1 Retiree Medical FAQs For Retirees The following questions and answers are general in nature and are intended to provide basic information to retirees regarding the change to Dominion s Retiree Medical coverage. These questions and answers apply to retirees who will transition to a Dominion-funded Health Reimbursement Account through Extend Health. If you have specific questions regarding this change or to obtain additional information, please call Extend Health at Note: You ll see and hear different names associated with our new Medicare exchange: Extend Health, a Towers Watson Company; Towers Watson; Towers Watson s OneExchange; and OneExchange. E 2: I am retired and my spouse is under 65 and I am over 65 (or vice versa). Do we need different coverage? E 3: I am a retiree who is age 65 or older, and I cover my spouse who is under age 65 and not yet eligible for Medicare. Will my spouse s premium for the Dominion medical plan still be automatically deducted from my pension check? E 4: Will I be able to change insurance coverage as my needs change by selecting different carriers or plans? In other words, once enrolled with a plan selection in 2014, can I change my plan selection for 2015? E 5: I am a Dominion retiree and my spouse is still actively employed by another employer. Can I be covered on my spouse s medical plan instead of buying a plan through Extend Health? If I don t enroll in a plan through Extend Health, can I come back and get an HRA when my spouse s employment ends? 3 General G 1: Why are we moving away from Dominion s traditional retiree medical plan? In recent years, high-quality health coverage for Medicare-eligible retirees has become more affordable in the open market, largely due to changes in Medicare rules and increased competition. At the same time, costs under employer-sponsored retiree medical plans like Dominion s have continued to rise. In considering these changes, like many other employers, we have determined that Dominion can provide better medical benefits for our Medicare-eligible retirees and spouses by subsidizing the cost of open-market plans instead of maintaining a single Dominion sponsored plan. Dominion is not eliminating retiree medical coverage for its retirees we are merely changing the way the benefit is provided. Today retirees have a single choice the Dominion retiree medical plan. Beginning on April 1, 2014, retirees will have access to numerous plans in the open market, which will allow the retiree to choose a plan that fits their medical needs. G 2: How does this change benefit Dominion? Dominion s future costs of providing retiree medical coverage will be lower because Dominion s costs will be limited to the amount contributed to the Health Reimbursement Accounts (explained below) on behalf of eligible retirees. For our retirees, our analysis has shown that with this HRA funding from Dominion, most retirees will be able to purchase insurance that provides equivalent or better benefits, with many plans available at a lower cost than the current Dominion retiree medical plan. G 3: Who is impacted by this change? This move to Extend Health affects most non-union retirees and their covered spouses who retired on or after January 1, 1993, once they have reached age 65. G 4: Who is not impacted by the change? Retirees and their covered dependents who are under age 65. Certain retirees in Pennsylvania, Ohio, and Arizona who currently participate in a Medicare HMO instead of Dominion s group plan are also not impacted by this change. The change is not being applied to retirees from the Millstone Power Station because of a pending union election at that station. G 5: Why doesn t this change apply to Millstone retirees? Because of the pending union election, this change is not being applied to active employees at Millstone or to former employees who retired from Millstone. Federal laws governing union elections require that Dominion maintain the status quo at the plant by refraining from enhancing the terms and conditions of employment before the election takes place. Dominion does not want to do anything that could be perceived as improperly influencing the election. G 6: Will this change apply to current and future Millstone retirees after the election? The legal restrictions applicable during union campaigns prohibit us from making any statements or promises about what will take place after the election. Because of this, we feel it would be improper to comment on that while the election is pending. G 7: Why doesn t this change apply to union retirees? Dominion has notified the unions about the change, and we will be talking to them about it at the appropriate time. G 8: When do I have to choose new coverage in order to be covered on April 1, 2014? Enrollment begins on January 2, 2014 and ends on March 31, You must enroll in a new plan by March 31, 2014 so that your new coverage can begin effective April 1, Regardless of the plan you choose, you should complete your enrollment by March 31, 2014 to ensure that you do not face any penalties or coverage gaps. G 9: What happens if I don t enroll in a plan through Extend Health before April 1, 2014? If you are age 65 or older your Dominion retiree medical plan coverage will end on March 31, In order to receive a Dominion-funded Health Reimbursement Account, you must enroll in a plan available on the Extend Health private exchange. 12/05/2013 3

