This is your annual opportunity to review and update your benefit elections. Sysco Canada Inc. offers a number of benefits to you and your family, and it is important that you understand all of your options and make the best choices for your family. This newsletter provides you with the resources needed to make those decisions. All changes become effective January 1, 2016
2 What s New for 2016? Over the last few months we have reviewed our benefit program to ensure that it remains comprehensive, flexible, and competitive in our industry. Based on our review, we are pleased to introduce the following positive changes that will come into effect as of January 1, 2016: Good News No Cost Increases: Once again we are pleased to announce that there will be no increases to your benefit premiums for Although market trends show that plan costs continue to be on the rise, for the 3rd year in a row the cost of your health and dental will remain unchanged! An Exciting New Flex Option: This year we are adding the option to put your flex dollars into an RSP right from the start! In the past RSP was only a roll-over option after Wellness dollars had remained unused for 3 years. Starting in 2016 you can now direct your flex to an RSP in the same year that you receive it! This option requires that you have an active SYSCO RSP account open through Sun Life. As such anyone who selects the RSP option will receive a letter of direction and an RSP enrolment form (if required) in January Deposits to your RSP account will happen by April 1st, Reminders If you are NOT making any changes to your benefits or flex dollars your current selections will remain the same. We will no longer be defaulting to HCSA. Flex allocations will follow what was selected the previous year. Optional Life insurances will not be a selection on the Open Enrolment site this year as it is not subject to an annual timeline. You are eligible to apply for or cancel this benefit at any-time. If you wish to add, increase or remove optional/ spousal/dependent life please complete a benefit change form at your convenience. Open Enrolment Checklist Register or sign-into Green Shield online at to view your flex dollar balances, review claim history, sign up for direct deposit, submit claims online and so much more! Review your existing benefit elections and consider upcoming events or changes that may affect your choices for the coming year. Review and compare your spouse s benefit plan to take advantage of coordination and see what works best for your family. Log into the open enrolment site at ht tp://nationalbenefits.sysco.ca in order to allocate your 2016 flex dollars and make any changes to your existing benefit options. 6 Basic life insurance This benefit pays a lump sum of $30,000 to your beneficiary in the event of your death. This coverage reduces to 50% when your each age 65, and ends at age 70, when you leave the company, or when you retire, whichever is earlier. Sysco pays the premium for this benefit. Accidental death and dismemberment (AD&D) insurance This benefit is equal to your basic life coverage and pays a lump sum of $30,000 to you in the event of your accidental injury, or to your beneficiary in the event of your accidental death. This coverage reduces to 50% when you reach age 65, and ends when you reach age 70, terminate your employment with Sysco, or when you retire, whichever is earlier. Sysco pays the premium for this benefit. Optional life insurance In addition to the employer paid basic life insurance of $30,000 you may purchase additional life insurance for yourself and your family members. Additional employee or spousal life insurance requires proof of good health and approval through Blue Cross. Please complete a benefit change form to apply. Employee Optional Life You may purchase additional life insurance in units of $10,000 (with evidence of good health) to a maximum of $500,000. Cost is based on the amount you apply for, your age, and smoking status. Spousal Life You may purchase spousal life insurance in units of $10,000 (with evidence of good health) to a maximum of $500,000. Cost is based on the amount you apply for, your spouse s age and their smoking status. Dependent Life You may purchase life insurance for your unmarried, dependent children under age 21 (or if a full-time student) for an amount of $5,000, 10,000, or $15,000. One premium will insure all of your eligible children, regardless of the number of children you have. You are automatically the beneficiary for any spousal or dependent life insurance that you have. The coverage for any additional life insurances end when you reach age 65, terminate your employment with Sysco, or retire whichever is earliest. You are able to convert any life insurance to an individual policy upon termination. Optional AD&D insurance This benefit is equal to your optional, spousal, or dependent life