Yes This is for the purposes of Financial Services Authority reporting. Please read these notes before filling in your application This form is split into three parts: Part A (sections 1-7) all applicants must complete this. Part B (sections 8-10) this needs to be completed on behalf of the scheme which is making the transfer payment to your AEGON Scottish Equitable Flexible Pension Plan. The transferring scheme can provide these details in a separate document as long as this meets all our requirements. Part C this contains notes giving you information about this application and the contract it forms. Please make sure you read these before you sign the declaration. We also give you more detailed guidance within the sections you complete and explain the terms we use throughout. Words in italic are defined terms that we explain in part C. Whenever you see this icon, we re asking you to send us additional material with this form. This application is for a contract with us. This application will form the basis of a contract of insurance with AEGON Scottish Equitable (the trading name of Scottish Equitable plc). When you answer a question, you must give all relevant information. This means that you must completely and accurately disclose all of the facts when answering each question. Please also note that you are not required to make any personal assessment about the relevance or otherwise of any information. If you don t disclose all relevant facts, the protection that the plan provides could be lost or cancelled and your claim rejected. Please check any details that are already filled in. If any details are already completed (based on what you ve told your financial adviser), please check these before you sign the declaration. 2 Part A Must be completed by all applicants About protected and non-protected rights In this application, you re applying at the start for three arrangements under your AEGON Scottish Equitable Flexible Pension Plan: We ll pay all non-protected rights contributions to the insured element of your plan, for instance your personal and your employer s contributions and payments (such as a transfer payment), into one arrangement under the Scottish Equitable Self-administered Personal Pension Scheme. We ll pay all protected rights contributions and payments into one arrangement under the Scottish Equitable Personal Pension Scheme. We ll pay all non-protected rights contributions and payments to the self-invested element into one arrangement under the Scottish Equitable Self-administered Personal Pension Scheme. If you need more details, please contact your financial adviser. Any reference in this application form to the Scheme shall be to either or both of the Scottish Equitable Self-administered Personal Pension Scheme and the Scottish Equitable Personal Pension Scheme. 1. 4 4. Death benefits 4.1 Do you want any lump sum death benefits under the insured element of your plan to be put into trust? These benefits include return of the retirement fund, apart from any contracted-out benefits. I understand the scheme administrator has absolute discretion about which of the beneficiaries named below (if any) it chooses. This nomination is only an expression of my wishes and isn t binding on the scheme administrator. If my circumstances change, I understand that I can change my nomination by filling in a new form. 5 6. Entitlement to protection of benefits (if applicable) 6.1 If you ve registered for protection with the Revenue, you must provide us with the original certificate. For enhanced protection to continue, this transfer must be a permitted transfer. I also apply to become a member of the Scottish Equitable Personal Pension Scheme in respect of any protected rights paid to the plan either now or in the future. Any reference to the Scheme will be to one or both schemes as appropriate. I understand that this may mean that one Scheme does not contain any benefits for me. I understand that where non-protected rights are to be applied to the Scottish Equitable Self-administered Personal Pension Scheme and protected rights are also applied to the Scottish Equitable Personal Pension Scheme, the transfer cannot be a block transfer to retain lump sum protection and/or a low protected pension age after the transfer. b I apply for an AEGON Scottish Equitable Flexible Pension Plan. I therefore apply for you to issue me with a plan or plans on your standard provisions and incorporating any special terms or provisions appropriate to my contract or contracts or any part of them which may be applicable. These special terms or provisions will be in your current form or, where you do not have one, as you think reasonable to reflect the special terms or provisions. c I agree to be bound by the documents and rules of the Scheme. d I consent to the transfer payment from the transferring scheme being paid to the Scheme to provide benefits for me. Where the transfer comes from an existing AEGON Scottish Equitable contract, I understand that the actual amount transferred will e f be the transfer value calculated at the date you receive all completed documentation. I also agree to the amendment of the new plan to your normal terms to deal with internal transfer values. Where the transfer is from an existing AEGON Scottish Equitable scheme and the source is a personal pension plan, stakeholder plan, retirement annuity contract, or a plan or contract issued or assigned in my name, I renounce all claims and demands under the plan numbered in section 5 which relates to the transfer payment. Where the transfer payment represents all of the plan funds, I hereby deliver the plan for cancellation. Where the transfer payment applied for is subject to a divorce order, and you agree to accept the application: i I understand that you envisage carrying out the order will result in you having to do further administrative work, which will mean further administrative expenses. ii I agree that you will change the terms of the plan (from your standard or other terms) as necessary for you to be able to carry out the actions described in (iii) below. iii You may make an extra charge each year for such further administrative work you have to carry out. The charge will be payable from the date on which you agree to carry out this further work (or any later date that you agree to), and yearly after that. If I take benefits from my plan between these dates, you will apply a pro rata charge. You will let me know in writing the initial amount of such further administrative expenses that you incur, and this amount may vary from time to time. You will recover this amount by deducting it from a contribution paid to the plan and/or by cancelling allotted units under the plan to the value of the administrative expenses. You will take into account any deductions that apply under the plan for surrendered units. Page 5 of 12

