At that time, the contract value will be reset to the greater of the two values. However, the GMAV equals zero and terminates at the time of any withdrawal from the contract during the withdrawal charge period. The GMAV is not available for cash value surrender. Rider charge for Guaranteed Lifetime Income Benefit (GLIB) (0.95% of Income Base charged at the beginning of each contract year), withdrawal charges and Market Value Adjustments (MVA) apply to amounts in excess of the free withdrawal amount 3 Withdrawal charges apply to amounts withdrawn in excess of the free withdrawal amount during the first 10 contract years. The free withdrawal amount is 10% of the contract value as of the previous contract anniversary, available in all years in most states (state variations apply). 5 Year Charge 10% 9% 8% 7% 6% 5% 4% 3% 2% 1% 0% An income bonus may be provided at four distinct points within the life of the contract, as long as no income election has been made. Adjustment (MVA) 3,4,5 Applies during the withdrawal charge period on withdrawals in excess of the free partial withdrawal amount. The annual Guaranteed Lifetime Income Benefit amount (GLIB amount) will equal a percentage, known as the Income Benefit Factor, of the Income Base. ForeCare affords your clients more control to choose what's best for their individual needs. 1 Initial premium net of any optional benefit charges in California. 2 This is called the ForeCare Multiplier: it provides two or three times (depending on underwriting eligibility) the amount of contract value in long-term care coverage to spend on qualified long-term care expenses. There is a monthly benefit fee associated with the long-term care protection that ForeCare provides. The realized crediting return will be less than the declared interest rate due to the fee. However, even after deduction of the benefit fee, the contract value at month end will not be less than the contract value at the prior month end, less any applicable withdrawals. To qualify for benefits, receipt of proof is required from a licensed health care practitioner stating you are chronically ill and are incapable of performing, without substantial assistance, two Activities of Daily Living (ADLs) for at least 90 days, and/or you have a cognitive impairment that requires substantial supervision. The ADLs include: Bathing, Continence, Dressing, Eating, Using a Toilet, Transferring/Mobility. Covered Long-Term Care Services must be provided in conjunction with a written Plan of Care submitted by a Licensed Health Care Practitioner, and approved by Forethought Life Insurance Company. 9-year schedule: 8%, 8%, 7%, 6%, 5%, 4%, 3%, 2%, 1%, 0% thereafter (state variations apply) A Market Value Adjustment, which may be positive or negative, applies during the Withdrawal Charge period to any withdrawals that are subject to withdrawal charges. (state variations may apply) Free Withdrawals 10% of the beginning of year contract value can be withdrawn annually, free of charge except in year of full surrender. Long-term care benefit claims will reduce this amount. 1 Spouse is as defined for Federal tax purposes. 2 Subject to state variations.
