3 Serious Injury and Fatality Reduction Initiative Develop enterprise-wide strategies and metrics for the prevention of fatalities and serious injuries to support business unit level efforts to develop specific solutions to eliminate exposure to those hazards.
4 SIF Reduction Initiative The initiative will focus on: Educating leadership on the new paradigm for serious injuries and fatality prevention. Institutionalizing SIF metrics. Analyzing data for the identification of precursor situations and activities likely to result in serious injuries/ fatalities. Implementing controls to mitigate these precursors. Incorporating SIF initiatives into business unit level Safety Improvement Plans.
5 Institutionalizing SIF Metrics/ Educate Leadership Develop SIF Definition Mercer/SEE group October 2012 injuries and near miss events for both contractors and employees have been categorized with these criteria and documented in the H&S database. Reported to CEO and executive leadership weekly. Tracking injuries and near miss events that have the potential to be SIF. Reporting to senior management on a weekly basis. Monthly H&S Performance Reports includes the number of SIF incidents per business unit. SIF metrics are communicated on a regular basis to the H&S Performance Council and the Executive Leadership Team.

8 Business Unit Plans Data for individual business units will be analyzed so situations and related activities that are precursors to SIFs can be identified. H&S organization will work with business unit personnel to: Develop an inventory of situations and related activities that are precursors to SIFs; Assess the risks of these situations and activities; Assess the effectiveness of current controls; Prioritize the situations and activities Identify organizational weaknesses. Select key number situations/ activities to focus on. Define additional controls and interventions to reduce the risk of the situations /activities identified. Plans for two pilots in More in 2014.
9 Definition of SIF Serious Injury/Fatality (SIF): Work-related injury that results in one of these outcomes: Fatality Amputation (involving bone) Concussions and/or cerebral hemorrhages Injury to internal organs Bone fractures excluding fingers and toes Tendon and ligament tears Herniated disk (neck or back) Lacerations resulting in severed tendons, a deep wound requiring internal stitches 2nd degree burn (>10% body surface) or 3rd degree burns Eye injuries resulting in eye damage or loss of vision Injections of foreign materials e.g. hydraulic fluid Severe Heat Exhaustion and all heat stroke cases. Dislocation of a major joint (hip, shoulder, elbow, etc.) The following are excluded: non-preventable vehicle accidents; medical conditions such as heart attacks and seizures; injuries from normal body movements; hairline fractures; chipped teeth. 9
10 2013 Serious Injuries/ Fatalities Employee Serious Injuries & Employee/Contractor Fatalities Through April 15 th. Employee dislocated his shoulder when he slipped while descending a fixed ladder. PGO / Gibson IN. Employee received internal injuries when he was ejected from bucket truck and fell 30 feet to the ground while raising conductors for movement of a large structure. CDO / Hartsville SC. Employee carrying an extension ladder slipped on wet grass, fracturing his ankle. CDO / Lewisville NC. Employee died in automobile accident when her car ran off the road. Environmental Services / Brazil. Employee received internal injuries when he was struck in the abdomen when the apparatus being used to extract a bushing from a valve failed. PGO / Crystal River Fossil FL. Employee ruptured his bicep tendon while carrying plywood. Midwest Delivery Operations / Florence KY. Employee was climbing a wood pole when his gaff cut out and he grabbed the pole, tearing a tendon in his elbow. CDO / Raleigh NC. Contractor died from electrical contact after boring into underground conductor with post hole digger. DEI/ Peru. Contractor died when he was struck during tree felling operation. Transmission / Morganton NC. 10
11 SIF Potential Data SIF Potential analysis provided an additional 99 near misses and 25 injuries to review. Most frequent cause of injuries with SIF Potential is motor vehicle accident. Most frequent cause of near miss events with SIF Potential is dropped object. Average reported each month: 6 injuries/ month with SIF Potential. (Dec to March) 16 near miss events/ month with SIF Potential. (Oct to March) % of Total 16% of recordable injuries had SIF Potential 21% of total near misses had SIF Potential Not all near misses are reported. 11
