4 Health+Savings Are you ready to make the most of Health+Savings? It s like two plans in one: a medical plan that provides coverage and a Health Savings Account (HSA) that helps you pay for care. Follow this roadmap so you know how to get started and what you can expect when you get care. Finish opening your HSA 1 2 You may need to provide more information to PayFlex, our HSA administrator. You can use your HSA debit card, pay another way and then reimburse yourself, or pay your provider with a check from your HSA. See page 13. 3

6 Health+Savings details Health+Savings gives you more control over how and when you pay for health care. It s more than a medical option it can also work like a 401(k) for your health care expenses. That s because there are two unique parts to Health+Savings: { Part 1 } A high deductible health plan (HDHP) provides comprehensive medical coverage when you need it. It s a Preferred Provider Organization (PPO) plan just like the Standard and HealthPlus Options. { Part 2 } A Health Savings Account (HSA) gives you a way to pay current or future out-of-pocket medical expenses using before-tax dollars. How the Health+Savings Option works You can use any doctor or hospital, but you ll pay less when you use in-network providers. In-network preventive care and certain generic preventive drugs are covered at 100%. For non-preventive medical services and prescription drugs, you have an annual deductible to meet before the plan pays benefits. Unlike with the HealthPlus Option, the full family deductible applies to the You + Child(ren), You + Spouse and You + Family coverage options. After you pay the annual deductible, you and BP share the cost of services you receive through coinsurance and copays. Once you reach the out-of-pocket maximum* for the year, BP pays 100% of eligible expenses for the rest of the year. To avoid the lower out-of-network benefits, use the BP customized DocFind tool from Aetna to ensure your doctors and facilities are in-network. Go to to access. 8 How the HSA works Once you have money in your HSA, you can use it to pay for eligible health care expenses for you and your eligible family members, including things like deductibles and coinsurance. You choose how to use it. You control your HSA, so you have the flexibility to decide how, when and where to spend your health care dollars. It helps you save. If you don t spend the money in your HSA, it can continue to grow, year after year, for use in the future. Your HSA earns interest, and the earnings on your HSA balance are also tax-free. It s yours to keep. Unlike with the Health Care Spending Account (HCSA), there s no use it or lose it rule. And your HSA is portable it belongs to you. This means that even if you change health plans, take a new job or retire, you can still use the money in your HSA to pay for eligible expenses. 7
9 { { Reminder: Always use your Aetna and Express Scripts cards to receive discounted rates and to ensure that what you are paying for is being applied toward your deductible when applicable. Savings like no other Yes, the Health+Savings Option has a higher deductible and out-of-pocket limits. But, it costs you much less than other options, BP helps you earn funds for your HSA, and the HSA gives you a triple tax advantage: Your contributions are automatically deducted from your paycheck before-tax. + Your HSA earns interest tax-free. You can also invest your funds for more potential earnings. + You can use tax-free money to pay for eligible medical expenses. HSA and your taxes Although your HSA contributions and distributions (paid claims) don t increase your income or taxes, you ll need to note both during your annual tax filing. 10 Getting started Step 1: Finish opening your HSA When you enrolled in Health+Savings, Aetna sent your information to PayFlex, our HSA administrator, so PayFlex could begin setting up your account. However, PayFlex needs to verify your identity before they can finish the process. PayFlex will send you a welcome letter to let you know your HSA has been created. In some cases, PayFlex may need additional information before your HSA can be opened. In this case, PayFlex will send you a letter within three days of receiving your eligibility information. The letter will explain what information is required to set up your HSA. It will also ask you to provide proof of your name, Social Security number, home address and date of birth. PayFlex cannot open your HSA until you provide this information. If you don t respond within 30 days, PayFlex will send you two reminder letters. If you do not respond to these letters, PayFlex will close your HSA enrollment request. Once your HSA is open, you ll need to enter a few more details on the Inside the HSA section on the website. For detailed instructions, refer to the Health+Savings section on the homepage of LifeBenefits at bp.com/lifebenefits. Note: You will first need to register your account. If you re registering before the effective date of your HSA, go to If you re registering on or after your effective date, go to Aetna via bp.com/lifebenefits. If you have any trouble with the registration process, please contact Aetna customer service at and select the HSA option. You will be connected to a PayFlex customer service representative who can assist you. 9
