The vendor should fill up all the details with the supporting documents given in the form & Annexures. Only those vendors who fulfill the following criteria are eligible to enroll. 1. Minimum Eligibility Criteria, Category Category I Up to 20 lakhs Estimated Cost of work (in Rs.) Minimum Solvency (20% of estimated cost) Average Annual Turnover for last 4 years 20 Lakhs 4 Lakhs 40 Lakhs Minimum value of work completed in last Seven years Three similar completed works costing not less than Rs.8 Lakhs. or Two similar completed works costing not less than Rs.12 Lakhs. or One similar completed work costing not less than Rs.16 Lakhs. or One completed work of any nature (either part of the above or a separate one) costing not less than the Amount Rs.8 Lakhs with some Central Government Department / State Government Department / Central Autonomous Body / Central Public Sector Under taking / State Public Sector Under taking / City Development Authority /Municipal Corporation of City formed under any Act by Central / State Government and published in Central / State Gazette. Separate Enrollment form with processing fee shall be submitted for enrollment for TamilNadu works and Kerala works under Southern Zone. 3. Minimum Value of work completed in last four years should be the work done in Tamil Nadu & Kerala states. 4. Contractors should have GST registration for respective state for enrollment for work in that state. 5. Enrollment form available in our web site shall be downloaded and filled in enrollment form should be submitted. Last date of submission of Enrollment Form is on 05.01.2018 at 17.00 Hrs and the same shall be submitted at the following address :- The Chief Engineer, LIC of India, 9 th Floor, Engineering Department, Southern Zonal Office, Anna Salai, Chennai 600002. Note: These instructions for filling and submission of Enrollment Form shall also be signed in all pages and submitted along with Enrollment Form and Annexure A to H. LIFE INSURANCE CORPORATION OF INDIA ENGINEERING DEPARTMENT Southern zone ZONE INSTRUCTIONS FOR FILLING & SUBMISSION OF ENROLLMENT FORM The Enrollment Form along with the Annexure A to H shall be completely filled in all respects and to be submitted to this office along with these instructions for filling & submission of Enrollment Form & Enrollment fee of Rs. 2000/- (Rupees Two Thousand only ) for each work in the form of D.D. in favor of L.I.C. of India, payable at Chennai, on or before 05.01.2018 (17.00 Hrs) and the agencies downloading the enrollment form from web site shall deposit the fee in the form of DD along with the filled in enrollment form. Please note that no consideration will be given for postal delays. 1. Contractors to note that all particulars required as per the form and Annexures shall be filled in completely in relevant blanks strictly as per the format. 2. The forms not submitted strictly as per the above instructions within stipulated period are liable to be rejected. 3. The eligible contractors who will be selected for issue of tenders after scrutiny of Enrollment forms shall be informed by a letter. Please note that no enquiries or correspondence regarding the selection for issue of tenders shall be entertained. 4. Latest Solvency Certificate from any Nationalized/Scheduled Bank for minimum amount as mentioned in the Minimum qualifying requirement as per advertisement should be submitted along with Enrollment Form. 5. The contractors are advised to follow the instructions given below:- (a) Enrollment Form shall be filled-up in clean handwriting in capital letters or typed. (b) (c) (d) Full address of the site of work, owner or authority under whom the works have been carried out should be given (Please refer Annexure D & E ) The contractors should ensure to submit the satisfactory Completion Certificate giving the value of work, year of completion and it should also tally with the value of final bill in Annexure D. The annual turn over should be based on latest Income-Tax Clearance Certificate, duly cleared by Income-tax Department / Audited Balance sheet of Chartered Accountant. 5

Please note that the submission of this Enrollment form does not confer any right on you to claim issue of tenders and the Chief Engineer reserves the right not to issue tender to any/all applicants without assigning any reason whatsoever. I/We understand and agree that the appropriate Life Insurance Corporation of India Authority has the right as he may decide, not to issue tender form in any particular case and also to suspend, remove or blakhsklist my/our name from Life Insurance Corporation of India list of contractors in the event of my/our furnishing false particulars in the Enrollment form or submitting non-bonafide tenders or for technical or other delinquency in regard to which the decision of appropriate Life Insurance Corporation of India Authority shall be final and conclusive.
