Please take a moment to read the following guidance before completing the application. Identification of the Registered Holders (in Section 2 to this Application Form) It is necessary for each Registered Holder to provide one item of evidence to confirm each of the following: 1. Their permanent residential address; and 2. Their identity. Note: A single item of evidence provided cannot be used to verify both of the above (i.e. at least two separate documents are required). Please see below what forms of evidence are acceptable. If any of the Registered Holders is unable to comply with this request, they should contact CAF Financial Solutions in the first instance: A Confirmation of Verification of Identity issued by an independent financial adviser; or Government issued documents to include full name, photograph and either residential address or date of birth (e.g. Valid Passport or valid full UK Driving Licence); A valid (old style) full UK Driving Licence (to be used for evidence of permanent residential address only); Evidence of entitlement to a state or local authority funded benefit (including housing benefit and council tax benefit), tax credit, pension, education or other grant, dated within the last 4 months; Instrument of a court appointment (such as liquidator, or grant of probate); Council tax demand letter, or statement, dated within the last 4 months; Bank statements, or credit/debit card statements, issued by a regulated financial sector firm in the UK, EU or comparable jurisdiction (but not items printed off the internet), dated within the last 4 months; Utility bills (but not in respect of a mobile phone or any items printed off the internet), dated within the last 4 months. IMPORTANT: Each item of evidence provided must be certified as a true copy of an original by a person from the following list: A person/company covered by the money laundering regulations (this includes banks, IFA s, insurance companies, stockbrokers, investment managers, mortgage brokers); Professional individuals (such as a UK lawyer, accountant, teacher, doctor, minister of religion, post master/ sub-post master); UK government departments. Certified photocopies of identity and address must: Be signed, dated and marked sufficiently to evidence that the original has been seen along with printed contact details of the individual who certified (i.e. name, occupation and contact address); The signature on the certified document must be an original signature; If the photograph on a Passport or Driving Licence is of poor reproduction the copy should also certify that it is a good likeness of the applicant; The person certifying a document cannot be the same person to which the application relates. Common Reporting Standard and Foreign Account Tax Compliance Act Both the Common Reporting Standard (CRS) and the Foreign Account Tax Compliance Act (FATCA) are aimed at reducing tax evasion by citizens using foreign accounts outside of their jurisdiction of tax residency. In the case of the UK, both require all financial institutions to report certain details regarding the holdings of investors to HM Revenue and Customs (HMRC). All reporting will be handled by FundRock Partners. 7. Income Payments (Income Shares only) Please complete this Income payment section if you wish the income arising from your Income shares to be paid directly into your bank or building society account. If you leave this section blank income will be paid by cheque. Please note that banks and building societies may not accept Direct Credits to some types of accounts. Regular Withdrawal Facility (Accumulation Shares only) The Regular Withdrawal Facility is only available on lump sum investments into accumulation shares, with a minimum qualifying investment of 10,000. The facility is not available if you are investing new money by direct debit in a savings scheme on a monthly basis. We will use this information to open your account, maintain the Register; process subscriptions, redemptions and exchanges of Shares and payments of dividends; perform controls on excessive trading and market timing; comply with applicable anti-money laundering rules or anti-terrorist financing rules; or comply with our reporting obligations to regulatory bodies or tax authorities as well as our obligations under other applicable laws and regulations, monitor calls and electronic communications to process and verify of instructions, or for investigation and fraud prevention purposes. The legal basis for this processing of your personal data is our legitimate interests, namely the proper administration of your investment, the operation the Fund by us, our delegates and the service providers in relation to a Fund; the performance of the contractual obligations between you as a Shareholder and us; to provide you with information, products and services that you request from us; to notify you about changes to our services or to a Fund or the Company; and to comply with applicable laws and regulations. You have the right to refuse to give us your personal data in which case we may at our discretion refuse to issue Shares to you; refuse to pay the proceeds of a redemption of Shares; refuse to pay income on Shares; or compulsorily redeem your holding. (2) With regard to each of your visits to our website, we will automatically collect certain information about you or your computer. We will do so for administration purposes and to analyse the use of our website and services. Further details are available in the Prospectus and on our website. The legal basis for this processing is our legitimate interests, namely monitoring and improving our website and services. Retention and deletion of your personal data We will not keep your personal data longer than is necessary for the purpose that we process it or for any purpose. We will generally retain your personal data for a minimum of 7 years, or for such period as is necessary for compliance with a legal obligation to which we are subject, or in order to protect your vital interests or the vital interests of another natural person. Rights of an individual As an individual, you have certain rights under the Data Protection Requirements.
