1 Individual/Joint Application Checklist This is an exciting time for Westpac Broking we are in the process of appointing our service provider, Australian Investment Exchange Ltd, to act as the new sponsoring broker for our clients. As this is a time of change we have made some modifications to our ordinary Westpac Broking application form in this modified application form you will be asked to agree to the provision of broking services by both Westpac Securities Limited and Australian Investment Exchange Ltd. This will allow us to provide you with continuity of service as we implement this change to the sponsoring broker. To ensure your application is processed as quickly as possible, follow the steps below. 1 Complete ALL required sections Please refer to the table below for the sections which apply to the account type you wish to open. All correspondence will be sent to Applicant 1. 2 Ensure ALL required sections are signed and dated. 3 Proof of Identification All persons noted in sections A, B or C who are not existing Westpac or BT Margin Lending customers must provide original certified copies of your identification documents, which must be certified by an eligible person. Please refer to the following page for a list of acceptable documentation and a list of eligible persons. 4 Direct Debit Request Section F must be completed for you to trade with Westpac Broking. If the bank account you nominate as your settlement account is not a Westpac account, you must attach a copy of a recent bank statement from your nominated bank account, which must show the residential address provided on your application form. Note: For a joint account your nominated bank account must be under the same joint names provided on the application. 5 Transfer Holdings Section G must be completed if you wish to transfer existing Financial Product holdings to be broker sponsored by Westpac Broking or BT Securities Limited. 6 Mail ALL required documentation to: Westpac Broking Reply Paid 7777 Cloisters Square PERTH WA 650 (No postage stamp required) Page 1 of 6 P/C 196 (11/09)

2 Acceptable Identification Documentation for Individuals Two forms of identity documents must be provided from the lists below (at least ONE must be from the Primary Identification Documents list) which verify the following details: Full name (surname and given name/s) Residential Address (not PO Box) Date of Birth Please note the following: If you do not have two forms from the primary list, you may choose one from the primary list and one from the secondary list. All documents must be current unless specified otherwise. If any document is written in a language other than English, it must be accompanied by an English translation prepared by an accredited translator. Once this appointment has been finalised, share trading through Westpac Securities Limited will be provided by The Participant. This is expected to occur on or around 25th January On and from that date, I/we agree: (a) To be bound by The Participant s Share Trading Conditions as described in the Terms and Conditions; and (b) To be bound by The Participant s Online Access Conditions as described in the Terms and Conditions; and (c) To be sponsored by the Participant under the terms of the Participant Sponsorship Agreement as described in The Participant s Terms and Conditions. 6. I/We instruct Westpac Securities Limited to transfer any Holder Identification Number (HIN) I/We may have along with ALL my CHESS Sponsored Holdings held with Westpac Securities Limited to The Participant on or as soon as practical after the date The Participant is appointed as sponsoring broker for the Westpac Securities Limited clients. 7. I/We agree for The Participant to send my/our Confirmations, Trading Account information and Statements to me/ us electronically.. I/We declare that I have the legal capacity to make these declarations and accept the conditions. I/We agree and enter into the agreements referred to in points 1, 2, 3, 4, 5, 6 and 7 above.
