2 Section 2 Health, Safety and Environment Training 1. NVQ/SVQ Exemption (applicable for blue/gold cards) If you have obtained a relevant Level 2 or Level 3 National Vocational Qualification (NVQ/SVQ) or NVQ Diploma within the past two years, tick this box and include a copy of your completion certificate with your application. Please Note: C&G 6129 or 6035 VRQ certificates are NOT acceptable. The exemption period is for a maximum of 2 years from the date on the completion certificate. 2. CITB Health, Safety and Environment (HS&E) Test If you have passed the CITB HS&E test within the past 2 years please tick this box and ensure you send a copy of your test pass letter with this application. 3. CITB Supervisors + CITB Managers and Professionals HS&E Test (applicable for Supervisors/Managers cards) If you have passed either the CITB Supervisors or the CITB Managers and Professionals HS&E Test within the last 2 years please tick this box and ensure you send a copy of your test pass letter with this application. 4. Other applicable Health and Safety Tests If you have passed any other acceptable Health and Safety tests/courses within the last 2 years please ensure you send a copy of your test/course certificate with this application. 7 Section 16 Guidance Notes PHMES Supervisor UK-PHMES CSCS Registration Card (Gold) i. A Gold JIB-PMES CSCS Card is issued to Plumbing supervisors who hold suitable NVQ/SVQ Level 4 qualifications OR have equivalent site experience and who can provide endorsements from their employer/clients to support their application. ii. This card is also available to supervisors who have completed the Construction Awards Alliance (CAA) or CSkills Awards L3 NVQ in Occupational Work Supervision (Construction) - providing that they can also demonstrate a minimum of five years industry experience in a supervisory role. iii Registration Cards issued on this basis are valid for up to five years, and may be renewed after that period (subject to renewal rules). iv. This route of entry is permanently open. v. To obtain a Plumbing Supervisor UK-PHMES CSCS Registration Card applicants must provide the following: A correctly completed application form and the appropriate fee. Evidence of passing the CITB Supervisors Heath, Safety and Environment test Evidence of an acceptable NVQ/SVQ Level 4 qualification (visit our website for details) OR Equivalent site experience - two endorsements must be provided on the application form, a card issued without the above qualification will be on an INDUSTRY ACCREDITATION basis.
