1 The Hague University of Applied Sciences ODI (occupational disability insurance) Questions and answers What is occupational disability insurance (ODI)? ODI is occupational disability insurance cover specially for The Hague University of Applied Sciences employees. This occupational disability insurance provides for the income of The Hague University of Applied Sciences employees to be supplemented to at least 70% in case of partial disability, with an additional supplement of 10% of their insured income in case of full disability. As The Hague University of Applied Sciences believes it is important that all its employees have this insurance cover, it has arranged a collective (or group) contract with Loyalis, under which your employer pays the entire monthly premium on your behalf. The Hague University of Applied Sciences employees are therefore insured free of charge, in case of partial as well as full disability. So in all cases! For further details of the insurance, go to There is also an English language page: Who benefits from the Loyalis ODI? This insurance provides cover for everyone employed by The Hague University of Applied Sciences, whether on a permanent or temporary contract. On-call workers (with zero hours contracts and min/max contracts) do not qualify for this insurance. I have a temporary contract. Am I eligible? Yes, temporary staff also are covered via their employer, unless you have signed a waiver. If you join this cover and are later offered a permanent contract or switch employers, you will retain your clean acceptance. That means you do not have to answer any medical questions and will be unreservedly accepted.

2 I don t want this cover. What should I do? Only in the event of very compelling, moral grounds not to join the collective insurance arranged through your employer may you sign a waiver. If you decide later that you would like to take out this cover, we will ask you a series of medical questions. I work part time and have a low income. May I join the collective disability insurance cover via The Hague University of Applied Sciences? Yes, you are automatically covered. If you have a low income (less than 19,000 per year) and are a sole earner, you may be entitled under the Supplementary Benefits Act (Toeslagenwet) to have your disability benefit from UWV Employee Insurance Agency supplemented up to the level of the social minimum. The disability insurance cover provided by Loyalis guarantees you retain at least 70% of your income. It therefore acts as a safety net in case of changes to the Supplementary Benefits Act. The cover additionally provides for non-contributory pension accrual in case of disability. If you have a partner with his/her own income, you are not entitled to benefit under the Supplementary Benefits Act (Toeslagenwet). As the occupational disability insurance supplements your insured income to at least 70%, it makes sense to take out this cover also if you have a lower income. What does Loyalis ODI cover cost? Nothing at all. Your employer, The Hague University of Applied Sciences, pays your entire monthly premium for this cover. I already have occupational disability insurance elsewhere. What should I do? You decide yourself whether or not to switch to Loyalis. It is cheaper to arrange cover via The Hague University of Applied Sciences, since your employer pays your entire monthly premium. - If you opt to join the collective arrangement via your employer The Hague University of Applied Sciences, you need do nothing. You are automatically insured, free of charge, in all cases: partial as well as full disability. (You can consider cancelling the other similar cover you have.) - If you decide to remain with your current insurer and not to join the collective arrangement, you should tick the box No, I am not joining on the waiver. This form is provided to you by The Hague University of Applied Sciences and should be filled out and forwarded to The Hague University of Applied Sciences HR Service Center (P&O servicecentrum). The HR Service Center can also answer any questions you may have.

