SHBP Website at ADP Page 1 www.myshbpga.adp.com DO NOT PROCRASTINATE! 300,000+ members all need to make an election in the same 19 day period! Make changes as often as you like. The last change you make before the website closes will be your election for 2018. Be sure to click Finish, then print the confirmation page and keep for your records. 3

SHBP Website at ADP Page 2 DO NOT PROCRASTINATE! Login problems are common. The closer to November 3 rd the longer the hold time at 800-610-1863. Sometimes you can t get through at all. Passwords expire after 45 days. If you can t remember last year s password, click Forgot password and follow the prompts. You can have it emailed or texted to you, but be sure you have the same email or phone number as last year. Check your Clutter, Spam, and Junk folders if you chose to have it emailed and don t see the email. 4

SHBP Website at ADP Page 3 Too many login attempts will lock you out for 1 hour. After that you can try again. Questions or problems with the SHBP website must be directed to 800-610-1863. FCS has no power or authority over the SHBP website. FCS can not reset your password. The call center will not speak with us about your account. 5

SHBP Website at ADP Page 4 Recommendations: Access the SHBP website prior to OE to ensure you will be able to log in. Check your address on the SHBP site. If incorrect, email Benefits with your ID# and the correct address. The health plan and tier you have this year will rollover to next year unless you make a change. If you don t have health coverage with us and don t want it, do nothing. There are no automatic defaults to tobacco surcharge. 6

Employee Express Changes Page 2 Before making elections in Employee Express, be sure to add your dependents. Social Security Numbers, Dates of Birth, and phone numbers are required for any dependents. If electing Dental, Vision, or LifeLock be sure you specify which dependents should be covered. If you can t see all the fields to enter, maximize your window or tab around. Be sure to walk through all six steps and Save before you exit. Not saving = Never happened. Print and keep a confirmation for your records. 8

Don t forget about Charitable Fund Online! Charitable campaign will be October 16 November 3, during Benefits Open Enrollment. Make one-time or per-pay-period elections in Employee Express, by choosing either: Charitable Fund 1 Time dates will be 03/16/2018-03/31/2018 Charitable Fund Year dates will be 01/01/2018-12/31/9999 One-time elections will occur on the 3/31/18 check only. Forms are used only for those who contribute by personal check. 9

Short Term Disability (STD) Plans Page 1 What is it? STD replaces your salary if you become ill or injured and can t work, and you have exhausted your sick leave. 10

Short Term Disability (STD) Plans Page 2 Do I need it? Ask yourself these questions: Do I need my paycheck to pay my bills? Can my spouse/partner support our family without my salary? How many sick days have I saved? Am I in the sick leave bank? Do I have savings or any other source of income? 11

Short Term Disability (STD) Plans Page 3 OK, I need it. Which plan should I choose? 15-day waiting period is for people who haven t built up much sick leave and can t yet join the sick leave bank. 45-day waiting period is for people who have built up some sick days, or who have joined the sick leave bank. 12

Short Term Disability (STD) Plans Page 4 What s a waiting period? The amount of time between the date of disability and when payments can start. Use your sick days or sick bank days to cover the gap. Must exhaust all sick leave before STD can start. 13

Short Term Disability (STD) Plans Page 5 How much does it pay, and what does it cost? 15-day wait plan replaces up to 50% of monthly salary and costs $0.34 per $100 of monthly salary (gross pay of $2,000=$6.80). 45-day wait plan replaces up to 60% of monthly salary and costs $0.156 per $100 of monthly salary (gross pay of $2,000=$3.12). 15-day wait plan is used much more often, so it costs more. Move to the 45-day wait plan when you build up sick leave and join the sick leave bank. 14

Short Term Disability (STD) Plans Page 6 Things to Know No health questionnaire required to elect STD if you elect it, you will have the coverage. A pre-existing clause does apply so, for example, someone already pregnant won t have this pregnancy as a covered disability. Must be on an approved Leave of Absence to apply. Can lead into Long Term Disability (LTD). 15

Sick Leave Bank Use after you exhaust your own sick leave. Elect during next OE after one year anniversary of your employment. Donate 2 days of sick leave to join. Can be granted up to 30 working days. 15-working-day waiting period before days are granted. 16

Long Term Disability (LTD) Plan Page 1 What is it? LTD replaces up to 60% of your salary (after a 180-day [6 month] waiting period) if you become ill or injured and can t work for an extended period of time, possibly until retirement age. 17

