I (and Cooperating Users) hereby pledge to strictly adhere to the following Terms and Conditions when using data approved for use by RISS. Furthermore, I am aware that a violation of the following Terms and Conditions will lead to a revocation of authorization provided by RISS. In addition, I will raise no objections in the event that my authorization (and that of Cooperating Users) to use the Micro-data of RISS is fully evoked. I (and Cooperating Users) agree, also, that in such a case RISS may disclose my name (and that of Cooperating Users), as well as a description of the violation(s) committed by me (or other Cooperating Users) on its website. Terms and Conditions for use of RISS Micro-data 1. RISS Micro-data shall be strictly managed during usage CDs and other material provided by RISS shall be stored in a safe area under lock and key for the duration of the usage period. CDs and other material provided by RISS shall not be used at any location other than that indicated in the User's application. RISS Micro-data shall be used exclusively by the person whose signature is appended to this application, and shall not be provided to or used by third parties. RISS Micro-data shall be used exclusively for the analysis objective stated in this application, and shall not be used for any other purpose. RISS Micro-data shall not be compiled on computers or similar devices, except in cases of temporary storage

2 (Form 1: Usage Application) 2. RISS Micro-data shall be returned to RISS immediately upon the end of the usage period. Furthermore, copies and the like of Micro-data stored on computers shall be deleted immediately, and a report confirming such deletion has taken place shall be submitted to RISS. 3. Questions and the like pertaining to surveys shall be directed to RISS. 4. Publication of analysis results shall be carried out through RISS, and such publications shall state that Micro-data was provided by RISS. 5. Two copies of papers and the like created on the basis of RISS Micro-data shall be submitted to RISS. 6. The instructions of RISS shall be followed. 7. RISS shall be in no way responsible for damages or losses arising from use of RISS Micro-data processed for confidentiality

4 (Guidelines for Completing Application Forms) Guidelines for Completing Application Forms If you have any questions on how to complete an application form, please the RISS secretariat. *Notes on application Before making an application, please confirm the Terms & Conditions of usage, as well as penalties applicable in the event of violations. In cases where information entered on an application form is found to be false, it may be deemed that a violation of the Terms & Conditions of usage has occurred. Please take careful note of the qualifications of the applicant and Cooperating Users. The signatures or seals of all Users are required. Before making an application, please acquaint yourself thoroughly with conditions applying to use of RISS Micro-data. Information regarding RISS Micro-data and survey items contained in all such data can be found on the RISS website [ 1 Title and year of survey Please state the title and year of usage of the survey that comprises the relevant RISS Micro-data. 2 Survey forms and purpose of use Please state the title of the research plan in 15 words or less, and circle all means by which research results will be published. Usage is not permitted if results will not be published. In addition, in order to clarify the type of research to be conducted using data provided by RISS, please enter concisely the research objectives, referring to the following guidelines on writing research objectives. 3 Sources of research funding If no specific sources of research funding exist, please circle [Funding not received]. This section exists to clarify that the research has not been outsourced by a for-profit corporation; therefore, please state clearly the provider of the research funding and the title of the funding itself. If the source of funding is a university, please write [University research funding]. 4 Cooperating Users Please state the name, job title and affiliation of all Cooperating Users. (the name of the Cooperating User must be entered by the Cooperating User in signature form) Cooperating Users must sign their own name. Researchers receiving a Grant-in-Aid for Scientific Research are requested to enter their researcher number

Please state the address of the organization to which the applicant is affiliated, as well as the telephone number of the relevant laboratory. As a rule, RISS will make enquiries by . Please check the relevant address regularly during the usage period. 7 Research methods Please state the intended research methods clearly on a separate sheet of paper, referring to the guidelines on writing research methods. Permission to use RISS Micro-data may be denied if this section is not completed clearly. Note that RISS may request revisions of this information. 8 Certificate of employment Please attach a certificate of employment issued by the organization to which the applicant is affiliated. Individual certificates are required for the applicant and all Cooperating Users. Graduate students and research students are requested to submit a certificate of enrollment issued by the organization to which they are affiliated

7 (Research methods) Guidelines for writing research methods *Please give details regarding all research methods. *Please state all research methods hypothesized for use at the application creation stage, even if it is not known whether the relevant method will be used in practice. [Examples of research methods] Research method 1: Analysis of factors determining employment status of the wife of the head of a household The employment status of the wife of the head of a household will be subjected to probit and logit regression analysis. The explained variable is the employment status of the wife.
