on a long walk with his dog than spend time crunching numbers on his medical coverage. Marco s wife has a family history of breast cancer, so they both know how important preventive care can be. It can save lives. A few years ago, Marco was diagnosed with diabetes. Due to this condition, he needs coverage that allows him to get the treatment he needs to manage his condition and overall health. Marco wants easy access to information so that he can quickly compare his options and choose what is best for him and his wife. WHY BIND COULD WORK FOR MARCO: Marco is experienced in choosing his Medtronic healthcare coverage, but he s always wanted to try something different. Not only does Marco think Bind is clear and understandable, but it covers what matters most, and leaves him more time to spend with his family. More to follow with Marco as he and his wife review their coverage options this open enrollment. BACK 9 NEXT

Marco is an engineer and number crunching comes second nature to him. Knowing enrollment decisions take time, he and his wife start to think about open enrollment in early fall every year. They have been with the PPO plan for years and are comfortable with how it works, but would like to understand the new Bind option. After comparing contribution costs, Marco discovers the Bind plan would be the less expensive medical plan from a paycheck contribution perspective. As an informed healthcare consumer, Marco and his wife don t forget about the cost of care. Together, they make a list of common medical services they use every year: MARCO DOES THE MATH Marco Compares with ALEX Now that Marco understands what he ll pay for care under Bind, he decides to use ALEX to take his decision-making process to a new level. Not only will ALEX do the rest of the math for him, but it will also help him compare his options and costs across all of his available plans.