Long Term Disability (LTD) Plan Page 2 How do I elect it? FCS provides LTD to all benefit-eligible employees, unless an employee chooses to pay for it themselves. Employer paid when you receive disability payments, they are taxable as income. Employee paid when you receive disability payments, they are not taxable as income because you paid taxes on the premium. Cost is $0.42 per $100 of monthly salary ($2,000=$8.50). 18

Life Insurance-Basic Provided by FCS based on salary If your salary is: <$12,000 you have $24,000 in coverage. $12,000 to $17,999 you have $30,000 in coverage. $18,000 to $25,999 you have $40,000 in coverage. >$26,000 you have $50,000 in coverage. 19

Life Insurance-Group Term Life (GTL) True Open Enrollment no health questions required to enroll up to Guaranteed Issue limits. Can elect for yourself, spouse, children. Pays in the event of death due to any cause. You choose the beneficiary for your life coverage, your spouse and child life is paid to you. 20

Life Insurance-GTL EE Can elect up to 5X salary. Maximum $300,000 without health questionnaire, $500,000 with underwriting (if salary permits). Electing the minimum of $10,000 on yourself allows you to increase later with no health questions. Who needs it? Anyone whose family depends on their income. 21

Life Insurance-GTL Spouse Can elect up to the employee s amount. Maximum $100,000 without health questionnaire, $500,000 with underwriting. Electing the minimum of $5,000 on spouse allows you to increase later with no health questions. Elect your coverage first, Employee Express won t allow Spouse coverage without Employee coverage. Who needs it? Anyone whose family depends on their spouse s income. 22

Life Insurance-GTL Child(ren) Can elect either $10,000 or $15,000. One flat premium covers all children up to age 19 (age 26 if full time student). Only one parent can cover children if both are employees. Elect your coverage first, Employee Express won t allow Child coverage without Employee coverage. Employee must have $20,000 to have $15,000 Child. 23

Life Insurance-Beneficiaries If you have not reviewed your beneficiaries in 5 years, do so. Deaths, divorces, children turning 18, etc. are all reasons to review your beneficiary. Minor children should not be beneficiaries, as they can not legally receive the proceeds. Benefits does not have your beneficiary on file, they are kept securely at Unum, our life carrier. If you can t remember who you chose, it s time to renew. 24

Permanent Life Individual policy that you own. Pay via payroll deduction. Keep even when you resign or retire. Coverage available for you, spouse, child, grandchild. Premium stays flat throughout life of coverage. Available in $10,000, $20,000, $30,000, $40,000, or $50,000. Critical Illness Individual policy that you own Pay via payroll deduction Keep even when you resign or retire Critical Illness pays a flat amount for 1 st diagnosis of: -- INVASIVE CANCER -- HEART ATTACK -- STROKE -- KIDNEY FAILURE -- BLINDNESS -- ALS -- MAJOR ORGAN TRANSPLANT -- PARALYSIS OF TWO LIMBS -- CORONARY BYPASS (25%) -- CARCINOMA IN SITU (25%) Call 1-844-268-0640 to inquire about or elect coverage, or go to www.trustmark.benselect.com/enroll to enroll yourself. 26

Long Term Care Insurance (LTC) Page 1 Coverage pays for nursing home, assisted living, and home care (not the same thing as Long Term Disability). Income and assets can be exhausted by one spouse, leaving little for the survivor to live on. Average cost of care in metro Atlanta: Home care $2,160/month (4 hours/day) Adult Day Care $1,800/month (8-12 hours/day) Assisted Living $3,060/month Nursing home care $5,910 for a semi-private room and $6,630 for a private room 27

Long Term Care Insurance (LTC) Page 2 Individual policy that you own & keep, even when you resign or retire, but pay for via payroll deduction. Coverage available for you, spouse, parents & grandparents, parents- and grandparents-in-law. You choose the amount of coverage you need per day and for how many years. The older you are when you purchase it, the higher the premium. Call LTC Solutions at 877-286-2852 for more information. 28

LifeLock 2 levels available: Elite and Ultimate Plus. 4 tiers available: Employee Only, EE + Spouse, EE + minor Child(ren), EE + Family. Children can be covered to age 26. Dependents birthdates, SSNs, and a phone number must be in Employee Express to be protected. In Employee Express, check the box next to the names of dependents to be covered. 29

Delta Dental No changes to coverage or premiums. 2 tiers available: Employee Only or Employee + Family. Children can be covered to age 26. Don t forget Wellness Incentives (BCBS and UHC) Complete a RealAge Test and Biometric Screening to earn $240 in incentives. Earn another $240 in incentives by either: Completing 4 telephone coaching calls ($60 per call) OR Record 60 Green Days in a 90 day period ($120 [max. 2]). Spouse can earn $480 also, if covered by SHBP. HRA plans $$ is added to HRA bucket. HMO plans copays are reimbursed from $$. HDHP members have a threshold before they can be used. 40