11 Step 2: Activate your HSA debit card Once your account is open, you ll receive a welcome letter and HSA debit card in the mail from PayFlex. The debit card gives you access to your HSA dollars. Follow the instructions that come with your card to ensure it is activated. Then sign the back of the card and it s ready for you to use. See page 13 for more information about how to pay with your HSA debit card. Step 3: Manage your HSA You re in complete control of your HSA you decide how and when to use it. Log in to the Aetna Navigator member website at to track your HSA balance, see how much you ve paid toward your Health+Savings deductible, view claims, make post-tax contributions to your account and much more. Note: You must contact the BP Benefits Center if you want to change your HSA contributions through payroll deductions. Details are on page 11. PayFlex Mobile The PayFlex Mobile app is available for Android TM and iphone mobile digital devices and BlackBerry smartphones, and it gives you virtually 24/7 access to your: Account balances Claims processed Transaction details How to register Your welcome letter will tell you how to register your online account. Once you do that, you can register for the PayFlex Mobile app. You will use the same username and password for both. Once the registration is complete, you can download the PayFlex Mobile app to your smartphone from your phone s app store, then log in. If at any time you forget your password, simply log in to your PayFlex account via Aetna Navigator and you will be able to reset it. 10
12 Growing your HSA You can use your HSA to get a head start on saving for the future. You can grow your HSA in four ways: 1. BP s contributions for completing a Physician Certification Form and returning it to StayWell. BP contributes $1,000 for employees and $1,000 for covered spouses/domestic partners that s up to $2,000 per family! See page 12 for details. Timing: BP will make a contribution to your HSA approximately 30 days after you turn in your completed form to StayWell. State taxes may apply HSA tax laws vary by state. Seek the advice of a tax advisor or go to for more information. 2. Your before-tax contributions, which are paid through convenient before-tax payroll deductions. The maximum you can contribute is up to $3,250 if you cover yourself, or $6,450 if you cover dependents too. If you are age 55 or older, you can also contribute up to another $1,000. BP may make contributions into your HSA too. Therefore, the amount you contribute plus the amount that BP contributes cannot be more than your annual contribution limit. { { TIP: To help you decide how much to contribute, use the HSA savings calculation tool, available in the HSA section of the Aetna Navigator member website at You can also change your monthly contributions at any time by logging on to the BP Benefits Center and clicking the Life Events tab. 3. Tax-free interest your account earns. 4. Investment earnings. Once your HSA balance reaches $1,000, you can begin investing. You can invest any amount over the $1,000 minimum balance required. For example, if you have $1,500 in your HSA, you can invest up to $500. For more details, see the Investing your HSA section on page 16 of this guide or log in to the Aetna Navigator member website at and access the HSA section. 11

It s 100% covered when in-network, and BP will contribute $1,000 to your HSA if you get your annual physical and complete and return a Physician Certification Form to StayWell. Your physician must certify that three out of the five of your metabolic syndrome screening results are within normal ranges. If you don t meet the minimum metabolic syndrome screening requirement, you can still qualify for the BP contribution if: Your physician documents on your form that those areas are being managed; or You complete a specially designed coaching program through StayWell. If you enroll your spouse/domestic partner in your Health+Savings Option and if your spouse/domestic partner also completes a Physician Certification Form, he or she will also earn $1,000, for a family total of $2,000. If your spouse earns the incentive, that money will go into your HSA. If your domestic partner earns the incentive, that money will go into his or her HSA (not yours). Note: There s no minimum metabolic syndrome screening requirement for spouses/ domestic partners they only need to complete the Physician Certification Form. How to earn your $1,000 Step 1 Download and print the Physician Certification Form. To access your form, go to the StayWell portal via bp.com/lifebenefits, select My Points, then click the link to download the Physician Certification Form in the New! HSA Funding section. (Available April 1) Download and print your form. Each form is specific to the individual; therefore, you must print out your own form. Step 2 Have your form completed by your doctor at your annual physical appointment. Your physician can also have his or her Physician s Assistant (PA) or a Nurse Practitioner (NP) complete the form. Step 3 Return your completed form to StayWell. Follow the instructions on the form and mail or fax your completed form to StayWell. BP will make a contribution to your HSA approximately 30 days after StayWell receives your completed form. This form must be submitted by December 15 to earn your contributions for the calendar year. BP Physician Certification Form (Employee) For employees in the Health+Savings plan, please complete this form with your doctor to determine eligibility for your HSA incentive ($1,000). In the event that StayWell's services are transitioned to