3 I already have occupational disability insurance with Loyalis. What should I do? You don t need to do anything. Your individual policy will be automatically cancelled and your cover will be continued under The Hague University of Applied Sciences collective arrangement. It is cheaper for you to be covered via The Hague University of Applied Sciences in this way, since your employer pays your entire monthly premium. If there is an exclusion on your current individual policy, this will cease to apply, provided that you are not now on sickness absence. Do I have to answer medical questions? No. As an employee of The Hague University of Applied Sciences you are assured of acceptance, regardless of your medical history. You are automatically insured from the moment you commenced employment with The Hague University of Applied Sciences. You don t have to do anything. Only in the exceptional circumstance that you are unable to work due to illness, i.e. on sick leave, on your employment contract commencement date will you not be automatically and fully accepted for this insurance cover until you have been completely back at work for an interrupted period of 4 weeks. Illness causing your absence from work at the moment your employment commenced and that results in disability is not insured. In that case you are not entitled to any benefit. What should I do if I become ill after the insurance has commenced? If you have been off work ill for at least 6 months, you can inform Loyalis of this sickness absence. This must be done within 12 months of the first recorded day of illness. The sickness absence notification form (ziekmeldingsformulier) is available on our website: > Contact & Service. Alternatively, you can phone our Customer Service department: (on business days, between 8 am and 5.30 pm). What impact does the increased state pension age have on the ODI? The Loyalis occupational disability insurance is aligned with the state pension age. This means you are entitled to benefit until your state pension age. At this moment, the state pension age is 67. Payment of benefit and insurance cover therefore extend until you reach this age. We will inform you of the consequences for the premium if the government increases the state pension age further. NB! A state pension age of 65 continues to apply in case of illness reported before , resulting in occupational disability, on your current policy. Is this insurance really necessary, given the existence of the WIA in the Netherlands? Everyone working for an employer who becomes ill and who is registered as at least 35% incapacitated after 2 years illness is indeed entitled to WIA benefit in this country. WIA = Wet werk en inkomen naar arbeidsvermogen (Work and Income (Capacity for Work) Act)). If you become partially incapacitated and have little or no income according to the definition of UWV

4 Employee Insurance Agency, your income will be drastically reduced. After a while you may be left with a (family) income at below welfare level. Occupational disability insurance makes a lot of sense, therefore. If you arrange this cover via your employer s collective contract, you are guaranteed at least 70% of your insured income. And if you become fully incapacitated (80% or more), you are entitled to an additional 10% of your insured income. When do I receive disability benefit? If you are off work ill for longer than 2 years and your employment is terminated or you are reassigned due to disability. During the first 2 years of illness, your employer pays at least 70% of your income (depending on the collective labor agreement). After that you are either reassigned or your contract is terminated. You may still be entitled to WIA benefit or supplementary benefit from your disability pension, but your income will be drastically reduced and is often no longer sufficient to maintain your current standard of living. The Loyalis occupational disability insurance guarantees you additional benefit to at least 70% of your insured income until your state pension age (max. 67) in case of partial disability (35% - 80%). And if you become incapacitated at 80% or more, you are entitled to an additional 10% of your insured income. What is my insured annual income under the Loyalis occupational disability insurance? This is your annual income on January 1 of the year before your contract is terminated due to disability. Let us say your contract is terminated in Your insured income is then your salary for January Do I receive a policy or certificate in my name? The insurance is a collective policy taken out by your employer. That means that The Hague University of Applied Sciences is the policyholder and you, as an employee, are the insured/beneficiary. You therefore do not receive a policy or insurance certificate yourself. Can I transfer this insurance with medical acceptance to another employer? No, that is not possible. The following applies, however: If your new employer also has a purely collective contract with Loyalis, you will be unreservedly accepted from your contract start date. Does your new employer have a semi-collective contract with Loyalis? If so, you do not need to answer any questions if you are insured within six months after commencing employment. After these 6 months you will have to answer a series of medical questions to be accepted for the insurance.

5 Can I prematurely cancel the insurance? You can only indicate that you do not wish to participate (any longer) in the collective insurance arranged through The Hague University of Applied Sciences by completing a waiver. This form is provided to you via The Hague University of Applied Sciences. Does the collective cover also extend to my partner? No. Semi-collective cover without medical acceptance only applies to ODI for The Hague University of Applied Sciences employees and not to people with ODI Partner s Insurance (AOV Partnerverzekering). Partners can take out their own cover with Loyalis, however, provided they are in paid employment. Who do I inform if I do not wish to receive any mail? Your employer will inform you about this Loyalis occupational disability insurance during the promotional period.
