Optional Riders Accelerated Death Benefit for Long Term Care (issue ages: 18-70) Extension of Benefits for Long Term Care (issue ages: 18-70) Total Disability Premium Waiver (Employee only; issue ages: 18-55) Total Disability Payor Waiver of Premium (Spouse only; issue ages: 18-55) Children s Term (issue ages: your age 18-65) The riders listed above have exclusions and limitations. Partial Withdrawals, Surrenders and Non-Qualified Additional Benefit Rider Charges and Loans May Be Subject To Taxes and Penalties Partial withdrawals, surrenders, non-qualified additional benefit rider charges and loans from life insurance policies may be subject to ordinary income taxes and possibly an additional 10% federal tax penalty. Outstanding loan balances and withdrawals generally reduce the death benefit and cash value. With proper planning, the death benefit can pass to your beneficiaries free from state or federal estate taxes. Please consult with your tax advisor for specific information. Life Benefit You may be required to answer health questions at enrollment. Coverage may be available with reduced underwriting for new members during your initial enrollment period. If you enroll after your initial enrollment period, answers to health questions are required. 8

9 Important Information About Coverage Group Critical Illness Issue ages are 18 and over, if Actively at Work. Benefits Specifications (see Benefit Amounts) Heart Attack Exclusion - A cardiac arrest is not a heart attack and is not covered by this benefit. Stroke Exclusions - Does not include: Transient ischemic attacks (TIAs), head injury, chronic cerebrovascular insufficiency and reversible ischemic neurological deficits. Coronary Artery Bypass Surgery Exclusions - Does not include: abdominal aortic bypass, balloon angioplasty, laser embolectomy, atherectomy, stent placement, or other non-surgical procedures. Invasive Cancer Exclusions - Does not include: carcinoma in situ, tumors related to HIV, non-invasive or metastasized skin cancer, or early prostate cancer. Includes: Leukemia and Lymphoma. Carcinoma in Situ Exclusions - Does not include: other skin malignancies, pre-malignant lesions (such as intraepithelial neoplasia), or benign tumors or polyps. Second Event Initial Critical Illness Benefit Conditions - There must be at least 12 months between each diagnosis. A covered person can receive a Second Event Benefit only once for each initial critical illness. Second Event Cancer Critical Illness Benefit Conditions - There must be at least 12 months between each diagnosis. Not payable if the covered person receives treatment during that 12-month period. Treatment does not include maintenance drug therapy or routine follow-up office visits. A covered person can receive the benefit only once for each cancer critical illness. Advanced Alzheimer s Disease Conditions - Must have impaired memory and judgment, and be unable to perform 3 or more daily activities.* Advanced Parkinson s Disease Conditions - Must have 2 or more physical signs and be unable to perform 3 or more daily activities.* Benign Brain Tumor Exclusions - Does not include: tumors of the skull, pituitary adenomas, or germinomas. Paralysis - Permanent loss of use of 2 or more limbs. *Daily activities are: bathing, dressing, toileting, continence, transferring and eating. Wellness Benefit - Pays a benefit each calendar year for one of the following: Bone Marrow Testing; Blood Tests for Lipid Panel (cholesterol), Triglycerides, CA15-3 (breast cancer), CA125 (ovarian cancer), CEA (colon cancer); Chest X-ray; Colonoscopy; Flexible Sigmoidoscopy; Hemoccult stool analysis; Mammography, including Breast Ultrasound; Pap Smear (ThinPrep Pap Test included); PSA (blood test for prostate cancer); Serum Protein Electrophoresis (test for myeloma); Biopsy for skin cancer; Stress test on bike or treadmill; Electrocardiogram (EKG); Carotid Doppler; Echocardiogram. Benefit Specifications Group Voluntary Critical Illness (GVCIP2) Conditions, Limitations and Exclusions Affecting Your Benefits Conditions and Limits - Benefits are not payable for any critical illness diagnosed prior to the effective date. Benefits are also subject to the Pre-Existing Condition Limitation, if applicable, as well as all other limitations and exclusions. All critical illnesses must meet the definitions and dates of diagnoses stated in the policy and be diagnosed by a physician while coverage is in effect. The date of diagnosis for each illness must be separated by 30 days. Emergency situations while you are outside the U.S. will be considered when you return to the U.S. Your Eligibility - Issue ages are 18 and over. Dependent Eligibility/Termination - (a) Coverage may include you, your spouse or domestic partner and children under age 26; (b) coverage for children ends when the child reaches age 26, unless he or she continues to meet the requirements of an eligible dependent; (c) spouse coverage ends upon divorce or your death. Domestic partner coverage ends upon the end of the domestic partnership or your death. When Coverage