6 Step 2&3: Retirement Notification Once You notify your supervisor and Human Resources, you will be asked to complete a Retirement Notification Form You can not use annual leave (i.e. vacation or sick time) to extend your retirement date. 6
7 Step 4: MyLife.JHRPS.com As a retiree you will have access to your Retirement Account with John Hancock. Be sure to keep your User ID and password, which is used to log into your account. Need Assistance with your account? Call John Hancock at Press #0 to speak to a representative. 7
8 Step 4: Retirement Application You need to be within your pension plan to start the retirement application process 8

12 Step 4: Retirement Application You can complete your application up to 90 days prior to your Benefit Commencement Date. Meaning you are retiring June 3 rd your pension would start July 1 st 90 days prior to July 1 st is when you can submit your application. 12

17 Step 6: Benefit Continuation 18 Month Continuation Upon retirement you would also be offered medical continuation for up to 18 months or up to age 65 (which ever occurs first). The 18 month continuation costs are available under Leaving the Company. If you elect 18 month medical continuation, you will not have the option of electing retiree medical after the 18 month continuation period ends.
18 Step 6: Benefit Continuation 30 Days before Retirement Medical Under Age 65 Over Age 65 Dental Vision Life Insurance Flexible Spending Account Health Savings Account 18
19 Step 6: Benefit Continuation Medical Coverage Under Age 65: You may have the option of continuing your medical coverage with Farm Credit Foundations. Over Age 65: Your medical coverage will end, you may have the opportunity to purchase supplemental coverage through AARP Dental and Vision Coverage You will have the opportunity to continue coverage for 18 months. You must be enroll in the coverage in order to continue after Retirement. If you cancel coverage during retirement, you cannot re-enroll. 19
20 Step 6: Benefit Continuation After Your Retirement, you will receive information to continue your Life Insurance Coverage Basic/Optional Basic Life: You will have the option of continuing your life insurance coverage with Minnesota Life. Group Universal Life Coverage: You will have the option of continuing your life insurance coverage with Minnesota Life Accidental Death and Dismemberment (AD&D) There is no continuation of AD&D Coverage Long Term Disability LTD Coverage ends if you end your working career Health Savings Account 20
21 Step 7: 401(k) Distributions 21
22 Step 7: 401(k) Distributions You will receive a letter from John Hancock Retirement Plan Services asking you to contact them about your 401(k) account. This letter will arrive within 3-4 weeks after your last paycheck. If your account balance is less than $1,000, you will be required to take a cash distribution or rollover. If your account balance is more than $1,000, your options include the following: Keep your money in your account; Elect to receive a distribution lump sum, partial, installments; Elect a direct rollover / transfer; or Transfer the money to a New York Life IRA. 22
23 Step 7: 401(k) Distributions If you want to set up a distribution of your 401(k) or take periodic withdrawals from your 401(k), you will need to logon to and request a Distribution form (rollover or installments) or Partial Distribution form (periodic withdrawal) be mailed or ed to you. If you have an outstanding 401(k) loan(s), contact John Hancock Retirement Plan Services at or logon to your account through the website at to obtain a loan payoff information. You have the option of continuing to make your monthly loan payment(s) to John Hancock Retirement Plan Services. Or you can default on the remaining balance of the loan making the remaining balance a withdrawal which is subject to federal and/or state taxes.

29 After Retirement If you elect Retiree Medical Continuation, each November you will have the opportunity to change your medical plan election (i.e. change from Standard PPO to Consumer Choice). During this timeframe you will also be notified of the plan changes and premiums for the upcoming year. Upon Turning Age 65, your Farm Credit medical coverage will cease at the end of the month prior to your 65 th birthday. Farm Credit Foundations will send you a communication about 60 days prior notifying you of the transition 29