4 G 10: Does this change affect my current retiree life insurance through Dominion? No. If you currently have retiree life insurance benefits through Dominion, those benefits are not impacted by this change. G 11: Where can retirees go for more help or information? Beginning on December 10, retirees can call Extend Health Dominion Helpline at for additional information and help enrolling in coverage. Retirees can also visit the Extend Health website at for additional information. G 12: I was hired after 2007 and therefore I am covered under the Retiree Medical Account. How does this change affect me? You will keep your RMA balance. You may use your RMA to purchase plans through Extend Health, if you wish. However, you will not receive any additional HRA contributions from Dominion. Medicare M 1: How does Medicare work with the plan that I choose through Extend Health? Medicare may work differently depending on the type of plan you choose. You can choose coverage under a Medicare Advantage plan or a Medigap plan and Part D Rx plan. The Extend Health advisors will help you decide which plan works best for you based on your medical needs. A Medigap plan fills the gaps in original Medicare Part A and Part B coverage, meaning it helps pay the difference between the total costs and the amount original Medicare pays. These plans provide additional coverage for doctor visits and hospital stays as well as other expenses partially covered by original Medicare. Medigap plans do not provide prescription drug coverage. A Part D Rx plan provides prescription drug coverage. These plans help pay for prescription drug expenses. A Medicare Advantage with Prescription Drug coverage (MAPD) plan provides an all-in-one plan that bundles Part A, Part B and prescription drug coverage together with additional benefits. These plans provide coverage for doctor visits, hospital stays and prescription drug expenses. Just like Dominion s current retiree medical plan, you must have Medicare Part A and B to enroll in the plans described above, which are administered by private insurance companies. Part A is generally free, but you must pay a premium for Medicare Part B. M 2: Do I need to keep paying my Medicare Part B premium? Yes. In order to qualify for a Medigap or a Medicare Advantage plan, you must be enrolled in and continue to pay for Medicare Part B, just as you would be required to do under Dominion s current retiree medical plan. M 3: My premiums are taken out of my Social Security checks. Will my current Medicare Part A and B coverage change? Your Medicare Part A and B premiums will not change (except for annual increases on January 1) and will continue to be taken out of your Social Security checks. You may choose to use your Dominion-funded HRA to receive reimbursement for your Medicare Part B premium. M 4: How can I get more information about Medicare? You can call MEDICARE ( ) or visit Medicare s website at Each fall, you will also receive from Medicare the federal Medicare and You Guide, which includes important information on your health insurance options. Individual Coverage IC 1: What are health insurance exchanges? Health insurance exchanges, or marketplaces, are entities that create a more organized and competitive market for health insurance. They offer an assortment of health plans, establish rules for offering and pricing insurance, and provide information to help consumers better understand their available options and enroll in plans that fit their 12/05/2013 4

5 needs. Dominion has engaged Extend Health to provide retirees access to its private exchange; Extend Health will help retirees understand their options and choose a plan that fits their needs. IC 2: What kinds of plans are available through the Extend Health exchange? Medicare Advantage, Medigap (Medicare Supplement), Medicare Part D prescription drug coverage, and dental and vision plans are all available through Extend Health. IC 3: Will the Extend Health plans be as good as the Dominion medical plan? Extend Health operates the nation s largest private health insurance exchange for Medicare-eligible individuals. Extend Health works with the top national and regional insurance companies to ensure that retirees will have quality individual plan options. The multiple options give retirees the ability to find a plan that closely matches their specific needs, and Extend Health benefit advisors will help you find a plan that best suits your needs. Our analysis has shown that with HRA funding from Dominion, most retirees will be able to purchase insurance that provides equivalent or better benefits, with many plans available at a lower cost than the current Dominion retiree medical plan. IC 4: Since the new coverage will start on 4/01/2014, what happens to my deductibles under the Dominion s medical and prescription drug coverage? Will they transfer to the new coverage? If you are age 65 by January 1, 2014, your deductibles under Dominion plans, both medical and prescription drug, will be waived for January through March If you turn age 65 between January and March 2014, your remaining deductible will be waived starting the first of the month you attain age 65. (Deductibles for retirees who are