1 NESTLÉ HOTTEST TICKET IN TOWN SWEEPSTAKES ( Sweepstakes ) OFFICIAL RULES NO PURCHASE OR PAYMENT NECESSARY TO ENTER OR TO WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING A PRIZE. Sponsor: Nestlé USA, Inc. ( Sponsor ). Promotion Period: The Sweepstakes will begin at 3:00 p.m. Central Daylight Time ( CDT ) on Wednesday, June 19, 2013, and ends at 3:00 p.m. CDT on June 21, 2013 ("Promotion Period"). The Sweepstakes is subject to all federal, state and local laws and regulations and is void where prohibited by law. 1. ELIGIBILITY. The Sweepstakes is open only to legal residents of Illinois, Indiana or Wisconsin, who are eighteen (18) years of age or older at the time of entry ( Participants ). Potential winner ( Potential Winner ) will be required to provide proof of age and address before they can be declared a grand prize winner. The Sweepstakes is not open to persons who are employees, suppliers or vendors of Nestlé USA its advertising or promotion agencies, their immediate family members and/or those living in the same household of each. 2. HOW TO ENTER. During the Promotion Period, Nestlé street ambassadors will be distributing Sweepstakes entry forms at designated locations throughout the downtown Chicago area. Complete all information on the entry form and return the completed entry form to the street ambassador. LIMIT: One (1) entry per person.

2 No mechanically reproduced entries permitted. Sponsor is not responsible for lost, late, mutilated, misdirected entries or entries not received in time for the random drawing. Incomplete/illegible entries are void. Participants MUST be available to be contacted by Sponsor or Sponsor s designee at 5:00 p.m. CDT on Friday, June 21, 2013, in order to be eligible to receive the prize. Entries become the property of Sponsor and will not be returned. 3. PRIZES. One (1) Grand Prize available. Grand Prize will consist of: Two (2) tickets to Game 5 of the Finals to be held on June 22, 2013 at the United Center, Chicago, IL at 7:00 PM, one (1) $50 Visa Pre-Paid Gift Card, and Two (2) Coupons each good for one free DiGiorno;s Pizza. Coupons are redeemable at retail locations only and are subject to terms and conditions thereon. The approximate retail value (ARV) is $594. In the event Potential Winner cannot be contacted or cannot accept the Grand Prize in the manner designated by Sponsor, Grand Prize will be forfeited in its entirety and an alternate Potential Winner may be selected. 4. RANDOM DRAWING. One (1) Grand Prize Winner will be selected in a random drawing on June 21, 2013, at 5:00 p.m. CDT from among all eligible entries received during the Promotional Period. Decisions by Sponsor are final on all matters relating to this Sweepstakes. Odds of winning depend on the number of eligible entries received. Sponsor will attempt to contact and verify potential Winner at the time of the drawing. Potential Winner will be contacted via provided telephone or . Potential winner will have one (1) hour to respond for verification of eligibility status, or prize may be forfeited in Sponsor s reasonable discretion. Sponsor is not responsible for any change in Participants addresses, mailing addresses and/or telephone numbers, transposition of numbers or letters, or other errors that may occur when trying to contact

3 Potential Winner If Potential Winner is unable to accept the Grand Prize or if Sponsor is unable to contact Potential Winner, an alternate Potential Winner will be selected. Winners shall be responsible for all applicable federal, state and/or local taxes and the reporting consequences thereof and for any other fees or costs associated with the applicable prize, including transportation to and from the United Center, souvenirs, food and beverages other than as provided herein, parking and any other expenses not expressly provided for herein. No cash or other substitutions may be made except by Sponsor, who reserves the right to substitute a prize (or component thereof) with one of equal or greater value if the advertised prize is not available for any reason as determined by Sponsor in its sole discretion. Prize is not transferable, assignable or redeemable for cash. Potential Winner is subject to verification, including verification of age and state of residency. If a Potential Winner is found not to be eligible or not in compliance with these Official Rules, the Potential Winner may be disqualified. Potential Winner and Potential Winner s guest may be required to complete, sign and return an Affidavit of Eligibility/Liability Release, and, where lawful, a Publicity Release, or Grand Prize may be forfeited. 5. GENERAL TERMS AND CONDITIONS. By participating in the Sweepstakes, Participant agrees to be bound by these Official Rules and by the decisions of the judges, which shall be final and binding in all respects. The Participants, by entering, agree to release and hold harmless the Sponsor, the Chicago Blackhawk Hockey Team, Inc, the National Hockey League, their franchisees, licensees, distributors and dealers, each of their respective owners, affiliates, subsidiaries, media partners, prize providers, printers or suppliers of Sweepstakes materials and advertising, promotion

