4 Health Insurance Overview Follows CALENDAR YEAR (January 1 December 31) Premiums Employee Only Health Insurance No Cost 50% State Contribution for Dependents Part-time rates (50/50 EE, 75/25 Dep) Enrollment Automatic Enrollment for full-time employees Employees must log in to ERS to customize within 60 days following first day of employment Dependent enrollment and part-time enrollment employees 60 days following the first day of employment Effective Date First of the month following the 60th day of employment

8 Same-Sex Spouses Following the U.S. Supreme court s ruling on June 26, 2015 regarding same-sex marriage, employees will be able to enroll their same-sex spouses and dependent step children of those spouses in the same coverages and services, and at the same cost, as opposite-sex spouses.

9 Health Insurance Overview $5,000 Life Insurance policy connected with EE medical plan Tobacco Tobacco User Certification required for you and your dependents. ERS System default = tobacco user

10 Tobacco User Premium Choose to Quit Recent federal regulations require ERS to offer a new alternative to paying the tobacco-user premium, to members and their dependents enrolled in a GBP health plan and certified as tobacco-users. For additional information go to the ERS Choose To Quit Q&A.

12 Dental Insurance * Functions on CALENDAR YEAR * 3 plans Fully Paid By Employee 30 days to enroll Effective 1 st day of month following enrollment Humana Dental This plan works like an HMO. Prenegotiated rates. You and your eligible dependents must get your dental care within the Humana Dental HMO service area (inside the state of Texas). $9.12 EE only State of Texas Dental Choice Plan Administered by the Humana, this plan allows you to visit any dentist, and you and eligible dependents can enroll regardless of where you live. $23.58 EE only Dental Discount Plan Careington Dental The State of Texas Dental Discount plan is not an insurance, but a discount program for dental services. You can save 20% to 60% on most dental work with 20% savings on orthodontics for children and adults. Rates vary per dentist. $2.25 EE only

15 Dependent Term Life Life insurance coverage for each covered family member $5,000 policy $1.38 per month ($1.38 covers EE + all dependents) *Administered by Minnesota Life

16 Voluntary Accidental Death & Dismemberment (AD&D) Accidental death In the event of your death as the direct result of an accident, your beneficiary will receive the amount of insurance you select. Office of Human Dismemberment If you have an accident and experience bodily losses as a direct result of the accident, you will receive AD&D benefits. Cost Depends on the amount of coverage you purchase Purchased in increments of $5,000 Minimum coverage available is $10,000 Maximum coverage available is $200,000 You and your dependents.02 cents per $1000 for EE only /.04 cents per $1000 for EE & Family Example: EE only for $200,000 is $4.00/mo EE + Family for $200,000 is $8.00/mo

17 Disability Insurance Office of Human Short Term Premium is.30 per $100 of monthly salary Must be fully disabled Must be off work 30 days before coverage begins Insurance pays 66% of salary for a maximum of 5 months Maximum benefit: $6,600 per month Long Term Premium is.63 per $100 of monthly salary Must be fully disabled Must be off work 180 days before coverage begins Insurance pays 60% of salary until age 65 Maximum benefit: $6,000 per month

18 TexFlex Reimbursement Accounts TexFlex Health Care Account: $2550 TexFlex Day Care Account: $5,000 NEW ADMINISTRATOR: ADP Office of Human Fees - $12 annual administrative fee (2015/2016 holiday) $15 annual debit card fee (Debit card not an option for Day Care) Reimbursement - mail, fax, or use of a debit card. Use it or Lose it - money contributed must be used during that plan year, (September 1 st - August 31 st ) or it is forfeited (grace period until November 15 th ). Eligible to rollover $ to the following plan year for TexFlex healthcare only.

19 Opt-Out Health Insurance Opt-Out Program Employees who choose to not elect the State s health insurance due to other coverage. Office of Human Credit provided for employees to use toward dental and Voluntary AD&D premiums. $60/mo full time $30/mo part time

WATCH YOUR MAIL - Health Insurance Cards will be mailed to you by the effective date MAKE A CHOICE - Primary Care Physician (PCP) required. Office of Human MAKE A CALL - Call United Health Care with your PCP choice as soon as you receive your insurance cards. CERTIFY - You must certify Tobacco Use for you and dependents or be charged a $30.00 premium for you and up to $90.00 per household. KEEP SYSTEM UPDATED - Personal Information (address, etc) must be updated in ERS by you!