Ends - Coverage under the policy ends on the earliest of: the policy is canceled; you stop paying your premium; the last day of active employment; you are no longer eligible; a false claim is filed; when all critical illness benefits have been paid. Continuing Your Coverage - You may be able to continue coverage when coverage under the policy ends. Refer to your Certificate of Insurance for details. Recurrence of Cancer - Cancer critical illness benefits are payable for a diagnosis of a recurrence of cancer as long as you are diagnosed after the effective date of coverage and have been free of any symptoms and treatment of cancer for 12 consecutive months immediately preceding the effective date of coverage, or any 12 consecutive months thereafter. Policy Exclusions and Limitations - We do not pay benefits for: (a) war, participation in a riot, insurrection or rebellion; (b) intentionally self-inflicted injury or action; (c) illegal activities or occupations; (d) suicide while sane, or self-destruction while insane, or any attempt at either; ((e) substance abuse, including alcohol, alcoholism, drug addiction, or dependence upon any controlled substance. 9

10 Benefit Specifications (continued) Group Voluntary Disability Insurance (GVDI) Conditions, Limitations and Exclusions Affecting Your Benefits Benefits Specifications Total Disability - Monthly benefit starts after the elimination period has been met. Benefits will not continue beyond the maximum benefit period. Partial Disability - Pays 50% of the monthly benefit after at least one month of total disability. Payments continue while partially disabled for up to 3 months, but not beyond the maximum benefit period. Pregnancy - Pays a benefit for a pregnancy if total disability first begins after the certificate has been in force for at least 9 months. Organ Donor - Pays a benefit when disabled from donating an organ. Waiver of Premium - Pays the premium after monthly disability benefits are payable for 30 days in a row, for as long as monthly benefits are payable. Concurrent Disability - Being disabled from more than one cause does not extend the payment of benefits under the maximum benefit period. Recurrent Disability - Pays a benefit when disabled from the same or related cause within 6 months without a new waiting period or maximum benefit period. When Coverage Ends - Coverage ends when the policy is canceled; premium payments stop; the last day of active employment, unless coverage is continued through Temporary Layoff, Leave of Absence or Family and Medical Leave of Absence; you or your class is no longer eligible; or fraud or material misrepresentation is discovered. Pre-Existing Condition Limitation We do not pay benefits for disability that starts within 12 months of your effective date from a pre-existing condition. You have a pre-existing condition if: (a) Your disability began during the 12 months after the effective date; and (b) you received medical treatment, consultation, care or services, diagnostic measures, took medications or followed treatment recommendations in the 12 months prior to the effective date of coverage, or the date an increase in benefits was effective; or (c) symptoms existed in the 12 months prior to the effective date or the date an increase in benefits was effective. Certificate and Rider Exclusions and Limitations (a) Benefits are not paid for: (1) war, participation in a riot, insurrection or rebellion; (2) illegal activities or participation in an illegal occupation; (3) intentionally self-inflicted injury or action; (4) (if applicable to your coverage) substance abuse, to include abuse of alcohol, alcoholism, drug addiction or dependence upon any controlled substance; (5) participation in aeronautics except as a fare-paying passenger in a licensed common carrier aircraft; (6) voluntarily inhaling fumes or gases; (7) cosmetic surgery (complications are covered); (8) pre-existing conditions during the first 12 months of coverage; (9) occupational sickness or injury, unless covered by an on-the-job disability rider. (b) We do not pay for disability during incarceration. Definitions Total Disability - When, because of sickness or injury, you are: (1) unable to perform the material and substantial duties of your own occupation; and (2) under the regular care of a doctor; and (3) unable to engage in any occupation. Partial Disability - When, because of sickness or an injury, you are: (1) unable to perform the material and substantial duties of your own occupation on a full-time basis, but are able to work part-time; and (2) under the regular care of a doctor. Elimination (Waiting) Period - A period of continuous total disability which must be satisfied before you are eligible to receive benefits. Own Occupation - The occupation you are performing when a period of disability begins. Group Universal Life (GUL) Conditions Affecting Your Benefits It is possible that coverage will expire when either no premiums are paid following the initial premium or subsequent premiums are insufficient to continue coverage. 10
