Dear Sir, Sub-File-Adv-1 Re : Providing Insurance Cover to all women SHGs that are credit linked to Banks The Hon ble Finance Minister, during his Budget Speech of had focused on progressive measures for improvement in the living standard of women. In our efforts towards this we have credit linked large number of women SHGs. To further improve their living conditions, providing Life Insurance cover to members of women SHGs & also to meet their children s educational expenses at reasonable cost,it is decided to implement the Janashree Bima Yojana of the L.I.C. of India for the women SHG members in our Bank. In this connection, LIC of India has informed us that the coverage under Janashree Bima Yojana which is a Social Security Group Insurance Scheme, will provide women with life cover for self & also give free scholarship to their children as an add on benefit. We have vide our letter No. BCC:R&AB:100/1056 dated have given advance information about this scheme to all Zonal/Regional Heads. The important features of the scheme are as below : 1) The benefits under the scheme : Natural Death : Rs.30000/ Accident Benefit o On death due to accident : Rs.75000/-. o Permanent total disability due to accident : Rs.75000/-. o Loss of 2 eyes or 2 limbs or one eye & one limb in an accident Rs.75000/-. o Loss of one eye or one limb in an accident Rs.37500/-. o Scholarship of Rs.600 per half year per child for max period of four years. (Two children per member (family)studying between class IX to XII.). 2. Premium payable by the SHG member :Rs 100/- member/year. The detailed operational guidelines of the scheme along with the copies of the formats are enclosed. We advise you to ensure that large number of women SHG members which are credit linked by the branches are covered under the scheme. 2 1) Objective: JANASHREE BIMA YOJNA (For SHGs credit Linked to banks) The object of this scheme is to provide life insurance protection to the rural and urban poor persons living below poverty line and marginally above poverty line. 2) Eligibility: Members of the women SHGs credit linked by Bank and between 18 to 59 years of age. 3) Benefits : Sr. No. The benefits under the scheme are as below : a) Natural Death : Rs.30000/- b) Accidental Benefits : Type of Loss Amount of coverage (Rs.) 1 Life Permanent total disability due to accident Loss of 2 eyes or 2 limbs OR one eye and one limb in an accident Loss of one eye or one limb in an accident ) Premium: c) Scholarship to children of insured members : Scholarship of Rs.600/ per half year per child will be paid for maximum 2 children studying in 9 th to 12 th standards for maximum period of 4 years as an add on benefit. Rs.200/ per women SHG member per annum will be shared as under : Rs.100/to be paid by the member. Rs.100/ will be born out of Social security Fund of LIC of India & will be paid directly by the fund. 5) Procedure for coverage: a ) For Insurance: Our corporate office will be the nodal office for implementing the scheme in our Bank. All the branches will obtain the application (Form A-II) of each women member of the women SHGs credit linked by it along with the premium amount of Rs.100 per member. The amount of premium may be debited to the SHGs saving/loan account under their authority. The total premium amount is be remitted by I schedule drawn on nodal branch identified by the Region. Unique Number should be given to each member of SHG for easy identification of member by the bank & LIC. The Number would consist of : i) City code 3 digit } ii) Bank code 3 digit } MICR code of respective Branch iii) Branch code 3 digit } iv) SHG code 6 digit - Loan A/c No. (Last 6 digits only) v) SHG member code 3 digit - unique sr. no. to be given to each member of SHG at the branch.

3 : 2 : The hard copies of the applications will be retained by the branches for their record. Only soft copies in the excel format are to be submitted to the Regional Office (Annexure-I) along with the I schedule for the premium amount drawn on the branch identified by the Region. The Region will consolidate the branch wise data (soft form) and submit a single file along with the schedule for premium amount drawn on BANEAS. b) For Scholarship under Shiksha Sahyog Yojana The member having children eligible for scholarship will give details of the children as required in the application form (A-III), for coverage under Shiksha Shayog Yojana i.e. for scholarship scheme. All the data as above and the premium amount will be submitted by corporate office to the LIC who will issue a single policy in the name of Bank and will pass on the beneficiaries particulars to their respective P&GS unit office. The LIC has 69 P&GS units at different locations all over the country (list enclosed in annexure II) which will be mapped with our branches for smooth and easy servicing under the scheme. Thereafter the branches will only remit premiums for the existing members, deducting the members deleted and adding the amount for new members added during the year. The applications for new members only are required to be obtained & forwarded to us in soft copy as mentioned above. 6) Claim Procedure: a) For Insurance benefits: The nominee of the deceased member will submit the claim form (A-IV) along with the original death certificate to our branch. The branch will submit the same duly verified to the LIC s P&GS unit. (List enclosed). The LIC will settle the claim by sending A/C Payee Cheque directly to the beneficiary. In case of accidental claim police enquiry report will also be required to be submitted. b) For claiming scholarship: 1. The application (Proforma A-III) has to be submitted every year for claiming the scholarship mentioning that the child is promoted to next standard. The list of beneficiary students has to be submitted to the concerned P&GS Unit for disbursement of scholarship by the our concerned Regional Office, with full details such as name of the student, school name, class, members name, master policy number,membership number etc. 2. The scholarship will be disbursed to the beneficiary students through the Bank on half-yearly basis i.e. on 1 st July & 1 st Jan every year. The LIC will send the account payee cheque in the name of the Bank, along with list of beneficiary students, who will pass on the scholarship to eligible students on half yearly basis payable on 1st July and 1st January every year. The bank has to submit utilization certificate before claiming the next due of scholarship (Proforma A-V).