4 and other coordinating agencies and service providers involved in this Sweepstakes, including, but not limited to, Wunderman, L.L.C. and all of their respective owners, agents, officers, directors and employees ( Promotional Parties ) from any and all claims, injuries, damages, expenses or losses and/or liabilities of any nature that in any way relate to or arise from Participant s participation in the Sweepstakes or receipt or use of any prize, including, but not limited to: (a) unauthorized human intervention in the Sweepstakes; (b) printing errors; (c) defects on Sweepstakes materials; (d) errors in the administration of the Sweepstakes; (e) injury or damage to persons (including death) or property that may be caused, directly or indirectly, in whole or in part, by Participant s participation in the Sweepstakes or receipt of any prize. ANY ATTEMPT BY ANY PERSON TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS IN VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW, INCLUDING CRIMINAL PROSECUTION. If the Sweepstakes is terminated in whole or in part before the designated end date, Sponsor will select Winners in a random drawing from among all eligible, non-suspect entries received as of the date of the event giving rise to the termination. In no event will Sponsor and/or Promotional Parties be responsible or liable for any damages or losses of any kind, including, but not limited to, direct, incidental, consequential or punitive damages arising out of or in connection with participation in the Sweepstakes. Some jurisdictions may not allow limitations of the exclusions of liability for incidental or consequential damages; therefore, some of the aforementioned limitations or exclusions may not apply, depending on place of residence.

5 6. OTHER CONDITIONS. Sweepstakes entries are automatically void if counterfeited, mutilated, forged, altered or tampered with in any way; if illegible; if mechanically reproduced; if obtained outside authorized legitimate channels; if they contain printing, production, typographical, mechanical or other errors; or if they are rendered inoperable due to any type of malfunction, whether technological or otherwise. Decisions of Sponsor or judging organization are final, binding and conclusive in all matters. ANY PERSON ATTEMPTING TO FORGE SWEEPSTAKES MATERIALS OR TO DEFRAUD SPONSOR IN ANY WAY IN CONNECTION WITH THIS SWEEPSTAKES MAY BE PROSECUTED TO THE FULLEST EXTENT OF THE LAW. No more than the intended number of prizes as outlined in these Official Rules will be awarded in the Sweepstakes. By accepting any prize, Winner consents to the use of his/her name, likeness, voice and/or limited biographical information for advertising and promotional purposes without additional compensation, except where prohibited by law. In the event there is a discrepancy or inconsistency between disclosures and other statements contained in any Sweepstakes materials and the terms and conditions of these Official Rules, these Official Rules shall prevail, govern and control. 7. Subject to applicable law, Participants agree that any and all disputes, claims and causes of action arising out of or connected with the Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in Cook County, Illinois. Any

6 and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, excluding attorney s fees and court costs. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, Participants rights and obligations or the rights and obligations of the Sponsor in connection with the Sweepstakes shall be governed by and construed in accordance with the laws of the state of Illinois, without giving effect to any choice of law or conflict of law rules (whether of the state of Illinois or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the state of Illinois. 9. FORCE MAJEURE. If for any reason the Sweepstakes cannot be conducted as planned by reason of tampering, unauthorized intervention, fraud, errors or technical failures, or any other causes beyond the control of Sponsor which, in the sole opinion of Sponsor, corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, the Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify or suspend this Sweepstakes.
