Complete all sections in CAPITAL LETTERS, using a black pen. If you make an err while completing this fm, do not use crection fluid. Cross out your mistakes and initial your changes. Please double check that you have done the following: entered your invest number and invest name as they appear on your latest periodic transaction statement selected which Fund(s) this instruction applies to entered the amount you wish to withdraw in dollars units if you are making a partial withdrawal provided your financial institution account details, if not previously provided signed the fm as per the Signing instructions in Section Send your redemption request to us. Withdrawals by fax In der to protect our invests from fraud, we will not allow payments to be made to third parties requests to deposit to bank accounts which have not been nominated by you in previous mail. Requests by fax f cheques to be mailed to third party addresses will also not be accepted. Transactions by fax will be accepted only when you have elected to use the fax facility option and agreed to be bound by the terms and conditions of its use. Terms and conditions of use of the fax facility By electing to use the fax facility, the invest accepts that we are not responsible to the invest f any fraudulently completed requests, and that we will not compensate an invest f any losses arising directly indirectly from using the facility. Invests acknowledge that should such fraud take place, the invest releases and indemnifies us against any liabilities whatsoever arising as a result of us acting on any communication received by fax in respect of the investment. These terms and conditions are in addition to any other requirements that may fm part of the invest giving instructions to us f whatever purpose, ie any instructions that relate to the completion of a particular authity. Joint Holding where the investment is in me than one name, all invests must sign, unless previously nominated otherwise.