covered today under Dominion s Medicare HMOs will not be waived.) IC 5: Can I be charged more for individual post-65 medical or prescription drug coverage, or be turned down for coverage entirely, based on my health status? No, not if you enroll for individual coverage effective April 1, If you do not enroll in coverage effective April 1, 2014, and later decide to enroll in individual Medigap (Medicare Supplement) coverage, you may pay higher premiums or be denied coverage, depending on the plan you choose. Medicare Advantage plans cannot turn you down or charge you more for coverage based on your health status with one exception: if you seek to enroll in Medicare Advantage coverage after you have been diagnosed with End Stage Renal Disease, the plan may deny your enrollment application. Part D plans cannot turn you down for coverage based on your health status, but may charge late enrollment penalties if you do not enroll when you are first eligible to elect Part D. IC 6: How will a preexisting medical condition impact my cost or my ability to obtain medical insurance? If you are over 65 and eligible for Medicare and a Dominion HRA, a preexisting medical condition will not impact your ability to obtain or the cost of your medical insurance if you enroll prior to April 1, The January 2 March 31, 2014 enrollment period for retirees who are at least age 65 is a special enrollment period during which the insurance carriers will provide policies to Dominion retirees on a guaranteed issue basis. Similarly, retirees under age 65 have a special enrollment period when turning age 65. The cost of the insurance varies based on the carrier, plan and area. Some Medicare Supplement plans also have costs that vary by age. Extend Health benefit advisors will answer your questions regarding the medical plans and costs in your area and assist you in choosing a plan that fits your needs. IC 7: Are there benefit limitations on pre-existing conditions under individual post-65 medical and prescription drug plans? No, provided you enroll for individual coverage effective April 1, However, if you do not enroll in coverage effective April 1, 2014, and later decide to enroll in individual Medigap (Medicare Supplement) coverage, there may be limitations on your benefits for pre-existing conditions. Medicare Advantage and Part D plans cannot apply preexisting condition exclusions. IC 8: If I don t like my new plan can I switch next year? In many cases, you will be able to switch from one plan to another annually. However, the ability to switch plans varies by the plan you select. Many plans offer the ability to make changes annually thereafter from November 15 through December 31, subject to all of the various Medicare rules and regulations, which are established by the Federal Government. The Extend Health benefit advisors can provide additional information about these rules and how they affect you. 12/05/2013 5

6 IC 9: Does Extend Health offer dental and vision insurance plans? Yes. IC 10: I have Veterans Administration (VA) drug benefits. Do I have to buy prescription benefits through Extend Health? To receive your HRA funding from Dominion, you only need to buy a medical plan through Extend Health (i.e., a Medicare Supplement or Medicare Advantage plan). Purchasing a Medicare Part D prescription drug plan is optional. IC 11: Do I have to choose a plan offered by Extend Health? To receive your HRA funding from Dominion, you must elect a plan offered by Extend Health when you are first eligible to do so. This same rule applies to your spouse; he or she must also enroll in an Extend Health plan when first eligible to do so in order to receive an HRA contribution at any time in the future. Eligibility E 1: What if I cover both a Medicare-eligible and a pre-medicare individual on my Dominion retiree medical plan? Dominion will contribute an HRA amount for each retiree and covered spouse who is age 65 provided that they enroll in individual plans through Extend Health. Individuals who are not age 65 will remain on the Dominion group medical coverage until they become age 65. Contact the Dominion Benefits Center at with questions on health care coverage for pre-65 individuals. E 2: I am retired and my spouse is under 65 and I am over 65 (or vice versa). Do we need different coverage? Yes. The person who is at least age 65 should choose a new plan with Extend Health for coverage beyond Medicare Parts A and B. The person who is under age 65 is not yet affected by this change and will automatically continue to be enrolled in Dominion s retiree medical plan until they reach age 65. E 3: I am a retiree who is age 65 or older, and I cover my spouse who is under age 65 and not yet eligible for Medicare. Will my spouse s premium for the Dominion medical plan still be automatically deducted from my pension check? Yes. Premiums for retirees or their dependents who are still covered under the Dominion retiree medical plan because they are not yet age 65 will continue to be deducted from the retiree s pension check. E 4: Will I be able to change insurance coverage as my needs change by selecting different carriers or plans? In other words, once enrolled with a plan selection in 2014, can I change my plan selection for 2015? Yes. Each year during the Medicare annual enrollment