2 HOW TO CHOOSE YOUR CHAIRMAT SALES POLICIES & PROCEDURES Order Placement You may place an order by fax, or U.S Postal Service, however we must have a correct written copy of your order before we can process products for shipment. For Faster Service: FAX your order to Mail: customerservice@deflecto.com Deflecto, LLC 7035 E 86th Street Indianapolis, IN Damaged Goods Inspect order upon receipt and have delivery carrier acknowledge damage or shortage, making notation on the freight receipt. This will allow Deflecto to assist you in processing a claim. Returned Goods Policy Returns are accepted only with prior authorization from Deflecto. Return authorizations are valid for 10 days and are subject to a 25% restocking charge. Chairmats must be returned in original packaging and in usable condition. Return freight prepaid. Warranty Replacement Policy To obtain a replacement chairmat, fax the following information to Deflecto at : 1) Proof of Purchase 2) Contact Name and Telephone Number 3) Ship to Address You may obtain a warranty claim form by visiting our website at TERMS AND CONDITIONS Terms: Net 30 Days Freight: $500 Net order for prepaid freight to states east of Montana, Wyoming, Colorado and New Mexico. $750 Net order for prepaid freight to Montana, Wyoming, Colorado, New Mexico and all states West. Prepaid to one location only. Prepaid freight dock-to-dock excludes accessorial charges. Excludes Hawaii and Alaska. Lead Time: Orders ship within 10 business days or less. Deflecto reserves the right to roll chairmats larger than 46" x 60" on freight orders. Orders can not be changed after 24 hours from receipt of original order. Warranty EconoMat Studded Chairmats - Limited One Year All Other Deflecto Chairmats - Limited Lifetime Deflecto LLC, warrants its vinyl, polycarbonate and PET chairmats to be free from defects in workmanship and materials at the time of shipment to the original purchaser. Should a Deflecto chairmat break or crack under normal usage and proper application, we will replace the chairmat upon proof of purchase. Duration: Economat studded chairmats are covered for 1 year from the date of purchase. All other Deflecto chairmats are covered for the lifetime of the original purchaser. Exclusions: Warranty does not include shipping charges to and from factory. The use of metal chair casters or casters with surface contact of less than ½ in width or use on painted or varnished surfaces voids the warranty. Liability under this warranty is limited to the cost of the product and shall not extend to any other special or consequential damages.