coverage and pays a lump sum in the event of an accidental dismemberment or accidental death. The coverage terminates when you reach age 65, terminate your employment with Sysco, or when you retire, whichever is earlier. This benefit is entirely voluntary and would be employee paid. Short term disability (STD) insurance This benefit replaces a portion of your income for up to 26 weeks if you are unable to work due to illness or injury. The first five days of illness are paid at 100%. Associates receive 70% of their income up to the monthly maximum benefit for the remainder of the first month of absence. For an absence that continues beyond one month, associates receive 65% of their income up to the monthly maximum benefit for the duration of the absence, up to a total absence period of six months. The coverage ends when you reach age 70; terminate your employment with Sysco, or when you retire, whichever is earlier. Sysco pays the entire cost of 5
7 this coverage. Associates must provide medical documentation for their claim to be approved and consult with the return to work case managers at Blue Cross for the duration of their absence. Long term disability (LTD) insurance This benefit replaces a portion of your income if, after the first six months of short term disability, you remain unable to work due to illness or injury. You have eight options to choose from for LTD coverage enabling you to balance the coverage and premium costs that best meet your needs. The premium for long term disability coverage is employee paid. The available LTD coverage options are: Option 1: a benefit of 50% of monthly income with a two-year duration and 12-month own-occupation definition. Option 2: a benefit of 50% of monthly income with a five-year duration and 12-month own-occupation definition. Option 3: a benefit of 60% of the first $2,500 of monthly income, plus 40% of remaining monthly income with 12-month own-occupation definition, up to age 65. Option 4: a benefit of 60% of the first $2,500 of monthly income, plus 40% of remaining monthly income with COLA and 12-month own-occupation definition, up to age 65. Option 5: a benefit of 50% of monthly income with a two-year duration and 24-month own-occupation definition. Option 6: a benefit of 50% of monthly income with a five-year duration and 24-month own-occupation definition. Option 7: a benefit of 60% of the first $2,500 of monthly income, plus 40% of remaining monthly income with 24-month own-occupation definition, up to age 65. Option 8: a benefit of 60% of the first $2,500 of monthly income, plus 40% of remaining monthly income with COLA and 24-month own-occupation definition, up to age 65. All options include a monthly maximum benefit of $10,000. When on long term disability, an associate s income from all sources (including government payments) cannot exceed 80% of your net pre-disability income. 8 Open Enrolment Instructions Please complete only if you are making a change. Sysco Benefits Update 1. Open the benefit enrolment form at ht tp://nationalbenefits.sysco.ca 2. To login enter your employee number (as it appears on your pay stub) and your OPCO location. For example: Employee Number: Location: Corporate Office 3. Once logged in you should see the benefit enrollment form. 4. Please verify or fill in the following fields: Date of birth Your location Your smoking status Note: A non-smoker is defined as a person who, in the immediate preceding twelve (12) month period has not used tobacco products. Selecting your annual benefit coverage for Health and Dental from the drop down menu select who the coverage will extend to. Opt Out Single (Employee Only) 1 or 2 dependents (Employee + 1 or 2 dependents) 3 dependents (Employee + 3 dependents) 4 or more dependents (Employee + 4 or more dependents) 7
9 Once you have selected your coverage, the flexible spending account field will automatically populate your eligible flex spending dollars. Single - $300 EE + 1 or 2 dependents - $600 EE + 3 dependents - $1200 EE + 4 or more dependents - $1500 Applicable Opt Out amount Please select your annual benefi t coverage: 6. Select how you would like to allocate your annual flex credits. This must be in increments of $ Place your cursor over the dollar amounts beside the Health, Wellness, Premiums or RSP option. Four boxes will appear for you to enter your flex credits as you would like them allocated. 7. Basic Life Insurance - All Full-Time Associates will automatically receive $30,000 of company paid life insurance. 8. Basic Accidental Death and Dismemberment - All Full-Time Associates will automatically receive $30,000 of company paid AD&D. 9. Long Term Disability Coverage - There are 8 options. This is an associate paid, mandatory benefit. 10. Dependent Information (if applicable) For Spousal information, please fill in all fields: If you do not have a spouse, please go to the dependent information and fill in all fields. Note: You can add more lines by selecting Add a Dependent * Please note that any dependents currently listed in the system reflect those entered as of the last time you used this tool. You will need to enter any new dependents or changes you have made since using the online enrolment. If you are new to the tool you will have to enter all dependent information. 8
