COST SAVINGS Not only are you saving money, but you are locking in the price of the merchandise upon enrollment. Whether that unfortunate day comes this week, next year, or thirty years from now, the price of the merchandise upon your enrollment is LOCKED in for the life of your membership. YOUR INSURANCE POLICY CAN PAY FOR IT You can assign a portion of your life insurance policy to pay for your plan. It is a revocable assignment and the remaining balance will be left for your beneficiaries. An example of this is if you have a $10,000 policy and you enroll in the Bronze Legacy ($1,500). At the point of death, Legacy Assurance would receive $1,500 (revocable assignment) and the beneficiary(s) would have the remaining $8,500 to pay for other funeral, medical, and miscellaneous costs.

Legacy Assurance offers fiberglass burial vaults that are lightweight and easy to install. These vaults have superior strength and are able to withstand 20,000 pounds of top-loaded pressure. They come with a 100 year warranty. Their purpose is to protect the casket and its contents and to keep burial sites from sinking. These vaults are water-resistant and are excellent for sandy or wet soil conditions.

This free service is included in the Bronze, Silver, and Gold Legacy plans at no additional cost. B RONZE LEGACY The Bronze Legacy Plan is our standard-level plan. By selecting the Bronze Legacy, you have the ability to choose from A) one of our many high-quality caskets or B) an urn and customizable monument of your choice. 14 GENERAL PRICE LIST The prices in effect at the time Membership is confirmed are effective for the lifetime of that Membership. These prices are effective as of May 1, The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. Concierge Service: Free Bronze Legacy Plan A (Casket with Concierge Service): $1,500 Bronze Legacy Plan B (Urn and Monument with Concierge Service): $1,500 Silver Legacy Plan (Casket and Vault with Concierge Service): $2,250 Gold Legacy Plan (Casket, Vault, and Monument with Concierge Service): $3, Gauge Steel Casket: $1500 Child-size Hardwood (White Gloss Finish): $1500 Any Legacy Assurance Urn: $250 Monument: $1,250 Fiberglass Burial Vault: $750 *In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements. AREAS OF OPERATION Legacy Assurance, LLC ( Legacy Assurance ) does not offer services or merchandise outside of the United States, and as of May , Legacy Assurance only offers services and merchandise in the Areas of Operation as detailed below: Legacy Assurance is growing into new markets and states every week. For an updated list of licensed and approved states, please visit our website at MEMBERSHIP DETAILS The Federal Trade Commission ( FTC ) requires that funeral homes accept any casket purchased from a third party or outside source, such as Legacy Assurance, without charging additional fees for doing so. Furthermore, FTC regulations require funeral homes to provide an itemized list of costs to be charged for goods and services, which helps prevent hidden charges or handling fees from being added to caskets or vaults purchased from Legacy Assurance. You may modify your plan, free of cost, at any time prior to the finalization of the purchase and the initiation of shipping procedures. Legacy Assurance Membership may be cancelled upon written notice at any time by either party. SHIPPING All merchandise prices include standard shipping. Using standard shipping, caskets and vaults will arrive at the ship-to address within 72 hours after the purchase is finalized and confirmed. Expedited shipping is available upon request, but will incur additional charges based on the location of the ship-to address and the expedited shipping services available. Severe weather conditions or other states of emergency can delay any delivery beyond the normal stated parameters. Caskets and Vaults will be delivered during normal business hours. If merchandise is delivered to a funeral home, you do not have to be present at the time of delivery. If merchandise is delivered to a location other than a funeral home, the carrier and the recipient must arrange for a delivery appointment, and someone must be present to accept the delivery at the appointed time. Should you choose not to use Legacy Assurance s concierge services, you are responsible for confirming the funeral home s shipping address with the funeral home director prior to shipment. All merchandise will be shipped FOB Thomasville, Georgia. Legacy Assurance s liability as to delivery ceases, and the title and risk or loss for the merchandise will pass on Legacy Assurance s delivery of the merchandise to a transportation carrier at the designated shipping point. RETURN INFORMATION Due to FTC regulations and some state laws and regulations, Legacy Assurance will only accept the return of a casket, vault, or other such merchandise if the product(s) arrives with damage caused by shipping. If merchandise is freight-damaged upon delivery, Legacy Assurance will not be obligated to issue any refund or replace any merchandise unless the following conditions are met: THERE IS NO SCIENTIFIC OR OTHER EVIDENCE THAT ANY CASKET WITH A SEALING DEVICE WILL PRESERVE HUMAN REMAINS. FOR CALIFORNIA RESIDENTS: THE SELLER IS NOT A FUNERAL DIRECTOR AND IS NOT LICENSED BY THE DEPARTMENT OF CONSUMER AFFAIRS, AND MAY NOT OFFER OR PERFORM FUNERAL SERVICES. STATE AND FEDERAL LAWS PROHIBIT A FUNERAL DIRECTOR FROM CHARGING HANDLING FEES FOR A CASKET SUPPLIED BY ANOTHER PARTY. THE MONEYS RECEIVED BY THE SELLER FOR THE PURCHASE OF A CASKET ARE NOT SUBJECT TO STATE LAW GOVERNING MONEY HELD IN TRUST. THE SELLER IS NOT BOUND BY STATE LAWS OR REGULATIONS THAT GOVERN FUNERAL HOMES AND CEMETERIES. THE PURCHASER ENTERS THIS AGREEMENT AT HIS OR HER OWN RISK.