period you may change plans through Extend Health. However, during these annual enrollment periods your current medical conditions may limit the plans available to you. Extend Health will able to discuss these details with you. E 5: I am a Dominion retiree and my spouse is still actively employed by another employer. Can I be covered on my spouse s medical plan instead of buying a plan through Extend Health? If I don t enroll in a plan through Extend Health, can I come back and get an HRA when my spouse s employment ends? If you do not enroll in a plan through Extend Health when you are first eligible to do so, you will be considered to have declined coverage and will not receive an HRA contribution at any time in the future. A decision to decline coverage is considered permanent and irrevocable and you would not be eligible to enroll in a plan through Extend Health at any future date. This same rule applies to your spouse; he or she must also enroll in an Extend Health plan when first eligible to do so in order to receive an HRA contribution at any time in the future. E 6: Must I cover my post-65 spouse as a dependent under the same individual plan I choose? No. While you and your spouse may both enroll in the same medical and prescription drug plan, your coverage elections are not linked. Each person over the age of 65 must choose his or her own plan separately. E 7: I am not currently covering my spouse under Dominion s retiree medical plan. Can my spouse receive an HRA contribution from Dominion under the new program? No. 12/05/2013 6

7 E 8: What if I am retired and under age 65, but I am Medicare-eligible due to disability? You will remain eligible to be covered by Dominion s retiree medical plan, regardless of your disability. You will not be eligible to receive an HRA and elect coverage under the Extend Health program until you turn 65 and are enrolled in Medicare Parts A and B. E 9: What if my spouse or dependent is age under 65 but Medicare-eligible due to disability? Your spouse or child will remain eligible to be enrolled in Dominion s medical plan, regardless of the disability. He/she will not be eligible to elect coverage under the Extend Health program until he/she turns 65 and is enrolled in Medicare Parts A and B. No HRA contribution will be made for children age 65 or older. * E 10: What happens if I am retired and die before my spouse? Does coverage for my spouse continue? The spouse continues to receive Dominion s retiree medical benefits after the retiree s death. Accordingly, the spouse receives coverage under the traditional retiree medical plan until the spouse reaches age 65, and at age 65, the spouse receives the HRA. Keep in mind that the retiree and the spouse each receive separate annual stipend contributions. This means that although the retiree s annual stipend would not continue to be contributed to the HRA after the retiree s death, the spouse s annual stipend would continue to be contributed to the HRA each year. Also, any remaining balance in the HRA at your death remains in the HRA and can be used by your spouse. Health Reimbursement Account HRA 1: Will Dominion continue to subsidize the cost of coverage under this new arrangement? Yes. Dominion will create an individual Health Reimbursement Account (HRA) for retirees. The HRA will be funded annually by Dominion for individuals who enroll in coverage through Extend Health. The amount of the annual funding provided by Dominion is generally comparable to the level of Dominion subsidy that the retiree would receive today under the current plan. Dominion will also make an additional annual contribution into the retiree s HRA for a retiree s eligible spouse provided he/she also enrolls in coverage through Extend Health. Retirees and spouses will be reimbursed for eligible medical expenses from the HRA by submitting receipts to Extend Health. HRA 2: When will Dominion make its contribution to the HRA? Dominion will make its contribution to each eligible participant s HRA on January 1 each year. However, for 2014, the contribution will be made on April 1. HRA 3: How is Dominion s contribution to my HRA determined? The contribution that Dominion makes for you and your spouse is a function of several variables including when you retired, your age at retirement and years of service. The amount of Dominion s contribution is generally comparable to the level of Dominion subsidy that the retiree would receive today under the current plan. Because coverage on the Extend Health exchange begins on April 1, 2014, the Dominion stipend for 2014 will be prorated to equal 9/12 of the annual stipend amount. HRA 4: How can I determine my 2014 HRA funding amount? To learn your individual HRA funding amount, retirees and spouses who will be at least age 65 by April 1, 2014 may contact Extend Health at (starting December 10) or visit the Extend Health website anytime at If you or your spouse will reach age 65 after April 1, 2014, then you will receive further information about your stipend closer to the time you or your spouse reaches age 65. HRA 5: Will Dominion s contribution to my HRA increase in subsequent years to cover the cost of inflation? The contribution will be adjusted annually to reflect the change in the medical consumer price index. HRA 6: You mention that Dominion will make an additional annual contribution for my spouse. What about my children? Generally, children who are age 65 or older would not be eligible for the Dominion retiree medical plan, except in rare cases of continuous disability. Therefore, children are not eligible for a contribution to your HRA. HRA 7: Can one person use both the retiree s and the spouse s HRA contribution or is each person limited to the amount of the contribution made on that person s behalf? 12/05/2013 7