10 11. Review and verify that all information on your form is correct. 12. Submit Form The Submit Form button is in the top right hand corner of the form. If you receive an error please see #13 A pop up should appear asking Are you certain all information regarding your benefits is correct? Yes or No. Select Yes if you are certain that your benefits are correct. 13. Error Messages If you do not see the above screen something is incomplete or incorrect and you will need to do one of the following: Verify that your flexible spending allocation equals the total. Verify that all information has been entered for your spousal and dependent information. All sections must be complete! Once you have completed, you should arrive at this screen: 15. Save a Copy of your benefit selections In the top right corner, click download PDF to print or save a copy of your enrolment form Congratulations! You have completed your Open Enrolment! 9
11 PLAN MEMBER update GSC S NEW CHANGE4LIFE HEALTH PORTAL IS HERE! GSC s Change4Life health portal is your all-in-one, online path to achieving better health and winning great rewards, but we ll get to those in a minute. Available through your Online Services (like, Plan Member Online Services!), the health portal is a comprehensive resource that supports healthy life choices by providing you with personalized tips, easyto-use online tools, and important information all at your fingertips. You ll get insight into your current health and learn how you can successfully improve it using a small steps approach. And, for those with a diagnosed medical condition (like high cholesterol, hypertension or diabetes, for example), the health portal s tools and information can make it easier for you to manage your condition. Here s just some of the things you can do on Change4Life Complete a health risk assessment survey Receive a personalized health report card (with an overall health score) and a recommended action plan based on your answers to the survey Access targeted health education articles based on your health score Sign up to receive Stick2It medication and other health reminder s Monitor your health and physical activity using innovative web-based tracking tools (including the ability to track your steps and diet) About those rewards As you use the portal and its great tools and features, you ll also be earning points points that can be used to enter for the chance to win great rewards like gift cards for popular retailers of home goods, sports apparel, electronics and more. We will be holding weekly (initially) and monthly (ongoing) draws the more you use Change4Life, the more points you earn, the more chances you have to win rewards. 12 How to coordinate your benefits If you are covered for health and dental benefits under your spouse s benefit plan, you can coordinate with your Green Shield Canada (GSC) benefits to receive up to 100% coverage by accessing both plans. Demystifying COB COB is an abbreviation of the term Coordination of Benefits which refers to coordinating your health and dental benefits between your benefit plan and your spouse s benefit plan to potentially receive more coverage than if you only submitted to your plan. Here s how to submit claims to multiple benefit plans Your claims: First, submit claims to GSC (this is your primary benefit plan). Next, submit the unpaid portion to your spouse s benefit plan (this is your secondary plan). Include a copy of the Explanation of Benefits statement received from GSC, as well as a photocopy of the original receipt(s). Your spouse s claims: First, your spouse must submit claims to their benefit plan (this is your spouse s primary benefit plan). Next, submit the unpaid portion to your GSC plan (this is your spouse s secondary plan). Include a copy of the Explanation of Benefits statement received from your spouse s carrier, as well as a photo copy of the original receipt(s). Your children s claims: If your children are covered under both benefit plans, first submit your children s claims to the plan of the parent whose birthday falls earliest in the year regardless of the year of birth. Next, submit the unpaid portion to the other parent s plan. For example: GSC Plan Member: John Smith (Birthday: September 1, 1970) Spouse: Anne Smith (Birthday: December 10, 1968) Dependent: Baby Smith John was born earlier in the calendar year than Anne, so they should submit Baby s claim to GSC first. If they were both born on the same day of the year, first they should submit the claim to the plan of the parent whose initial comes first in the alphabet. 11
