3 Policy Coverage & Benefits Insured Person Any student and Postgraduate / Research studentship holder of CUHK and All students of School of Continuing & Professional Studies whilst travelling on any of the University s approved assignments, duties, study tours and all other academic activities outside Hong Kong Age Limit 16 to 75

5 Policy Coverage & Benefits (Cont d) The following benefits are applicable to all students Benefits Accidental Death / Permanent Disablement Limits (per person) : HK$1,000,000 Medical Expenses : HK$390,000 - including follow-up treatment after returning to home country within 90 days. Follow-up treatment will be covered up to 100% of unused portion of sum insured. Follow-up treatment including expenses payable to Registered or Listed Chinese bonesetter, Acupuncturist, Chinese Medical Practitioner upto HK$150 per visit per day, and maximum HK$3,000 in aggregate. Burns Benefit (2 nd & 3 rd degree) : HK$100,000

11 Major Policy Exclusions War, direct participation in strike, riot and civil commotion Suicide, attempted suicide or intentional self-infliction Professional Sports Under the influence of alcohol or drugs Not taking reasonable precaution to safeguard his/per property or to avoid bodily injury AIDS or AIDS related complex, HIV or related diseases Manual labour or hazardous works

13 Extended Coverage Extended Period of Insurance For insured trip duration exceeding 270 days For personal deviation / trip exceeding 3 days before and 3 days after the insured trip All benefits, limits, terms and conditions as per existing Group Travel Insurance policy. Eligibility Period / Insured Additional premium : Subject to prior declaration : As outlined on CUHK s intranet

14 Claims Procedures Record the time, date and place of loss or incident. For personal accident or sickness, obtain (1) medical expenses receipt(s) with diagnosis; (2) medical record / discharge summary issued by the clinic / hospital. For loss of property, report to the local police or the relevant party (e.g. hotel, airline, public common carrier, etc.) within 24 hours and obtain a written proof Keep all the travel documents (e.g. itinerary, boarding pass, etc.) Complete and submit Claim Form with the above supporting documents and approval letter from CUHK All claims must be reported within 30 days after the accident / trip