8 The HRA funds for the retiree and covered dependents are combined into a single account with no limit on the amount available for any one person. The retiree and spouse may have different insurance needs and health care costs. The combined HRA provides flexibility to use the funds for either spouse s eligible health care expenses. HRA 8: What expenses are eligible for reimbursement from the HRA? The premiums that retirees pay for medical (including Medicare Part B), dental, vision, and prescription drug coverage will be eligible for reimbursement. Also, most out-of-pocket medical expenses will be eligible as well. However, prescription drug out-of-pocket expenses are not eligible for reimbursement under current law. To provide financial protection against the potentially high cost of expensive brand name drugs some retirees may require, Dominion will reimburse 100% of the cost of drugs after retirees have spent a certain amount on prescription drugs each year. That amount is governed by Medicare s donut hole rules, and it changes annually. In addition, that amount varies from person-to-person, depending on the mix of generic and brand drugs taken. We estimate that a low percentage of retirees would have drug expenses high enough to reach this threshold in a given year. For those few that may reach this threshold in 2014, we expect that the threshold would be about $3,000 on average, though by law it can be no more than $4,550. HRA 9: What kinds of expenses are not eligible for HRA reimbursement? The following types of expenses are not eligible for reimbursement through your HRA: - Expenses for individuals other than the HRA participants, - Prescription drug out-of-pocket expenses (note that Dominion will reimburse your prescription drug expenses beyond the donut hole, as described in question HRA 8), - TRICARE (military) premiums, - Employer group premiums (if you are covered by another employer plan), and - Long Term Care insurance premiums or custodial care out-of-pocket expenses (such as residential care charges at a long term care facility). HRA 10: Do I have to buy a medical plan through Extend Health in order to get the HRA? Yes. If you want to receive an HRA with annual funding from Dominion, you must purchase a medical plan through Extend Health when you are first eligible to do so. This same rule applies to your spouse; he or she must also enroll in an Extend Health plan when first eligible to do so in order to receive an HRA contribution at any time in the future. HRA 11: What if I have TRICARE (military) medical coverage, do I still have to buy a plan through Extend Health in order to get the HRA? Yes. HRA 12: If the retiree or spouse dies, what happens to the HRA account? In the event of a death, the couple s joint HRA balance may be used by the surviving retiree or the spouse. Beginning the next year, Dominion will deposit only the amount relating to the surviving individual into the account on an annual basis. * HRA 13: Will Dominion s contributions to my HRA (the stipend) be taxable to me? Will I be taxed when I take the money out the HRA? No. you will not be taxed when Dominion puts money into your HRA. You also will not be taxed when you take money out of your HRA for qualified medical expenses. Billing B 1: How much is my new individual coverage going to cost? That depends on the plan you choose through the Extend Health exchange. During the enrollment period which begins on January 2, an Extend Health benefit advisor will help you assess your options and provide you the cost of each option you are considering. If you would like to consider coverage from another carrier not offered by Extend Health, you are responsible for contacting that carrier for plan and rate information. Remember, if you do purchase a plan without using Extend Health s exchange, you will not be eligible for the HRA contribution. B 2: There is currently a monthly deduction from my Dominion pension check for medical insurance. Will this deduction be dropped when the new Extend Health plan begins? 12/05/2013 8

9 Yes. Dominion will no longer withhold contributions for medical coverage for individuals age 65 or older beginning April 1, Retirees and spouses who are age 65 will pay their premiums directly to the insurance company and should use the Dominion HRA account to reimburse premiums and other qualifying out-of-pocket health care costs. You will only have a pension deduction for medical insurance if you or one of your dependents is not age 65 and therefore is still covered on the Dominion group plan. B 3: I am enrolling in a plan offered through Extend Health. When should I expect my first bill? Once your enrollment is received and processed, you will receive your billing materials from the carrier you have selected within 2 weeks of acceptance of your application.