13 Your children s claims if you are separated or divorced: 1. If you have joint custody: First, submit your children s claims to the benefit plan of the parent whose birthday falls earliest in the year regardless of the year of birth. Next, submit the unpaid portion to the other parent s plan. 2. If you do not have joint custody: First, submit your children s claims to the benefit plan of the parent who has custody of the child. Next, submit the unpaid portion to the plan of the parent who does not have custody of the child. 3. If you do not have joint custody and you are re-married: Submit claims in the following sequence: (i) Submit to the benefit plan of the parent who has custody of the child. (ii) Submit any unpaid portion to the plan of the spouse of the parent who has custody of the child. (iii) Submit any unpaid portion to the plan of the parent who does not have custody of the child. (iv) Submit any unpaid portion to the plan of the spouse of the parent who does not have custody of the child. 12
14 Do it yourself Online Claims Submission Sysco Benefits Update The Green Shield Canada (GSC) self-serve website puts quick and easy claim processing at your fingertips you can submit claims whenever you want, wherever you are. Submitting claims is just a click away with real-time adjudication, so you know immediately how much your plan covers for any of the following applicable benefits: Physiotherapy Vision care Foot care (podiatrist and chiropodist) Massage therapy Wide range of medical items Acupuncture Chiropractic services Naturopath Speech therapy Counselling (psychiatrist, psychologist, social worker) Health Care Spending Account expenses How to submit your own claims: 1. Once registered for Plan Member Online Services, select MY CLAIMS/SUBMIT A CLAIM from the left menu. 2. Select the type of claim you are submitting. 3. Enter your complete claim details and click Submit. Note: If your spouse also has coverage with GSC, you can coordinate your claim with the other GSC plan right at the time of online submission. How cool is that? A confirmation page will display, showing your exact claim adjudication results. We encourage you to print a copy for your records. If you are selected for an audit, you will have to submit supporting claim documentation within a set timeframe to get your claim paid and to avoid potential suspension of access to this feature. Your supporting documentation can be uploaded online right from the audit notification. Be unplugged but still connected with GSC on the Go It s here! A new mobile app GSC on the Go brings you many of the cool features of Plan Member Online Services, but optimized for your smartphone or tablet. That means quick and easy access to your benefits plan information anytime, anywhere, 24/7. You must be registered for Plan Member Online Services and accept the online claims submission agreement before you can access GSC on the Go. Then all you need is your GSC ID number and the password you use to log into Plan Member Online Services. 13
15 Here s what you can do with the app Show your electronic GSC ID card to your health services provider, dentist, or pharmacist using your mobile device. Submit online claims for paramedical services. Search for a health provider near you and get directions using the GPS technology on your device. Use Drugs on the Go to check whether your prescription drug is covered under your plan. If your drug requires prior authorization from your doctor, you can download the forms onto your device or them to your doctor all before leaving your doctor s office. GSC on the Go is available for ios (Apple), Android, and Windows smartphones and tablets and it s free. Here s how to download GSC on the Go: 1. Use the internet browser on your device to go to the GSC website at greenshield.ca > What You Need > Online Services > GSC on the Go. 2. Open the applicable app store directly on your device and search for GSC or GSC on the Go. Important Reminders After you make your choices, your benefit selections are locked in for the year unless you have a life event change (marriage, divorce, birth, death, adoption, changes/loss of your spouse s alternate coverage) Dependent children are covered under the plan up until age 21 if they are not attending postsecondary education, and up until age 25 if they are. Proof of enrolment in a post-secondary program/institution is required in order to remain eligible under the plan. Basic Life and AD&D will automatically reduce to $15,000 at age 65 and will cancel at age 70. LTD coverage and all optional life coverage s end at age 65. Unused wellness dollars from 2013 will be expiring at the end of Any remaining balances can be rolled into a Sun Life RRSP. Employees without an active RRSP account will be required to open one.