another service provider, StayWell may deliver my Personal Information to the successor provider to maintain a continuity of services for me. Metabolic Syndrome Screening: Metabolic Syndrome is a cluster of five risk factors that raise your risk of developing cardiovascular disease and diabetes. Several national health care organizations, including National Institutes of Health (NIH) have established a consensus definition of Metabolic Syndrome. Metabolic syndrome is diagnosed when you have 3 or more of the following risk factors: 1. Waist Measurement > 40 inches for men or > 35 inches for women 2. Blood Pressure > 130/85 mm Hg 3. HDL Cholesterol < 40mg/dL for men or < 50mg/dL for women 4. Triglycerides > 150 mg/dl 5. Fasting Glucose > 100 mg/dl* Completed by Doctor s office: Please print clearly in dark ink. 14 Spending your HSA You ll get more from your HSA if you know what to expect when you receive care and when you pay for it. Keep receipts Always remember to save your receipts when you make payments from your HSA, in case you need to provide proof of your eligible expenses to the IRS. Ways to pay With an HSA, you need to have a balance to be able to use it. If you have enough money in your HSA to cover an expense, you have three ways to pay for eligible expenses: 1. Pay with your HSA debit card. 2. Pay for the expenses with other funds, and then reimburse yourself. Log in to Aetna via the BP Benefits Center at bp.com/lifebenefits. Select File a Claim, then Pay Me. 3. Pay your provider directly with a check from your HSA. Log in to the HSA section of the Aetna Navigator member website at and click on Make a Payment under the My Account menu. What to do when you get care or fill a prescription At the doctor s office: Present your Aetna medical ID card (not your HSA debit card) at your doctor s office to get discounted rates. Ask your doctor to file a claim with Aetna and bill you after Aetna processes the claim. Then, you can use your HSA or pay another way it s your choice. At a retail pharmacy: Present your Express Scripts ID card each time you drop off a prescription. This helps to ensure you receive the discounted rate. If payment is due, you choose whether to pay with your HSA or use other funds. Never postpone care It s never healthy to postpone receiving care because you want to avoid paying for services out of your pocket. Even though it may feel like you re paying more as you pay your deductible, you re actually saving money on your medical premiums every single paycheck. 13

Log in to the Aetna Navigator member website at and use the Member Payment Estimator. If a doctor asks you to pay at the time of service If your doctor isn t in the Aetna network, you may be asked to pay the full cost when you receive the service and submit your claim to Aetna. If your Aetna network doctor asks you to pay in the office, here s what you can do: For preventive care services: Tell your doctor s office about Health+Savings. Explain that it has no deductibles or copays for preventive care, and you re not expected to pay at the time of service. For non-preventive care services: Confirm your copay, or ask the office staff to verify with Aetna the amount you owe. They can do this while you wait by calling the number on the back of your medical ID card. Pay for the service. You can use your HSA debit card or another form of payment. Log in to the Aetna Navigator member website at to verify the amount. Make sure what you paid matches what Aetna says you owe. If the doctor charged you more, ask for a refund. 14
16 Eligible health care expenses When you re ready to use your medical plan, be sure you know the kinds of expenses that can be covered by Health+Savings and your HSA. What s covered in Health+Savings After you pay the annual deductible, the Health+Savings Option covers these types of medical expenses and you only pay the applicable copays or coinsurance: Doctor s office visits Maternity care and delivery Inpatient and outpatient hospital care Inpatient and outpatient surgery X-rays and medical imaging Lab tests Prescription drugs Emergency care Remember: When you use Aetna network providers, preventive care services are covered at 100%. Other HSA eligible expenses If funds are available, you can use your HSA to pay your deductible, coinsurance and copays. You also can use your HSA to pay for qualified, IRS-approved medical expenses such as: Acupuncture Chiropractic care Contact lenses and solutions Dentures Eye exams and glasses Hearing exams and aids Infertility treatments Laser eye surgery Orthodontia Smoking cessation programs For a complete list of eligible expenses you can pay from your HSA, visit and see IRS Publication 969, or call the IRS at