Don t forget Wellness Incentives (Kaiser Only) Complete a Total Health Assessment (THA). Complete a Biometric Screening (must be done by Kaiser). Complete all age and gender appropriate cancer screenings. Take one online Healthy Lifestyle Program course. Earn a $500 VISA gift card. Spouse can also earn $500 gift card, if covered. 41

Don t forget about Incentive Rollover! Wellness Incentive Credits will rollover between all BCBS and UHC plans. Change plans without losing what you ve earned. Credits will rollover in April 2018. Credits will roll into Kaiser, but you will not earn credits with Kaiser. 42

Important Changes to the 2018 Health Coverage Page 1 ExpressScripts will not be the Pharmacy Benefit Manager (PBM) for BCBS and UHC. CVS Caremark will be the new PBM. You are not required to use CVS pharmacies, but you ll receive incentives if you do. Healthways was purchased by Sharecare, so there will be a new Wellness Manager. 43

Strategy For Making an Election Page 1 As always, look at how you use health care. Minimal users might prefer the lower cost plans. People with frequent office visits might prefer an HMO plan. Remember that premiums are always deducted, while you may not always need services. Consider the level of benefits + premium + HFSA amount. 44

Strategy For Making an Election Page 2 The choice is going to come down to personal preference about costs, convenience, and freedom. Pay less premium per month, but more for services when they are rendered. Pay more premium per month and know how much you ll pay at the doctor or pharmacy. Kaiser is self-contained only one place to go for everything, and the only plan with no deductible. Having freedom to choose an out-of-network doctor means electing an HRA or HDHP plan. 45

Flexible Spending Account (FSA) What is an FSA? An FSA is an account funded with money deducted pre-tax from your check to pay for out-of-pocket medical, dental/ortho, and vision expenses, OR to pay daycare expenses for a child up to age 13 or an elder who you claim as a dependent on your taxes. 46

Health Flexible Spending Account (HFSA) Page 1 Set aside amounts to pay co-pays, meet deductibles, and pay for Rx. HFSA can also be used for dental and vision services. Can be used even if you don t use our health plan. Can be used for dependents charges even if spouse/children are not on an FCS plan. Also think about braces, glasses/contacts, LASIK, pending surgery, childbirth, etc. 47

Health Flexible Spending Account (HFSA) Page 2 Choose from $300 ($12.50/pay) to $2,600 ($108.33/pay) for the year. Set aside only what you know you will spend, because unused money over $500 is forfeited. Up to $500 remaining at the end of the year can be rolled over to 2019. May have to verify an expense (IRS Regulations). E-claims tracker makes reimbursement virtually painless. 48

Health Flexible Spending Account (HFSA) Page 3 HFSA scenario: you set aside $2,500 to finish paying off your child s braces. $104.17 deducted from each check before taxes. Full $2,500 is available January 1. Can use FSA debit card at provider s offices. You ll save $625 in taxes (25% tax bracket). If you itemize on your tax return, no need to meet the medical expense deduction threshold. 49

Dependent Care Flexible Spending Account (NFSA) Page 1 IMPORTANT! HFSA is used for ALL family member s health expenses. NFSA is used only to pay for daycare while you work, not for medical expenses for dependents. Set aside amounts to pay daycare expenses for children under age 13, or dependent adults unable to be left alone. Maximum is $5,000 ($208.33/pay) for married, both spouses working or full time students; $2,500 ($104.17/pay) for a single person. 50

Dependent Care Flexible Spending Account (NFSA) Page 2 IMPORTANT! Do not elect Dependent Care FSA to cover dependent medical, dental, or vision expenses. Can be used with any provider that is not paid under the table. SSN or Tax ID# required for reimbursement. Can also be used for summer day camp, after school care, or late pickup fees. Can be used for adult day care if the person lives with you >7 hours per day and is claimed on your Federal tax return. 51

Dependent Care Flexible Spending Account (NFSA) Page 3 NFSA scenario: you set aside $3,800 for after school care for your children ($70/school week X 2 children, excluding summers and holidays) $158.33 deducted pre-tax per check ($316.66 per month). Money is available 1 st of the following month. Pay $140 per week for care, claim for reimbursement or swipe card for up to $316.66 per month. Claim during summer for funds owed you and not yet reimbursed. You ll save $950 in taxes (25% tax bracket). No need to file the 2441 form. 52
