1 Open Enrollment Guide for Employees of Sacramento County This guide is designed as a tool to help you navigate through the upcoming Open Enrollment period successfully. It provides an overview of Open Enrollment, changes in the benefits offered, a calendar of events, plan costs, important things to remember and benefits staff contact information.
2 OPEN ENROLLMENT OVERVIEW What Is Open Enrollment? It is the one time each year employees can make changes to benefits without a qualifying event (marriage, birth of child, change in status, etc.). When is Open Enrollment? October 7 th -- November 8 th 2013 Are any benefits changing? YES! There are significant changes to the benefit options for 2014: Blue Shield is no longer being offered Health Net is no longer being offered Western Health Advantage (WHA) will be a new carrier offering HMO and High Deductible options Sutter Health Plus will be a new carrier offering HMO and High Deductible HMO options Employees who waive medical coverage or are enrolled in the Kaiser, WHA, or Sutter High Deductible plans can purchase optional vision coverage through VSP IMPORTANT if you are enrolled in Blue Shield or Health Net you will need to make an election and list your provider ID on your new enrollment to maximize the opportunity for continued access to your doctor. I have questions, where do I get answers? Changes in your health care can raise many questions. We have scheduled several sessions where you can talk with representatives from all health plans regarding your options. Attend a brief walk-in presentation followed by a Q&A session; the presentation will repeat at the top of each hour. 3 BENEFIT COST INFORMATION The chart below indicates the total monthly cost of medical coverage for each carrier for either single coverage or family coverage. The amounts listed are per pay period. Cashback allowances are shown in the 2014 MyBenefits Summary and are not reflected in this Guide.
4 HIGH DEDUCTIBLE PLANS AND HSA S Will a High Deductible plan with a Health Savings Account save me money? Below are examples of out of pocket costs for an HMO plan vs a High Deductible plan. HMO- you pay a premium each paycheck whether you use the plan or not, then you pay your copayment when you go to the doctor. High Deductible- you pay only when you use services; your maximum out of pocket cost is limited to the deductible, which is $3,000 for a family. Below are examples of differences in annual premium deduction costs for HMO vs HD plans for Tier B family coverage-- (assuming you have spent $3,000 in HD services). Kaiser HMO Kaiser HD WHA HMO WHA HD Sutter HMO Sutter HD Bi-Weekly Deduction $ $0 $ $0 $ $0 Annual Premium $3, $0 $3, $0 $3, $0 Difference $ Minimum Savings $ Minimum Savings $ Minimum Savings Since the most you can spend under the HD plan is $3,000 and services are covered at 100% after you meet that amount, the HMO premium alone is more than the maximum cost of services under the HD plan. And, if you do not use $3,000 in HD services, your savings is event greater! Health Savings Accounts-The idea is to contribute a bi-weekly amount to your HSA to cover estimated out of pocket expenses. You have complete flexibility to change the deduction amount anytime during the year. Your HSA account can also be used for dental and vision expenses, and there is no use it or lose it rule. More information is available in the MyBenefits Summary, or talk with a representative at an Open Enrollment event. VISION PLAN BUNDLES Do I have vision coverage? The medical plan you enroll in determines if vision coverage is included or if you can purchase it voluntarily. Vision premiums are $5.00/Single, and $12.80/Family. The new VSP benefit is 12/12/24 and an increased allowance to $130 for Medical Plan Vision HSA Eligible? Kaiser $15 HMO Kaiser Vision-Included Not eligible-can enroll in FSA-Medical WHA $15 HMO VSP-Included Not Eligible-Can enroll in FSA-Medical Sutter $15 HMO VSP-Included Not Eligible-Can enroll in FSA-Medical Kaiser High Deductible HMO Option to purchase VSP Yes-Wells Fargo WHA High Deductible HMO Option to purchase VSP Yes-Health Equity Sutter High Deductible HMO Option to purchase VSP Yes-US Bank WHA HD PPO (out-of-area) Option to purchase VSP Yes-Health Equity WHA PPO (out-of-area) VSP-Included Not Eligible-Can enroll in FSA-Medical Waiver Option to purchase VSP Not Eligible-Can enroll in FSA-Medical
5 THINGS TO REMEMBER: BLUE SHIELD AND HEALTH NET ENROLLEES you should select a new plan and list your doctor s provider number on your enrollment to maximize continued access to your doctor! Open Enrollment begins at 8am October 7 th and ends at 5pm on November 8 th. Open Enrollment changes are made online at 1) Make changes then click submit on the Review tab 2) You must provide supporting documentation if adding dependents or waiving medical coverage (i.e., group coverage enrollment, marriage/birth certificates if adding dependents, etc.). If you are enrolled in Health Net or Blue Shield and you do not select a new provider you will be default enrolled into Sutter Health Plus effective January 1, You must re-enroll in Flexible Spending Accounts for Medical Reimbursement or Dependent Care Reimbursement Accounts. A new election is required each year to continue enrollment, even if you are not changing your annual election. New contracts are also required, if applicable. If you waive your medical plan or are enrolled in the Kaiser High Deductible plan, you can elect to enroll in VSP and have the cost deducted from your pay-$5.00 Single/$12.80 Family. Changes made during Open Enrollment are effective January 1 st of the following year. The first paycheck of 2014 will reflect any changes in benefit plan premiums. Attend an Open Enrollment Event. The Employee Benefits Office staff and representatives from the health plan carriers are available at these events to assist you with questions about the plan or coverage. We encourage you to visit our website at to find answers to frequently asked questions and to review more information regarding the benefits available to you as an employee of Sacramento County. Make your 2014 elections early for a successful Open Enrollment! 6 PERMITTED BENEFITS CHANGES EXAMPLES OF CHANGES PERMITTED DURING OPEN ENROLLMENT Change plans-health Net to WHA, waiver to Sutter, Blue Shield to waive, etc. (proof of group coverage is required to waive medical) Change to Tier B-This election is irrevocable once made MEDICAL DENTAL FLEXIBLE SPENDING ACCOUNTS (Re-enrollment is required each year) DEPENDENT LIFE INSURANCE Add dependents-add spouse/dp and/or children (dependents must meet dependent eligibility requirements, dependent documentation is required) Drop dependents-remove spouse and/or children from coverage (no documentation required) Add dependents-add spouse/dp and/or children (dependents must meet dependent eligibility requirements, dependent documentation is required) Drop dependents-remove spouse and/or children (no documentation required) Enroll/re-enroll in Dependent Care Reimbursement Account (new DCRA contract required, if applicable) Enroll/re-enroll in Medical Reimbursement Account (new Ortho contract required, if applicable) Employees of UPE (BG 005 & 008) can enroll dependents for life coverage (action cannot be performed online; paper enrollment required). Coverage for dependents in all other units is automatic. EXAMPLES OF CHANGES PERMITTED ANYTIME DURING THE YEAR These changes can be made without a qualifying event; they may also be made during Open Enrollment OPTIONAL LIFE INSURANCE Increase coverage, decrease coverage, waive all optional life coverage, make beneficiary updates HEALTH SAVINGS ACCOUNT Enroll/Change annual election (must be enrolled in High Deductible Plan) DEFERRED COMPENSATION Enroll, increase contribution, decrease contribution, change investments, change beneficiary CHANGES THAT ARE NEVER PERMITTED These benefits are automatically provided by the County to all benefit eligible employees EAP Employee cannot waive EAP benefits BASIC LIFE INSURANCE Employee cannot waive the basic life benefit DENTAL COVERAGE Employee cannot waive dental coverage for self