17 Investing your HSA Did you know that your HSA can earn more for you? PayFlex offers you two ways for your HSA to grow. Interest Your HSA cash account earns interest. And your savings are protected in this FDIC-insured account. When you contribute to your HSA, your funds earn interest over time. Investments Once you have a balance of $1,000 in your HSA, you can open an investment account. You can invest any of your HSA funds over this $1,000 in an array of mutual fund options chosen by the bank. To invest your funds, you must keep at least $1,000 in your HSA. If you need funds to pay for an eligible expense, you can always transfer funds back to your cash account. Investments are not FDIC-insured. By transferring funds into an HSA investment account, you can potentially benefit from capital appreciation in the value of mutual fund holdings. However, you will also be exposed to a number of risks, including the loss of principal. To familiarize yourself with these risks, you should always read the prospectuses for the mutual funds you intend to purchase. When you have an investment account, you will be able to take advantage of the following benefits: Access to well-diversified mutual funds professionally managed by an independent investment advisor. Integration with Morningstar research data (a prospectus can be found online). 401(k)-like investment capabilities with no transaction fees. Once you have invested your fund, a monthly fee of $1.75 will apply. BP pays this fee for active employees. For domestic partners and retirees, this fee will automatically be deducted from your cash account. 16
18 Investment fund options All funds available for investment are traded at net asset value ( NAV ). NAV per share is computed daily. This is based on the closing market prices of the securities in the fund s portfolio. All mutual funds buy and sell orders are processed at the NAV of the trade date. How to get started Log in to the Aetna Navigator secure member website at 1. On the left side of the screen, select My Accounts and Services. Under Health Savings Account, click on View My Account. 2. From the left side of the screen, select Investments. 3. Next, click on Enroll Now, review and agree to the terms and conditions, and submit. 4. If your balance is at least $1,000, follow the online instructions to move any funds over the $1,000 minimum balance into one or more of the available options. You can also set up your funds to automatically invest all or a portion of your future contributions. The HSA investment account is an optional, self-directed service. Aetna does not provide investment advice for HSA investment account participants. You are solely responsible for any investment account decisions you make. Mutual funds and brokerage investments are not FDIC-insured and are subject to investment risks, including fluctuations in value and the possible loss of the principal amount invested. The prospectus describes the funds investment objectives and strategies, their fees and expenses, and the risks inherent in investing in each fund. Investors should always read the prospectus carefully before making any investment decision. System response and account access times may vary due to a variety of factors, including trading volumes, market conditions, system performance, and other factors. 19 Making your HSA last You re already paying less per paycheck for your medical coverage. Use these tips to save more and stretch your HSA even further. Schedule your preventive care. In-network preventive care like annual physicals, wellness screenings, immunizations and well-baby care is 100% covered without having to pay your deductible. Put your premium savings in the HSA. You re paying over 60% less now that you re in the Health+Savings Option consider putting your savings in this account. Use Aetna network providers. Aetna network providers charge prenegotiated rates, so you pay less for care when you use an Aetna provider. Get the right care at the right price. A trip to the emergency room may not be the best choice. Your costs will be much higher than if you first called your doctor or went to a convenient care clinic or urgent care center. Save on prescription drugs. Generic versions of prescription drugs are as safe and effective as brand-name drugs, because the active ingredients are chemically equivalent to those of the brand-name counterpart. But the generic version may cost as much as 70% less! If you take ongoing maintenance medications, use the mail order program. Just ask your doctor to write a refillable 90-day prescription, and submit your prescription to Express Scripts. Once Express Scripts receives your order, it will be delivered to your door generally within 14 days. Log in to for help with setting up mail order. Work with your doctor to understand your treatment options and their costs. Don t be afraid to ask questions. Make a list of questions before your appointment, and ask for more detail if you don t understand your doctor s explanation. Ask your doctor about the cost of procedures (e.g., X-rays, surgeries and tests). By doing so, you may be able to proactively find lower-cost options. Be a better health care consumer! Participating in the BP wellness program Employees hired on or after January 1, 2014, as well as employees on international assignment, are encouraged but not required to take the Annual Enrollment Health Questionnaire in If you participated in the HealthPlus or Health+Savings Option in the 2013/2014 plan year, both you and your covered spouse/domestic partner (if applicable) must each have earned 1,000 points in the BP wellness program by December 31, 2013 and complete the Annual Enrollment Health Questionnaire during annual enrollment to maintain eligibility for those options in 2014/2015. For new enrollees to the plan in the 2013/2014 plan year, you and your covered spouse/domestic partner (if applicable) are only required to complete the Annual Enrollment Health Questionnaire during annual enrollment, after which you and your covered spouse/domestic partner must each earn 1,000 points during the 2014/2015 plan year to continue to be eligible for the HealthPlus and Health+Savings Options in 2015/2016.
