Brief Description of the Type of Investigations Cases Reviewed There were five closed investigations reviewed for the 4 th Quarter CPI Review in Circuit 2. There were no removal cases reviewed. Two of the cases were closed with no indicators of physical injury. One was closed with no indicators of family violence threatens child and one with no indicators of substance misuse. One case was closed with verified findings of medical neglect. These maltreatments were listed as the most serious maltreatments in the cases. Services were offered in four of the cases. One case had no prior reports. The other four cases had five to eleven priors. Cases were open from 21 days to 47 days. B. Overall Performance in Achieving Safety Each standard is assigned a score of 9-, 7-Mostly, 5- Partially, 0-Not, or NA-Not Applicable. Scoring is based on the percentage of standards that are, Mostly, Partially, or Not. Not Applicable ratings are not considered in determining the overall level of compliance. Circuit 2 received a score of 90% for the 28 standards related to the Investigative Response Domain. There were no removal cases selected from the random sample from the fourth quarter. Circuit 2 received an overall score of 90% for the fourth quarter. *Please note the scoring methodology has changed since the first and second quarter reviews. The total score reflected for each standard as well as the 2

Conducting thorough assessments The following standards consider whether assessments were completed throughout the investigative process to include the development of realistic safety plans when needed. Opportunities for Improvement were noted in the following standard relating to Assessment: The CPI presented the case to CLS for a staffing when warranted and when the investigation was legally sufficient, a petition was filed or a valid reason for not filing a petition was documented. 50% of two applicable cases. The one case not achieving this standard involved a young mother with a history of domestic violence. The father also had an arrest for domestic violence. D. Observing and Interviewing Children, Parents, Others The following standards consider whether thorough interviews with children and other participants were conducted and substantive observations were made of behaviors and interactions between the children and family members. An overall score of 93% was received for Observations and Interviews. REVIEW STANDARD 2. Interviews which addressed all maltreatments with parents, other caregivers, alleged perpetrators (if other than the parents) and other adult household members were conducted. 100% of five applicable cases. Opportunities for improvement were noted in the following standard relating to Service Planning and Provision: An interview was conducted and addressed all maltreatments with the alleged child victim(s) and other child(ren) named in the report and/or residing in the home. 78% of three applicable cases. This standard considers scores from two sub-part standards to determine the overall score. Standard 4.1 considers interviews with child victims, which scored 100%. Standard 4.2 considers interviews with other children on the report or residing in the home, which scored 52%. One report involved a three year old child in the home and no interview or attempt to interview was documented for this child. The other two reports had general interviews documented with some of the other children but specific maltreatments were not addressed. E. Determining Maltreatment Findings, Family Needs and Services The following standards consider whether appropriate maltreatment findings were reached and needed services were provided to children and families to promote positive outcomes and improve child-well-being. An overall score of 87% was received for these applicable standards. REVIEW STANDARD 13. CPI worked with CPT to determine findings and service needs. 18. Prior to removal, CPI made efforts to provide services to allow the child to remain in the home Areas of excellence were noted in the following standards relating to Service Planning and Provision: The CPI worked in partnership with the Child Protection Team (CPT) to identify child maltreatment, current and long term concerns and child and family service needs. 100% of one applicable case. All maltreatment findings were supported by the information gathered and appropriately documented in the investigative record. 100% of all applicable cases. If at any point during the investigation placement of the child outside of the home was a possibility, the CPI requested an Early Services Intervention (ESI) Staffing to determine if the Community Based Care (CBC) should provide family preservation services that would allow the child to remain safely in the home. 100% of one applicable case. Based on the child/family needs, the immediate services and/or ongoing supervision needs were identified for the child, mother, father, other caregiver and/or caretaker responsible, if other than the mother or father. 91% of five applicable cases. If immediate services or ongoing supervision was needed, referrals for these services were completed for the child, mother, father and other caregiver or caretaker responsible (if other than the mother or father). 100% of four applicable cases. Opportunities for improvement were noted in the following standard relating to Service Planning and Provision: If documentation reflects the need for immediate services and/or ongoing supervision, the investigation record contained evidence the services were engaged. 75% of four applicable cases. The one case that did not achieve this standard documented a daycare referral. There was no follow-up to determine if the mother had utilized the daycare referral. 6

Planning for Safe Investigation Case Closure The following standards consider whether information gathered during investigations was appropriately shared between and among all parties including supervisors, case managers, substitute caregivers, and service providers, and acted upon as necessary. Two of the reports had additional maltreatments that were discovered during the investigative process. There was no guidance and direction to address the need to add the maltreatments to the report. The other case not fully achieving the standard lacked guidance to address the need for documentation of behavioral observations and interactions with the caregivers. When the investigation was being closed, the case file documents the CPI or CPI Supervisor ensured the receiving case management agency was notified of the closure, and the transfer of responsibilities from CPI to case management was clearly communicated. 0% of one applicable case. One case was referred to Community Based Care for on-going case management services. Documentation did not support the case manager or supervisor was notified when the case was closed or the findings. 2. Requests for Action A Request for Action (RFA), either administrative or child safety, is generated when there is an unresolved concern related to child safety, permanency, or wellbeing. There was one RFA generated during the fourth quarter file reviews. The CPI obtained a copy of the Domestic Violence Injunction regarding incidents that occurred on March 20, 2012 and March 28, The petition states the child hit her head when she fell off the couch during a domestic violence incident. The father was arrested for violation of the injunction. Family violence threatens child was not added to the report and service needs were not assessed to address the domestic violence. A new report was generated regarding the domestic violence maltreatments. The RFA was resolved with the generation of the new report. 3. Recommendations Circuit 2 is encouraged to focus on the areas identified as opportunities for improvement. 8