Declaration continued g h i j k l m n I understand and agree as follows: i Any contributions to the plan will be applied to investment funds, according to my instructions to you (which include any instructions set out in the Investment choice section of this application). ii I am aware that you have an investment fund allocation procedure and that this procedure may change from time to time. iii That allocation procedure will be part of the plan I am applying for. iv Whenever a contribution is given to you to apply under the plan, and you do not have clear and complete instructions from me about which investment fund(s) the contribution is to be applied to, then you will apply the contribution to the investment fund(s) chosen in accordance with the investment fund allocation procedure. I declare that where I have indicated in section 6 that enhanced protection applies to the funds being transferred. I understand that for protection to continue, this must be a permitted transfer. I agree to the scheme administrator or its representative obtaining from me (or any other person or body authorised to receive payment under the Scheme) any evidence and information that it may need in order to make payments. I understand that if the scheme administrator does not get the information they need to administer my benefits accurately, the scheme administrator has the right to make further charges. I understand that benefits cannot be commuted, surrendered or assigned, except in accordance with the Finance Act I declare that, to the best of my knowledge and belief, the information given in this form, whether in handwriting or not, is correct and complete. I agree that you or your representatives may approach any of the scheme administrators, providers or insurers of any other registered pension scheme to which I am contributing or have contributed in addition to the Scheme to obtain any information you feel is necessary to administer my arrangements under the Scheme. I understand that deliberate or careless failure to answer any questions correctly and completely to the best of my knowledge and belief may result in the benefits not being payable. Consent to processing of personal data Scottish Equitable plc is the data controller of any personal data that I give on this form. You will only hold this information for use in connection with the contract (and related services) which I have applied for. This includes administration, claims management and dealing with complaints. The information you hold on me may also include sensitive data. AEGON Scottish Equitable s confidentiality policy, means that all personal data is held securely and it s only seen by those who need to see it. You may share my information with: certain bodies when you legally have to, for example regulatory bodies or authorities such as the Financial Services Authority or the Revenue, and when you need to in order to comply with money laundering laws and for other purposes, for example detecting crime people acting on my behalf (for example my financial adviser) so that they can fully carry out their role as my agent you can only share permitted relevant information with them other companies in the AEGON UK plc group, who may let me know about their new products or services The AEGON UK plc group means AEGON UK plc and its subsidiary companies (including Scottish Equitable plc, which uses the trading names AEGON Scottish Equitable, AEGON Trustee Solutions and AEGON Actuarial Services). I understand that if I do not wish to receive such information, I should tick this box. I consent to the processing of data as explained above (and where appropriate for other persons named in this form). The information in this application is needed for income tax purposes and may be inspected by UK tax authorities. Pensions and divorce 9.1 Is any part of the transfer payment subject to a court order issued under Section 23 of the Matrimonial Causes Act 1973, or Section 8 of the Family Law (Scotland) Act 1985 (in other words, an earmarking order following the member s divorce)? If the transfer is from an existing AEGON Scottish Equitable scheme, we renounce all claims and demands under the plan numbered in section 5 which relates to the transfer payment. 