welcome to annual open enrollment Your annual open enrollment period begins Monday, November 9, 2009, at 8:00 a.m., Eastern Time (ET), and ends Friday, November 20, 2009, at 6:00 p.m., ET. During this annual open enrollment period, you can enroll in or change your 2010 elections for these coverages (if eligible): Medical and prescription drug Dental Use this step-by-step Quick Start Guide to walk yourself through enrollment, from start to finish. Spending just a few extra minutes reviewing this now might save you time later! Life Insurance REMEMBER...LATE ENROLLMENTS WILL NOT BE ALLOWED While you can change and confirm your elections as often as you want during the annual open enrollment period, your enrollment deadline Friday, November 20, 2009, at 6:00 p.m., ET will not be extended. The enrollment process is easy It usually takes just 15 minutes or less! Follow the steps outlined under What You Need to Do on the following pages. 1

what you need to do 1STEP ONE Review Your Personalized Coverage Details Last month, you started getting ready for annual open enrollment by saving the dates for enrollment. You may also have requested your password for the Your Benefits Resources Web site at http://resources.hewitt.com/alcatel-lucent (which is the same password to use for the Alcatel-Lucent Benefits Center). Now, it s time to understand your 2010 benefits options. Everything you need to make a coverage decision and enroll or make changes to your default coverage during the annual open enrollment period is contained in this kit. Hint: Review your default coverage first. Then determine if you need to take action by reviewing the bullets under Step 3 on page 3 of this Quick Start Guide. If no further action is required, you re finished! Be sure you have everything you need If you are Medicare-eligible and currently enrolled in SecureHorizons MedicareDirect Retiree Plan (PFFS), a Private Fee-For-Service retiree plan, an Annual Notice of Change (ANOC) was mailed to you in October from SecureHorizons. This information outlined coverage levels and other important notices about the SecureHorizons MedicareDirect Retiree Plan (PFFS) for 2010. If you didn t receive the ANOC, or are not currently enrolled but are considering the PFFS for 2010, request information by calling the SecureHorizons MedicareDirect Retiree Plan (PFFS) at 1-888-980-8117 (TTY: 711), from 8:00 a.m. to 8:00 p.m., local time, seven days a week. 2

2STEP TWO Think About Your Family s Coverage Needs Ask yourself: How do the 2010 changes impact me and/or my eligible dependent(s) (see What s Changing for 2010)? If I don t take action to enroll or make changes to my default coverage during the annual open enrollment period, will the default coverage meet my health and financial needs, and those of my family, next year? Do I have other coverage options that may be a better fit? Are the dependent(s) whom I m planning to cover eligible for coverage? Hint: Review Who Is Eligible on page 3 of the Benefits 2010: Information and Action Guide. 3STEP THREE Enroll or Make Coverage Changes, If You Need to (Before the End of the Annual Open Enrollment Period) Once you review your personalized details and consider all of your options, follow the instructions on how to use the Your Benefits Resources Web site at http://resources.hewitt.com/alcatel-lucent on pages 7 through 8 of the Benefits 2010: Information and Action Guide (the fastest way!), or call the Alcatel-Lucent Benefits Center at 1-888-232-4111 to enroll or make coverage elections during the annual open enrollment period. Still not sure IF YOU NEED TO TAKE FURTHER ACTION? You must take action, if you want to: Choose coverage other than your default coverage for 2010 (as shown on your personalized enrollment worksheet and the Your Benefits Resources Web site during the annual open enrollment period) Add or remove dependent(s) from coverage Re-enroll non-grandfathered class II dependent(s) (these elections do not roll over each year) Enroll in an Enhanced or Standard Point of Service (POS) medical option, if you are not eligible for Medicare and an Enhanced or Standard POS option is not shown on your personalized enrollment worksheet in this enrollment kit (see page 10 of the Benefits 2010: Information and Action Guide for more information) 3 If any of these situations apply to you and you do not take action during the annual open enrollment period, you will receive the default coverage shown on your personalized enrollment worksheet and the Your Benefits Resources Web site during the annual open enrollment period for 2010. Make sure to take action before 6:00 p.m., Eastern Time (ET), on the last day of the annual open enrollment period. No late enrollments will be allowed!

helpfulhints Get to Know Your Benefits Resources Your Personalized Benefits Web Site If you have Internet access, find out how the Your Benefits Resources Web site can simplify how you manage your benefits. The interactive Get to Know Your Benefits Resources (YBR) tutorial will help you understand the Web site features, including the steps to enroll online and where to review your personal benefits options and costs. You ll also learn the differences between the Your Benefits Resources and the BenefitAnswers Plus Web sites two Web sites that Alcatel-Lucent provides to make managing your Alcatel-Lucent health and welfare benefits easy all year. To access the Get to Know Your Benefits Resources (YBR) tutorial before, during or after the annual open enrollment period, visit the BenefitAnswers Plus Web site at www.benefitanswersplus.com, click Management/Non-Represented Plan Design Retirees and then launch the tool from the homepage. REMEMBER... Personalized information, such as 2010 plan options and pricing, will become available on the Your Benefits Resources Web site, or by calling the Alcatel-Lucent Benefits Center, on the first day of the annual open enrollment period. Since Benefits Center representatives will not have personalized information until Monday, November 9, 2009, please do not call the Alcatel-Lucent Benefits Center with questions about your 2010 plan options or pricing prior to that date. You Will Need Your Password During the annual open enrollment period (Monday, November 9, 2009, at 8:00 a.m., ET, to Friday, November 20, 2009, at 6:00 p.m., ET), you will need your password to log on to the Your Benefits Resources Web site or to enroll by phone through the Alcatel-Lucent Benefits Center. (The same password works for both resources.) If you requested a new password and have not received it by the start of the annual open enrollment period, call the Alcatel-Lucent Benefits Center at 1-888-232-4111 and speak with a representative to discuss your options. (During the annual open enrollment period, the Alcatel-Lucent Benefits Center has extended hours from Monday through Friday, 8:00 a.m. to 6:00 p.m., ET.) Never wait for a password again! Once you have your password, add your preferred e-mail address to the Your Benefits Resources Web site. If you ever forget or misplace your password again, you won t need to wait to have a new one mailed to you. Instead, you can request that a temporary password be sent to your preferred e-mail address! 4

In December, you ll receive a newsletter from Alcatel-Lucent containing helpful tips for using your coverage during the year. This will include information about your medical plan ID cards, important reminders, such as what to do when you experience a life changing event (such as a marriage, divorce or death) and how to find in-network providers, if a network applies to your plan option. 5
