BlueMedicare HMO (Plan and BlueMedicare HMO POS Summary of January 1, 2017 - December 31, 2017 This booklet provides a summary of what BlueMedicare HMO (plan and BlueMedicare HMO POS (plan 008) and what you pay for covered services and supplies. To get a complete list of services we cover, call our Member Services Department and ask for the "Evidence of Coverage." You may also view the Evidence of Coverage on our website, www.bluemedicarefl.com. Things to Know About BlueMedicare HMO (Plan and BlueMedicare HMO POS Eligibility requirements To join BlueMedicare HMO (plan or BlueMedicare HMO POS (plan 008), you must be entitled to Medicare Part A, enrolled in Medicare Part B, and live in our service area. Our service area includes the following County in Florida: Pinellas Which doctors, hospitals, and pharmacies can I use? BlueMedicare HMO (plan and BlueMedicare HMO POS (plan 008) have a network of doctors, hospitals and other providers. In most cases, you must receive care from network providers; care you receive from out-of-network providers is not generally covered by our plan. There are three exceptions to this requirement: We cover emergency care and urgently needed services you receive from out-of-network providers. If providers in our network cannot provide a type of Medicare-covered care you need, we will cover the care if you receive it from an out-of-network provider. You must receive authorization from our plan before seeking care from an out-of-network provider in this situation. We will cover care you receive at a Medicare-certified dialysis facility when you are temporarily outside our plan s service area. In most situations, you must use our network pharmacies to fill your prescriptions for covered Part D drugs. Some of our network pharmacies offer preferred cost-sharing. For some of these benefits, you may pay more in our plan than you would in Original Medicare. For others, you may pay less. Our plan members also get more than what is covered by Original Medicare.