9 Part C Application form notes Important notes about this application You should only use this application form for a transfer payment to a new plan. If you want to make an additional transfer to an existing plan, you should complete an additional transfer payment supplementary form. rmally you can t make a transfer if you re already being paid benefits under the transferring arrangements. An exception to this is a drawdown arrangement. If you want to make a drawdown to drawdown transfer, you need to complete a separate form. If you want to start paying regular or single contributions to the plan, you also need to complete a separate form. Important information for financial advisers Money laundering In order to comply with the requirments of the Money Laundering Regulations 2003 and the Prevention of Money Laundering/Combating the Financing of Terrorism Guidance tes for the UK Financial Sector, you re required to complete a separate confirmation of verification of identity certificate, which should be submitted together with this application form. We won't complete the processing of this application until we ve received a fully completed and signed confirmation of verification of identity certificate(s). A confirmation of verification of identity certificate must also be completed for the trustees/scheme administrator of the transferring scheme or arrangement set out in section 5. However, where the transfer payment is paid solely by a cheque from an insurer (insurer being defined in Part 4 of the Financial Services and Markets Act 2000), we don t need verification of the identity of the trustees/scheme administrator of the transferring scheme or arrangement. We need a verification of identity certificate for the applicant before any self-administered investment is made. So, if the Investment management section has been completed, please can you make sure that you send us a verification of identity certificate unless you have already sent us one for this plan. Even if the applicant isn t making a self-administered investment at this stage, we strongly advise that you send us a completed certificate with this application to avoid any delays if they make a self-administered investment in the future. tes on sections in the application form Personal details We don t need to see your original birth certificate, marriage certificate or civil partnership certificate before we set up your plan. However, we ll need to see these documents before we can pay you any benefits. We can t process your application without your National Insurance (NI) number. However, if the applicant is under 16 years of age, and so doesn t have an NI number, you should fill this in as YY Y. Currently you can choose a retirement age between 50 and 75 unless you have a protected low pension age/normal retirement age. However, from 6 April 2010 the minimum age that you can take your benefits will be 55. So if you choose a retirement age of less than 55, you must make sure that you ve reached this age before 6 April 2010 to take your benefits on that date. Investment choice (insured element only) Please make sure you enter the exact fund name (as detailed in our fund range guide), especially if you choose a lifestyle, UK equity, European or balanced managed fund. If you make a clear and complete investment choice, we ll invest the contribution(s) according to that choice. But if you don t make a clear and complete investment choice, we ll invest the contributions using our own investment fund allocation procedure for your plan. You should have received a copy of our investment fund allocation procedure with this application form. You can get a further copy on request. We ll invest any safeguarded rights in the same way as you ve asked us to invest any protected rights in this section. Investment management (self-invested element only) If you re going to make contributions to the self-invested element of your plan, you need to let us know who will be managing this investment. The three possible options are: Execution only you don t appoint an investment manager but you decide what your investment strategy is and give us written instructions about your investments. Advisory you appoint an investment manager only to give you independent investment advice but not to handle any investment money. However, you can authorise them to give us written instructions about your investments (subject to appropriate agreement). Discretionary you appoint an investment manager to deal with all aspects of your investments, including making all investment decisions and handling all investment money. Page 9 of 12

10 If you appoint an advisory or discretionary investment manager, they ll need to enter into an investment management agreement with us. You or your investment manager should send all instructions about where to invest self-invested element contributions, in writing, to: AEGON Scottish Equitable c/o Capita SIP Services 141 Castle Street Salisbury SP1 3TP Death benefits It s important that you consider completing this section as it may affect who will receive any lump sum benefits from your plan when you die. Insured element It s important that you consider writing the insured element under individual trust. If you do this, the people you appoint as trustees will decide who receives any lump sum death benefit. If you don t do this, the scheme administrator will decide who gets these, so you should consider completing the death benefit nomination table. Self-invested element It isn t currently feasible to write the self-invested element under individual trust in a reasonable manner. As it s the scheme administrator who will decide who gets any lump sum death benefit from this part of your plan, you should consider completing the death benefit nomination table. Death benefit nomination This nomination lets the scheme administrator know what your wishes are. However, please note that it s only an expression of your wishes. The scheme administrator has absolute discretion about which beneficiaries you name (if any) it chooses. If you want to nominate more than four people, please give the details on a separate sheet, date the sheet and attach it to your application. This nomination will apply to all plans that you have under the Scheme. For instance, if you re a member of the Scottish Equitable Personal Pension Scheme and have an Individual Personal Pension, a Group Personal Pension, a Retirement Control plan or a life protection with tax relief plan then the nomination will apply to all of the plans you have. Similarly, if you re a member of the Scottish Equitable Self-administered Personal Pension Scheme and have a Flexible Pension Plan, a Retirement Control plan, a Group Self-invested Personal Pension plan or a Reflex Control plan then the nomination will apply to all of the plans you have. Entitlement to protection of benefits (if