1 March / April 2017 Employee Benefits News You Can Use Introducing Florida Blue s Personal Health Advocate We understand that each person has unique health care needs and navigating the health care system is not always easy. So we ve set up specialized care teams, including a Personal Health Advocate, to make it easier to manage your health and maximize your health plan benefits. These services are available to you at no extra cost to help you in your pursuit of health. Nancy Byers, RN, CCM is your dedicated Personal Health Advocate. Locate and research treatments for a medical condition and find best-in class doctors, specialists and facilities Navigate within Florida Blue Assist with referrals Answer questions about test results and treatment recommendations Help prepare patients for healthcare appointments Help members understand their conditions and become active participants in their healthcare Develop care plans Serve as the contact point, advocate and informational resource for members Medicare Seminar When should I sign up? April 12, Sessions 4:00 p.m. & 5:30 p.m. Admin Bldg. - Cline Auditorium The Employee Benefits Dept. is hosting a seminar to help employees who will be turning 65 in the next couple of years understand Medicare, when they should sign up (if still working) and what are the different parts of Medicare. The host speakers will be from the Florida School Retiree Benefits Consortium (FSRBC) Our goal is to help employees be prepared before reaching their Medicare enrollment period. Don t miss this opportunity to learn more about Medicare and when you should enroll. RSVP by calling the Employee Benefits Dept. at Guests are welcomed. Spring Pre-Retirement Seminars April 19 th - 4:00 p.m. & 6:00 p.m. April 20 th - 4:00 p.m. & 6:00 p.m. The Employee Benefits Dept. will be hosting Pre- Retirement Seminars Wednesday, April 19 th and Thursday, April 20 th in the Cline Auditorium at the DCPS Administration Building. Some of the topics discussed at the seminars will be: The difference between the FRS Pension and Investment Plan What is DROP retirement? What is BENCOR? Saving for retirement through a 403(b)/457(b) Savings Plan If you are nearing retirement or just need to start saving for your future retirement, these seminars are designed to provide information on these topics and more. 2 FRS Retirement Seminars We invite you to participate in the free informative employee seminars that are periodically offered by the Florida Retirement System. These seminars provide valuable information and free independent resources. All seminars will be held in the Cline Auditorium. The topic and time for the next seminar is listed below: Investment Plan: Understanding Your Benefits Monday, May, 1 5 PM 6:30 PM For more information and to register for this seminar, call , Option 2 or go online We encourage all employees to take advantage of this free educational seminar. You may also bring a spouse/friend with you. A minimum participation of 25 is required. Retirement Appointment Reminder If you are planning on retiring, entering DROP or exiting DROP, you must schedule your appointment 2 months prior to this date. Contact the Employee Benefits department at (904) to schedule your appointment. Important reminder to employees considering retirement: Make sure your insurance coverage for the plan year in which you plan to retire reflects the coverage level you want to continue during retirement. For example, if you carry employee only health coverage, at retirement you cannot add your spouse to the coverage. Florida Blue Preventive Care Services Covered at 100% Certain preventive health care services for adults and children are covered with no cost share for you when provided by a network provider. Some of the preventive health services include, but are not limited to, periodic routine health exams, routine gynecological exams, immunizations and related preventive services such as Prostate Specific Antigen (PSA), routine mammograms and pap smears. Visit the Centers for Disease Control and Prevention (CDC) for recommended vaccines and immunizations. If you have questions about coverage for preventive services, log in to your Florida Blue account online at or call Member Service at Did you know? PayFlex has a Mobile app Manage your account and view alerts Snap a photo of your receipts to submit claims View PayFlex debit card transactions View common eligible expense items, and more Change in Status Event Under certain circumstances you may be allowed to make changes to your benefit elections during the plan year, such as adding or dropping dependents, depending on whether or not you experience an eligible qualifying event as determined by the Internal Revenue Service (IRS) Code, Section125. You must complete a Change in Status enrollment and provide supporting documentation to the Employee Benefits Department within 60 days of the qualifying event. 3 DIABETES MANAGEMENT PROGRAM You are invited to participate or continue in one of two diabetes programs created especially for the employees of Duval County Public Schools (DCPS). DCPS understands that just one program may not fit the needs of everyone. That is why Florida Blue, North East Florida Endocrine and Diabetes Associates (NEFEDA) and DCPS are partnering to offer two free, voluntary programs for you to choose from to take steps to be as healthy as you can be! Comprehensive Program at NEFEDA Each participant must adhere to the program requirements below: Complete FREE biometric screenings including laboratory testing, body mass scanning and metabolism testing at Northeast Florida Endocrine & Diabetes Associates (NEFEDA). Meet with a NEFEDA endocrinologist, diabetes educator and other medical professionals to set health goals. Participate in Wellness life! A 15-week diabetes group program presented by NEFEDA. Groups meet weekly for 1 hour. Meet with a NEFEDA endocrinologist and diabetes educator on a quarterly basis to manage your health goals. NEFEDA has four locations: Downtown: 915 W. Monroe St. Jacksonville, FL Southside: 3550 University Blvd. South Jacksonville, FL Orange Park: 1635 Eagle Harbor Pkwy Orange Park, FL Beaches: 1375 Roberts Dr. Jacksonville Beach, FL How to get started Call NEFEDA at (904) 355-WELL ( ) to register. Please note: Quarterly coaching (telephone or face to face) and biometric screenings must be completed after the 15-week program has ended to continue to receive medications, supplies and support at no cost to you. Diabetes Program through Florida Blue Each participant must complete the program requirements listed below: Attend one 8 ½ hour diabetes education class and then a two-hour refresher class each year thereafter. Your spouse or one adult family member may also attend. Note: Classes have limited seating. Participants are assigned to class dates based on the order of calls received. Complete a FREE blood screening test at a Quest Laboratory within two months after you complete the class. The following American Diabetes Association (ADA) recommended screenings must be included: o HbA1c o Lipid profile o Microalbuminuria Have a diabetic foot exam by a family physician, podiatrist or endocrinologist three months before or after attending the class and then annually thereafter. Have a dilated eye exam three months before or after attending the class and then annually thereafter. Talk with your health plan s nurse educator by phone, at least every three months. Please note: Each year, the ADA recommended screenings, physician exams and a diabetes refresher course must be completed to continue to receive medication, supplies and support at no cost to you.
