5 Benefit Eligibility Employees: Full-time employees are eligible for te Company s benefit plans after 90 days of full-time employment. Te 401(k) plan is open to employees age 21 and over, after completing 1,000 ours of service. Eligible Dependents: Eligible dependents include your legal spouse and cildren up to age 26. Te Company offers medical plan options troug United Healtcare; review your coices and coose te plan wic best meets your needs. You can manage all aspects of your medical plan at Coice Plus Plan Coverage troug te Coice Plus Plan is a more traditional ealt care option wit a deductible, set co-pays and coinsurance amounts. Wit te Coice Plus Plan, you ave te option to elect te Flexible Spending Account (FSA). See details on page 9. Healt Savings Plan Te Healt Savings Plan is a medical plan wit a Healt Savings Account (HSA). Te Company contributes to your HSA once your medical benefits go into effect: $500 for individual / $1,000 for family. Plus, you can contribute to your HSA tax free. See details on page 6. Use your HSA money to pay your out of pocket ealt care expenses. All medical and prescription costs are applied to your deductible; after you meet te deductible, tere are no co-pays and te plan covers 90% wit 10% employee coinsurance. Be a Smart Consumer! Wit te Healt Savings Plan, you ave te option to elect te Flexible Spending Account (FSA), but you may only use FSA funds to pay for dental and vision expenses. Even if you cange jobs, switc ealt plans, or retire, your HSA is yours you own it, and te money carries over from year to year. See details on page 6. 8 Healt Savings Plan wit te Healt Savings Account (HSA) Te Healt Savings Account (HSA) is offered troug Optum Bank to employees wo enroll in te Healt Savings Plan. Remember, you must set your weekly contribution amounts wen you enroll, but you may also increase or decrease your weekly contribution amounts any time trougout te year in Workday. If you coose to enroll in te Healt Savings Plan, you will receive a welcome kit and debit card from Optum Bank to pay for any out-of-pocket expenses. NOTE: You must be enrolled in te Healt Savings Plan to take advantage of te Healt Savings Account. Company HSA Contributions Te Company will contribute $500 for individual coverage and $1,000 for family coverage on te day medical benefits go into effect. NOTE: If you cange your Healt Savings Plan enrollment from individual to family or family to individual based on a qualifying event in te middle of 2018, te Company s contributions will remain at te plan coverage level at wic you initially enrolled. Your Pretax HSA Contributions In addition to te deposits te Company makes, you are able to contribute money on a pre-tax basis from your payceck to your HSA up to te combined annual IRS limit of $3,450 for individual coverage and $6,900 for family coverage. (Tese limits include te Company s contribution of $500 for individual and $1,000 for family.) If you are age 55 or older, you may contribute up to an additional $1,000 per year. HSA Benefit: Triple Tax Advantage Your contributions are made pre-tax, decreasing your overall taxable income. Your account grows tax free troug investments or interest payments. Money is witdrawn tax free wen you pay for qualified ealt care expenses. NEW for 2018: Your pre-tax contribution limits to your HSA ave increased tis year: $3,450 for individual coverage $6,900 for family coverage Using your Healt Savings Account Take advantage of te Healt Savings Plan s lower premiums and contribute te money you save on premiums into your HSA! Your HSA is a tax-free savings account wic you own and te money carries over from year to year. Mail Service Member Select (MSMS) gives you te coice to fill your maintenance medication at a retail parmacy or take advantage of te cost savings and convenience of mail order. Wat appens if I do noting on January 1, 2018? United Healtcare will be mailing out member communications notifying you if you are eligible for tis program. Before you reac your maximum number of two fills on your maintenance medication(s), you will need to eiter move your prescription to ome delivery or continue filling at a retail parmacy. 11 Healt and Dependent Care Flexible Spending Accounts (FSA) WageWorks Te Company s Flexible Spending Accounts (FSA) can elp you save money. By paying for eligible ealt care and dependent care expenses for you or your qualified dependent(s) wit an FSA, you reduce your taxable income and pay no federal, state or social security taxes on te money used for tose expenses. You must enroll eac year to participate. FSA plans contain a Use It Or Lose It provision please plan carefully! Sould you leave te Company, benefits terminate on your last day of employment. FSA plans contain a Use It Or Lose It provision please plan carefully! If you are enrolled in te HSA, you cannot use your FSA dollars for anyting oter tan dental and vision expenses. Healt Care Flexible Spending Account Use your ealt care FSA to pay for eligible medical, parmacy, dental and vision expenses for you and your dependents, including, but not limited to, te cost of co-pays, coinsurance, eye glasses, ortodontia, ciropractic care, and eligible over-te-counter drugs.