applicable) You need to complete this section if you re transferring from a pension arrangement or pension scheme through which you re entitled to benefits that built up before 6 April 2006 and those benefits have a protection certificate. Transfer payment (to be completed by the transferring scheme administrator/trustees) The amounts you enter will be the amounts at or about the date you complete this form. The actual transfer payment and the breakdown will be the amount paid when the transfer is made. The transfer payment will include any pension credit amount (including any safeguarded rights) referred to in section 9. You shouldn t include the pension credit amount (including any safeguarded rights) in the amount of non-protected rights and protected rights. Protected entitlement to tax-free cash of more than 25% of the fund If the transferring scheme is a section 32 buyout plan, a planholder may have a right to tax-free cash of more than 25% of the fund value if: they held the section 32 buyout plan at 5 April 2006, and their tax-free cash entitlement under the plan at that time was more than 25% of the fund value If the transferring scheme is an occupational pension scheme or a personal pension scheme, a member may have a right to tax-free cash of more than 25% of the fund value if: they were a member of an occupational pension scheme as at 5 April 2006, and their tax-free cash entitlement under that scheme at that time was more than 25% of the fund value, and the current transfer is being made from either: that scheme, or a subsequent registered pension scheme to which a block transfer was made for the member in relation to those rights Protected entitlement to low pension age If the transferring scheme is a section 32 buyout plan, a planholder may have a right to a low pension age if: they held the section 32 buyout plan at 5 April 2006, and they had an unqualified right under the plan to take their benefits before age 55, and they had the entitlement under the plan before 10 December 2003 or, if the plan started after that, would have done had the plan started before 10 December 2003 If the transferring scheme is an occupational pension scheme or a personal pension scheme, a member may have a right to a protected pension age if: they were a member of a personal pension scheme at 5 April 2006, and they had an unqualified right to take their benefits under that scheme before age 50, and their occupation at that time was one prescribed by Revenue regulations or they were a member of an occupational pension scheme at 5 April 2006, and Page 10 of 12

11 they had an unqualified right to retire before age 55, and they had the entitlement under the scheme before 10 December 2003, or if they joined the scheme after that time, would have done had they joined before 10 December 2003 and, in both cases, the current transfer is being made from that scheme or from a subsequent registered pension scheme to which a block transfer was made for the member in relation to those rights. Conditions for a block transfer A transfer into this receiving scheme can be a block transfer if: a all of the benefits for at least two members of the transferring scheme are being transferred at the same time to this receiving scheme b the transferring member hasn t been a member of this receiving scheme for more than 12 months at the time the transfer is made Please note that where non-protected rights are applied under the Scottish Equitable Self-administered Personal Pension Scheme and protected rights are applied under the Scottish Equitable Personal Pension Scheme, this can t be a block transfer. Pensions and divorce If an earmarking court order applies to this transfer, we won t process this application until we receive copies of the documentation and the details listed. We also reserve the right to decline such a transfer. The relevant amount of pension credit must be calculated at the date of the transfer payment. Permitted transfer For the purposes of retaining enhanced protection for this transfer, this means: all sums and assets or all pension rights relating to the individual under the transferring registered pension scheme are transferred (to avoid doubt, a partial transfer isn t a permitted transfer) the sums and assets or pension rights are transferred to form all or part of the assets of one or more money purchase registered pension schemes (to avoid doubt the Scottish Equitable Personal Pension Scheme and the Scottish Equitable Self-administered Personal Pension Scheme are money purchase registered pension schemes), and where defined benefit or cash balance pension rights are transferred to a money purchase arrangement, the value of the sums and assets received by the money purchase arrangement are actuarially equivalent to the rights being transferred Block transfer A block transfer is a transfer which allows a member to retain, under the receiving scheme, an entitlement they had at 5 April 2006 to: tax-free cash of more than 25% of the fund value, and/or take benefits at a lower age than the normal minimum pension age Self-invested element This is the part of the Flexible Pension Plan that is not invested in a plan or plans with AEGON Scottish Equitable. We ll invest any safeguarded rights in the same way as you ve asked us to invest the protected rights in section 2. Definitions Insured element is the AEGON Scottish Equitable plan or plans taken out as part of your Flexible Pension Plan. Recognised overseas pension scheme means a pension scheme which is established in a country or territory listed in the Pensions Scheme Regulations 2004 (Categories of Country and Requirements for Recognised Overseas Schemes). An overseas pension scheme may also be a recognised overseas pension scheme if it is a description prescribed in those regulations, or if it satisfies any requirement in those regulations. Registered pension scheme means a pension scheme which is registered with the Revenue under Chapter 2 Part 4 of the Finance Act Page 11 of 12
